While attempting to install a roof bolt in the face of the #3 entry at 135 block on the 1 North Mains Section, a piece of rock fell from the top striking the employee's right arm resulting in a fracture.
Injured employee was rib bolting on the 9 Right section when a piece of material 4" wide x 3" long x 2" thick fell from the top, glanced off the ventilation tubing and struck the left side of his face resulting in a broken tooth. Employee received dental treatment.
Employee stated that he was operating the continuous miner in the face of the #5 entry. He had taken the 1st sump in the cut sequence and had positioned the off standard continuous miner for the second lift when a section of draw rock fell from the top between the roof bolts and the rib, striking him in the back of the neck and left shoulder blade area.
Injured employee was roof bolting on an overcast ramp when a piece of rock fell from the top and struck the 4-hole strap to be installed causing the injured employee to fall into the roof bolting machine resulting in two fractures to the small bones of a vertebrae.
Injured employee was roof bolting on the 1 North Mains section when a piece of rock fell from the top and struck his right arm resulting in a bruise. Employee was initially released to return to work by Ruby Memorial Hospital. Further investigation determined that the employee is being kept off work by a physician in southern West Virginia.
Injured employee was installing rib bolts on the 7 Right section. As he repositioned the bolter after installing a bolt a piece of rock measuring 10-12" long by 3" thick fell from the roof and struck his upper back resulting in a strained and bruised back.
Injured employee was roof bolting on the right side of the dual head bolting machine and attempting to drill the inside bolt hole. The area had potted out. As he was drilling the bolt hole a piece of slate fell out of the top striking the employee's right collar bone and shoulder.
Employee was operating the roof bolting machine and was in the process of moving his drill steel from the bolt tray to the drill pot, when a small piece of rock fell and struck his forearm. He was diagnosed with a contusion and soft tissue damage to his arm and placed off of work until July 7 or further examination.
Bolter opr & co-worker were installing 6' bolts. Top was uneven w/sandstone. The oprs reduced spacing of straps from 2' to 20". They were scaling loose rock & material prior to bolting. Rocks & sandstone would not pry down. EE stated when pushing bolt w/wrench, simultaneously w/other opr, pushing bolt w/plate into strap when rotating bolt a piece of rock fell 5', struck rt hand.
Employee stated a piece of head coal fell and struck his head and neck while he was bolting on 1 North Mains. The coal measured 18" long x 4" thick x 5" wide.
Injured employee stated that while making up a bolt to be installed a rock measuring 18" wide X 20" long X 4" thick fell from the top and struck his right arm resulting in a fracture.
Injured employee had pulled in #158 Shield to hold adverse top. Employee had his left hand placed on the down pressure hose coming off #159 shield when a piece of rock fell from the top, striking the shearer and then striking his left hand resulting in a bruise.
Employee stated while hanging curtain, he was struck by a falling rock, striking his left knee causing a contusion. Employee began missing work on 1/25/2012.
Employee stated that while setting shields on the 14 Left longwall face at 100 shield a piece of rock fell from the top and struck his left ankle.
Employee was standing with his left hand resting on a rib bolt, a rock fell striking his left pinky finger causing a fracture and a laceration which required sutures.
Employee was struck by rock that fell from the mine roof while placing the wash down hose on the continuous miner. The rock measured 3' wide by 4' long and up to 3" thick. Employee was seen on 8/23/2011 by a neurologist at which time he was diagnosed with a back strain and given 5 days away from work.
Employee was struck on the left side of his face by a rock that fell from the mine roof causing a laceration above and below the left eye. Sutures were required to close the wound.
Employee stated he had just installed a bolt when the top cracked and a piece of material fell striking him on the back and back of the left leg causing a sprain.
Employee was hanging a reflector to mark an unbolted intersection. He was standing between the third and fourth row of bolts back from the intersection and a rock fell from between the bolts striking him in the head and back. The rock measured 35 inches wide by 80 inches long and was approximately 2 inches thick. He was diagnosed with two compression fractures in the lower back.
Employee stated he placed his left hand on the continous miner on the 6 right section and was struck by a piece of rock that had fallen from the top approx. 12" x 12" x 1.5", resulting in a fracture of the left little finger.
Employment stated that while walking to 10 Left bleeders at 25 block he bent over as a result of an area of lower top and a piece of slate fell from the top and struck his shoulder. This caused the injured employee to fall to the mine floor and strike his hand on a piece of rock causing a laceration requiring 15 sutures to close.
The employee stated he was bolting top and had let the head down and was reaching for the wrench when a small piece of rock fell from the roof and struck his little finger.
