Federal No 2 Coal

While placing a crib block under a section of panline, the employee caught EE's middle finger of EE's left hand between the crib and the section of pan. Employee completed EE's shift. EE went to the doctor the next day and was diagnosed with a fracture. EE continues to work alternate duty.

While watching a fellow employee troubleshoot a scoop electrical problem, an arc flash occurred. This caused a flash burn to the employee's eyes. Employee was brought outside and transported to the hospital. Was treated and released. Given antibiotic ointment by eye doctor.

Employee was working on a battery scoop. EE disconnected one lead of the dual lead batteries. As ee disassembled the battery plug, ee apparently made contact with both leads. This caused an arc flash that caused burns on hand. Employee was transported to the hospital where ee was treated and released.

Employee was assisting on the longwall move. Employee attempted to lift a 4" I beam. EE felt a "pop" in left groin. Employee finished shift and worked the next day. EE felt increasing soreness in the affected area and went to the doctor. EE was diagnosed with a hernia.

Employee was observing motormen attempting to put a derailed car back on track with inflatable lift bags. While inflating the bags, the middle bag in a stack of 3, flew out and struck the employee. Employee was knocked backward and landed on backside on the mine rail. Employee finished shift and worked the next day. Went to the doctor on the second day.

At 10:30 pm on October 23rd, a roof fall occurred at the 1 North sump. The fall measured 60 feet long, entry width wide, and 8 to 10 feet high. The fall did not impede travel of miners or disrupt ventilation.

Employee was pumping "glue" into the mine roof near the 11 Rt longwall face in the belt entry. As EE pumped the glue into the roof, a slab of rock/headcoal about 3'x4'x1' thick fell out between existing roof straps. The falling rock struck EE across back. EE stated that EE felt a pulling sensation in back. EE was brought out and transported to the hospital.

Employee was walking along the longwall stageloader when they stepped on a hydraulic hose valve. Their foot apparently activated the valve & the loose end of the hose abruptly snapped back & struck them in the face. Employee finished their shift. The employee visited a hospital emergency room the next day. Employee stated that the medication they were placed on made them ill.

Employee was installing supplemental roof bolts with a hand held roof drill. The drill steel caught, causing the drill to twist in grip. The twisting action caused a strain/sprain injury to right hand and arm. The employee worked shift on 10/05/16 but started missing on 10/06/16.

Employee was cutting a cable bolt out over the longwall headgate to gain clearance for the stageloader to advance. When EE completed cutting the bolt off, a rock about 18" x 18" x 3-4" thick dropped out of the roof. The rock slid down previously installed roof screen and struck EE on the right foot.

Employee was adjusting a sensor on the belt. EE was sitting on the bottom rail of the belt guard. The rail broke causing EE to fall backwards. EE put left hand out to catch self resulting in an injury to left thumb. Employee finished the shift and has continued to work since. On the 21st of September, x-rays revealed a fracture to the thumb.

Employee stated that while walking along the longwall face, employee tripped on a relay bar. Employee attempted to break fall by reaching with right arm. Impact caused a strain to shoulder.

Employee stated that they bumped their head on a low shield as they walked along the longwall face. Employee stated that their neck was sore.

Note: Employee reported this incident on 8/17/16 but did not start missing work until 8/27/16. Employee stated that they bumped their head on a longwall shield while walking along the face.

Employee was attempting to remove a piece of belting from underneath a belt tail roller with a set of come-a-longs. As EE tightened the come-a-long, the belt dog slipped. The sudden release of tension caused right arm to move suddenly. EE reports feeling a "pull" in the shoulder.

Employee was operating a portable winch to repair 1 East Belt. As employee tensioned the belt, one end of the winch raised up. When employee released the tension, the winch set down on employee's foot. Employee was transported to the hospital where x-rays revealed a fracture of one of the bones in employee's second toe.

Employee was attempting to get a portable generator from a storage area. As employee pulled the generator around a corner toward the entrance, employee heard and felt a "pop" in employee's knee. Employee finished employee's shift but the knee became increasingly tender.

While center bolting, roof bolt bent just above drill head striking employee in the left wrist.

Employee felt pain in employee's back while hanging high line.

When surprised by a sudden shift in the belt structure, employee stated that employee dove into a crosscut in fear of being struck injuring employee's rt shoulder.

While moving a box of drinking water from a skid to a supply car, employee stated that employee twisted and felt pain in employee's lower back.

Employee was walking to the headgate along the longwall face when employee stepped in a crack in the mine floor twisting employee's rt. knee.

Employee was working on 12 Right belt. EE was waiting beside the belt for the belt to be shut off and tagged out. When the belt was stopped, a connector broke on the belt rail and allow the rail to snap sideways striking employee on the hand. Employee suffered a contusion to the hand. EE continued working until 4/29/2016 when doctor advised EE to rest the hand.

