EE was assisting with final seal prep cleaning the bottom with a shovel when a piece of rock fell from the top and struck their hand against the shovel handle, causing a laceration. Injury was not the result of a training or compliance issue. 7000-1 was initially filled out 5/12/25, but the "Submit" button was inadvertently not clicked, thus this late submission.

Employee was pulling pallets of dust off of a supply trailer using a ratchet strap that was connected to a roof bolt plate by a 1/4 inch clevis. Employee was unhooking the strap when they were struck by a piece of loose rock that fell from the roof.

While positioned at the drilling station of the roof bolter, the employee was going to walk toward the back of the machine. As EE placed hand on the bolter tray to assist with stepping up into the bolter walkway, a rock fell from the roof pinching EE's hand between the rock and the tray. This resulted in a laceration to the left forefinger and two fractures to the left hand.

While roof bolting a piece of rock fell from the roof striking the employee on the left forearm. The employee was transported to hospital Philippi, WV where employee was examined and treated for a laceration to the left forearm requiring five sutures. The employee was released to work with no restrictions.

EE was operating a radio remote controlled continuous miner when EE was struck on the top of the head by a piece of roof rock measuring approximately 65 inches long by 21 inches wide by 1 inch thick. Resulted in a fracture to EE's C4 vertebrae.

The injured was struck on the head and shoulders by a piece of roof rock that fell out between roof bolts. The rock measured 4ft. 9 inches long, by 2 feet wide and was up to 8 inches thick.

While roof bolting the injured was drilling a hole into the mine roof when a portion of roof broke around the drill steel. The rotation of the drill caused the rock to spin around the steel. EE stopped the rotation and the rock broke and a portion of it struck EE on the forehead above the left eye. The rock caused a laceration and small portions had to be surgically removed.

The injured was roof bolting when he was struck on the left hand by head coal causing a laceration to the little finger of this hand requiring 3 sutures to close. He was wearing protective metacarpal gloves at the time.

Employee was hanging fly pads and after finishing he took his gloves off for a bathroom break. When he returned to the scoop he was reaching for his gloves when a piece of roof rock fell striking him on top of the right hand causing a laceration that took 10 sutures to close.

Principal placed glue and top end of roof bolt into the drilled bolt hole. As he was placing the head (or bottom) end of the bolt into the drill chuck a piece of rock fell striking his left hand and catching the thumb oh his hand between the rock and the bolt plate resulting in a laceration to his thumb.

The injured employee was cleaning off the miner when a rock fell from the top striking him in the head above the right ear causing a laceration that took 5 staples to close. The employee also struck his right elbow on the machine and x-rays were taken and results were negative.

Individual was struck in the shoulder by a roof rock. As the rock slid down his arm it caused a laceration that required 8 sutures to close the wound.

While roof bolting the face of the #8 entry employee was preparing to drill a hole. The drill steel was placed against the mine roof and a piece of roof rock fell striking the employee in the chin causing a laceration which required 4 stitches to close the laceration.

Individual was relocating fly pads when a rock fell from the mine roof striking him in the head then while the rock was sliding down his back he received a laceration that required four sutures to close the wound

Individual was drilling to shoot an overcast. As he was marking his hole spacing a rock fell from the top striking him in the face.

EE was kneeling down to work on a power center when a rock from the top fell striking EE on the head above the left eye. EE continued to work the rest of his shift and went to the doctor and received 9 stitches to close the wound.

Individual was standing on the walk way of the bolter attempting to pry a loose rock from the roof when the rock fell. It struck the canopy of the bolter and slid inward toward the walk way striking the individual on the left foot.

While operating the roof bolter, the employee was resting his hand on the drill head, when a rock from the roof fell striking his left hand and dislocating his left ring finger and causing a laceration which took sutures to close.

The employee was operating the continuous miner on the number 1 section in the number 8 heading. A rock fell striking the employee in the back. The employee was seen by a physician on 3-16-10 and was placed on light duty.

The employee was roof bolting when a piece of roof rock fell, striking him on the left hand, fracturing the 3rd metacarpil bone and lacerating the index finger.

Employee was operating a roof bolter on the right side. He had completed drilling a hole and was bringing the drill steel out of the hole when a small piece of roof rock fell and struck him on the right side of the upper lip. The wound required 3 sutures to close. He returned to work on his next scheduled shift without restrictions.

The injured was operating a radio remote controlled continuous miner when a rock fell inby the last row of roof bolts, it rode through the last row in a manner that it struck the injured as he truned to get back, resulting in an injury to his neck and a fracture to his left ankle.

Employee was operating a Fletcher Roof Ranger on the left side. Upon completion of drilling he was in the process of removing the drill steels from the hole when a small rock fell from the roof and struck him atop the left hand causing a fracture to the metacarpal bone.

Injured was struck by a falling rock, measuring 42 iches by 16 inches by 1 inch, while dismantling a "kennedy" stopping.