Employee stated while roof bolting he was struck by a piece of shale.
The employee stated he was bolting the rib when a piece of coal 24" x 24" x 2" thick fell and struck him on the shoulder. The employee did not go to the doctor until 1/7/09 when he started missing work.
The employee stated he was operating the continuous miner when a piece of rock fell from the roof approximately 3' wide x 9' long x 1/2" to 5" thick and struck him on the right side of body.
The employee stated he was bolting top and had started to install the right side outside bolt when a piece of coal popped off the rib and struck him in the back and knocked him to the ground.
The employee stated he was pumping a shield against the roof on the longwall when a piece of rock fell from the top and struck him on the back of the leg. The employee went to the doctor and was misdiagnosed. We are reporting this injury at this time but it is still under consideration. The employee did not start missing work until 9/5/06.
THE EMPLOYEE STATED HE WAS MOVING A ROLL OF 3" PLASTIC WATER LINE OUT OF THE ROADWAY WHEN IT GOT CAUGHT ON THE ROOF. HE LOOKED UP TO SEE WHAT THE LINE HIT AND A PIECE OF ROCK DISLODGED AND STRUCK HIM ON THE NOSE. THE ROCK WAS 7"X9"X1". HE SUSTAINED A SCRATCH ON HIS NOSE. THIS IS BEING REPORTED TO MSHA TO ABATE CITATION 7244154 WHICH THE OPERATOR INTENDS TO CONTEST
The ee stated he had laid his hand on the end of the atrs boom when a piece of rock fell from the roof and struck his left hand. the roof was approx. nine feet high and the rock fell 4 1/2" which it struck his hand.
THE EMPLOYEE STATED HE WAS RIDING IN NO. 4 MANTRIP WHEN A PIECE OF ROCK FELL FROM THE MINE ROOF AND STRUCK HIM ON THE TOP OF THE HEAD AND THEN GLANCED OFF AND STRUCK HIS LEFT HAND.
THE EE STATED HE WAS HELPING CHANGE A RANGING ARM JACK WHEN A PIECE OF MINE ROOF FELL AND STRUCK HIS BACK. HE WAS STANDING UNDER SHIELD 10 AND THE ROOF WAS APPROX. 75" L X 19" W X 2-6" THICK. THE EE DID NOT MISS WORK UNTIL 2-17-03.
EE STATED THAT HE WAS PICKING UP FLACK JACKETS FROM THE TOP OF THE CRUSHER WHEN APIECE OF ROCK MEASURING 27" X 13" X 2" FELL FROM BETWEEN ROOF BOLTS AND STRUCK HIS LEFT HAND CAUSING A LACERATI ON TO THE LEFT INDEX FINGER. SIX SUTURES WERE REQUIRED TO CLOSE THE LACERATION.
EE STATED 9-16-02 HE WAS SCALING TOP, AND A PIECE FELL BEHIND HIM, AND HIT THE TOP OF HIS HARD HAT.MEDICAL RECEIVED 9-23-02 SHOWS NO ACUTE INJURY, HOWEVER IMPRESSIONS TAKEN SHOWS OSTEOPHYTES ( SPURRING) AND SCLEROSIS OF THE CERVICAL SPINE.ALSO OSTEOPHYTIC LIPPING OF THE THORACIC SPINE.EE HAS PAST HISTORY AND MEDICAL ON HEAD AND NECK.
TWO EMPLOYEES WERE TAKING A GUARD OFF THE CONTINUOUS MINER WHEN A PIECE OF ROCK FELL OUT FROM BETWEEN THE HEADERS AND HIT ONE EMPLOYEE ON THE HEAD, SLID OFF AND HIT HIS RIGHT FOREARM. THE ROCK MEASURED 22" LONG, 15" WIDE, 3/4" THICK. THE INJURY TO THE EMPLOYEE RESULTED IN A LACERATION TO THE RIGHT FOREARM WHICH REQUIRED SUTURES TO CLOSE.
EE STATED HE WAS EXAMINING AREA PRIOR TO MINING WHEN A PIECE OF ROCK FELL FROM THE MINE ROOF STRIKING HIM ON THE HEAD AND RIGHT SIDE. HE COMPLAINED OF A HEADACHED AND NUMBNESS IN RIGHT LEG.
EE STATED HE WAS LOADING SUPPLIES ON THE BACK OF THE ROOF BOLTER, & A PIECE OF ROCK FELL STRIKING HIM ON THE NOSE & BOTH ARMS.