Employee was roof bolting. As he began to drill a piece of rock fell from the area of the drill steel and struck the canopy then struck the employee on the right shoulder.

Employee was platering a stopping. He stopped to get a drink from his dinner bucket. When he returned to the work site to complete the plastering of the stopping a rock approx. 18" long x 10" wide x 6" thick fell from the 8' high mine roof and struck him on the head and neck.

Employee was tramming a Joy 14cm-15 radio remote continuous miner into the face of #6 entry. He stopped the miner and bent over to handle the miner cable when a rock approx. 19'w x 30"l x 1/2" to 2 3/4" thick fell from between two roof bolts and struck him on top of the head, which was protected by a hard hat.

Employee was cleaning the #8 entry with scoop. He got out of the scoop to roll up a line curtain when a rock appr. 2' by 2" fell from between the right rib and the 1st bolt on the right side, striking him atop the right foot, causing a fracture to small toe and next toe.

While operating the roof bolter, a rock, measuring 1 foot by 1 foot by 4 inches, fell from the mine roof, striking his rt hand which was resting on the drill steel tray.

Employee and co-worker were beginning to bolt first cut out of crosscut. Employee was on helper side of roof bolter. ATRS was set lightly against the roof. Injured asked operator to back bolter up 2-3 inches. When he did a rock fell from the roof and struck the employee on the upper back and knocked him down causing him to strike his left knee against the mine floor.

Employee was in an upright position operating a radio remote controlled continuous miner, when a rock fell from between two roof bolts and the right rib, striking him on the left thigh causing a contusion. Employee had previously visually inspected the roof. A ventilation line curtain blocked his view of the rock dislodging and falling.

Employee was rock dusting behind seal at return shaft a rock 1"thick by 14" fell out, hitting EE on back of head. Mild concussions.

Injured was installing 5'W X 15'L with 4" opening wire screen against the mine roof, with a roof bolter, when a small piece of shale fell from the roof and struck him above the right eye causing a laceration requiring 9 sutures to close. Injured was released to normal duties beginning 11/14/2006.

Employee was in an upright position roof bolting. He had completed drilling the rib side hole in the roof and was reaching back to get his roof bolt when a piece of rock measuring 25"LX18'WX2"T fell from the roof and struck his right forearm. X rays negative. Employee later fainted making it an emergency.

Injured was installing roof support over the slope belt when a rock, 8" by 10" by 1" thick fell striking him on the right ear causing a laceration which required 3 sutures to close.

The injured employee was drilling a hole in the mine roof when a small/med piece of rock fell next to the rib, striking the injured on top of right shoulder & back of right leg.

While installing roof bolt, loose top started falling, injured started to move out of the way and tripped on a rock striking his elbow on the bottom.

The injured was installing a roof bolt into a drilled hole when bolt being installed slid against edge of hole causing small piece of slate to break away from mine roof striking injured on upper right lip. Injured returned to work same shift.

Injured was drilling his outside bolt hole at the end of x-cut 2-1 @ entry #1. The area had fell out 5 to 6' above the coal and a cutter had ran from the corner where he was drilling across #1 and outby. He examined the area prior to drilling but while drilling loose rock fell striking his hard hat, knocking it off and the next rock struck the back of his head on the right side

While starting his drill steel, a rock fell from the mine roof striking his left hand. The rock shoved his hand down against the drill head. His hand was flexed backward as it bent down past the side of the drill head. He had sound checked the area of the roof prior to drilling. Injured was wearing metacarpal gloves and forearm guards. Begin missing work until 11/15/05.

The injured was securing wire screen to existing roof bolts on the 6 Miner track to allow it to be installed with supplemental roof support. As he was securing the screen a rock measuring 30" x 28" x 4' fell from the roof onto the screen striking his left small finger causing a fracture. The area had been scaled down immediately prior to this

Employee was in a seated position operating a radio remote controlled continuous miner when a small piece of rock fell from the roof & rib and struck him on the back of his shoulder.

Employee was roof bolting #5 Entry. He was on the "off side" of the roof bolter in a squat position with the left knee on the mine floor and the right leg posed in a 90" position. While beginning to drill the first hole a piece of rock being drilled fell from the roof, broke against the canopy, and struck the right knee.

While operating the Joy continuous miner on the 2 left section, in x-cut #3 & #2 entries, a piece of rock 36" x 28" x 10" thick dislodged from the mine roof between the rib and rib bolts. The rock struck the employee on the right side of the head, middle and lower back and right foot. This rock had slick surfaces

EE backed miner out of face of #4 entry, shut off machine and walked towards a roof bolter parked in x cut #4 x #3 when rock dislodged from borw at edge of x cut of #4 entry. Rock struck EE on the neck area and upper back.

Injured was struck on left foot by falling roof rock when he swung the head in to install the inside bolt on pattern.

Injured was struck by falling rock from between the bolts. Bolt spacing was with requirements of plan

Injured was opening door for scoop to go through when a piece of rock fell striking him on left shoulder causing a bruise.