THE INJURED WAS SWITCHING OUT #72 BUS IN THE MAIN TRIP SPUR IN THE ROCK DUST HOLE ON THE BOTTOM OAT THE ELECTRIC SWITCH. HE WAS TRAMMING #72 BUSINTO THE SPUR AS HE LOOKED UP AT THE WIRE AND TH EN HE WAS STRUCK ON THE BACK OF HIS HEAD BY A FALLING ROCK. HE WAS TALKING OUTSIDE WHEN INVESTIGATED THE #72 BUS HAD ALREADY LEFT THE ROCK DUST HOLE. WE CHECKED THE AREA WE FOUND A PIECE OF R
THE EE STATED THAT HE WAS WORKING ON A BOOM JACKON A CONTINUOUS MINER IN #2 ENTRY WHEN A KETTLE BOTTOM MEASURING 36"LONG, 24"WIDE, AND 14"THICK FELL FROM BETWEEN THE HEADER BOARDS HITTING HIM ON THE OUTSIDE OF THE RIGHT FOOT.
EE WAS WALKING FROM OUTBY THE PILLAR LINE IN THE #1 ROOM & POSITIONED HIMSELF ALONG THE LEFT & SOLID RIB TO CLEAR A COAL HAULER. A PIECE OF COAL RIB BROKE LOOSE & STRUCK HIS LEFT LOWER LEG.
EMPLOYEE CLAIMS HE WENT THROUGH A LINE CURTAIN TO GET A BOTTLE OF DRINKING WATER OFF THE #4 MRS WHEN A PIECE OF MINE ROOF FELL AND STRUCK HIM ON THE BACK AND SHOULDER. MEDICAL RECEIVED 12/14/0 1 INDICATES NEGATIVE FINDINGS ON FIVE VIEWS OF THE LUMBAR WITH DEGENERATIVE DISC DUFFISELY. THE EMPLOYEE HAS A MEDICAL HISTORY ON BACK AND SHOULDER IN FOUR OTHER CASES. NO MEASUREMENT FOR ROC
EE WAS OBSERVING CREW ROOF BOLTING THE LONGWALL FACE TO PREPARE FOR A MOVE. HE PLACED HIS RIGHT HAND ON A SAFETY JACK WHEN A PIECE OF ROCK FELL FROM THE ROOF STRIKING HIS MIDDLE & RING FINGER.
EMPLOYEE STATED HE WAS BENDING OVER TO PICK UP A CABLE CROSSER WHEN A ROCK/COAL BROW FELL AND STRUCK HIM ON THE RIGHT LOWER LEG AND ANKLE. MEDICAL IMPRESSIONS SHOW LOWER LEG, ANKLE AND FOOT A RE NEGATIVE WITH NO FRACTURES OR DISLOCATIONS. HOWEVER HIS RIGHT FOOT WAS BRUISED AND THE RIGHT ANKLE SPRAINED.
EE WALKED OVER & AROUND THE CONTINUOUS MINER PARKED JUST OUTBY THE FACE OF #5 ENTRY. HE LOCATED HIS PERSON BETWEEN THE RIB & THE MACHINE. WHILE KNEELING, A ROCK FELL FROM THE ROOF NEAR THE RIB LINE & STRUCK HIS RIGHT LEG.
WHILE EMPLOYEE WAS OPERATING CONTINUOUS MINER, APIECE OF ROCK (19 1/2 X 22" WITH ONE END SHARPENED) FELL FROM BETWEEN THE ROOF BOLTS OF THE MINEROOF. IT STRUCK HIS LEFT HAND WITH WHICH HE WAS USING TO OPERATE THE REMOTE CONTROL. THE ROCK CAUSED AN ABRASION TO THE TOP OF HIS HAND & THE 2ND, 3RD, & 5TH FINGER. THE FACE AREA WAS ABOUT 8' HIGH WHERE THE ROCK FELL.
EMPLOYEE STATED THAT HE JUST RETURNED FROM LUNCHAND WAS ASSISTING THE ROOF BOLT CREW BY PREPARING BOLTS. WHILE MAKING UP BOLTS ON THE OPERATOR'SSIDE OF THE ROOF BOLTER TOWARD THE CONTROLS ON T HE FRONT, A PIECE OF OF ROCK APPROX 1" THICK 27" BY 52" FELL AND STRUCK HIM ON THE HARD HAT SLIDING DOWN HIS RIGHT ARM AND STRIKING HIS RINGFINGER ON THE RIGHT HAND. HE WAS WEARING GLOVES.