The hoisting facility at A shaft was inoperable, due to an electrical problem. There were no miners on the cage while it was down.

While attempting to remove dollies from mono-rail, employee's left thumb was pinched by wheel of dolly resulting in loss of tip of thumb.

Dozer operator was shoving coal into a reclaim feeder. Employee's dozer slid into a cavity. The dozer slid toward the feeder with the blade up. The operator notified the plant operator. A dozer and a backhoe removed enough coal for the operator to exit the dozer. Employee was in the dozer for around 70 minutes. The event was phoned into both WVMHS&T and MSHA.

Employee was attempting to change a fitting on a ram jack on a shield and when the staple was pulled, the hose blew out striking employee in the face.

Employee stated that while working around transfer area, fell striking right knee on 6 inch pipe.

A local truck driver was delivering a box of parts to the mine site. As the employee was lifting the box onto the shelf, employee felt a pain in employee's left elbow. The employee sought treatment on 2/8/16 and 2/15/16 and was told to take OTC ibuprofen and ice/elevate the area. On 2/22/16, the employee was prescribed a steroid and PT, categorizing the injury as reportable.

Employee was walking along the east haulage near bottom area stepped on uneven bottom and fell to the ground landing on right knee.

Employee was struck by a rock on the right hand that came off the tail drum of the shearer. This caused a fracture to a finger and the wound required sutures to be closed.

Employee was hanging an eight inch vent tube to ventilate a place when the tube dropped and struck employee's mouth resulting in a chipped tooth that required a medical repair.

While bolting on the Sandvik miner on the 1 left section, an offset in the rib rolled out, pushing employee down and employee caught self with employee's right arm, causing pain in employee's right shoulder.

While splicing the 11 right belt, a self tightening dog was used to pull slack out of the take up. After the splice was completed the take up was tightened before the dog was removed causing the dog to come off and strike the employee in the right arm.

Employee was standing in front of a shield facing the L/W face when the shield behind employee advanced. Both feet were pinned between the relay bar and each toe of the shield. Employee suffered two broken bones and the wounds required sutures to be closed.

Employee was removing barrel and parts from #2 raw coal pump when employee struck employee's head against a discharge pipe causing pain in employee's neck. Employee finished the shift.

Employee took gloves off momentarily to get a rag out of employee's pocket, placed right hand on the miner when a piece of rock fell striking employee's right hand resulting in a laceration. The wound required 11 sutures to be closed, the employee returned to work.

The dumping mechanism on the skip was damaged when the linkage used to dump the skip failed causing further damage, this resulted in the hoist being inoperable for more than 30 minutes.

As B shaft skip hoist was leaving the pocket, damage occurred resulting in the hoist being inoperable for more than 30 minutes.

Employee was walking and stepped over an edge of a flat bill scoop bucket and rolled ankle as he stepped to the mine floor. Received a sprain to the right ankle.

Injured employee was attempting to install a new gear box on a scoop on the 11 Right section. While using a chain hoist to lift the new gear box in place the hook came loose and struck the employee at the right eye area of his face. Employee was wearing safety glasses at the time of the incident.

Injured employee stated that while carrying a screen panel down a set of stairs on the third floor of the preparation plant he twisted his left knee.

While attempting to unclog the suction on a single head roof bolter on 1 Left, injured employee caught his left middle finger in the dust box door resulting in an avulsion to the tip of his finger.

Employee stated that while moving the rail bender while installing track at #27 spur on the 11 Right section he felt pain in his lower back.

Employee stated that while reaching to pick up a bundle of plates off of a pallet in the supply yard he felt sharp pain in his right arm.(Bicep)

A roof fall has occurred on the 1 left section off the 1 North mains, in the #1 entry in the intersection outby the face. The roof fell approximately 7 feet high, the fall was approximately the width of the entry (16 feet) and approximately 20 feet long.

While hanging vent. tubes, employee tripped on a rock injuring his right ankle.

Employee was sitting in passenger compartment of CO #51 mantrip which was located at the mouth of the hole waiting for clearance. While the mantrip was sitting still, it was struck from behind by CO #84 mantrip. Employee stated that this jarred him causing pain to his lower left back. The seat the employee was sitting in had no back to it.

Employee stated that while shoveling on the East #1 tail he twisted his body and felt pain in his lower back.

Employee stated that while loosening bolts on spiral pump motor he felt pain in his left shoulder. Employee finished the shift and did not notify his supervisor until two days later.

Employee sustained a laceration to his left index finger while attempting to couple two locomotives.

Employee stated that while walking through a check curtain he caught his foot on a hole in the curtain causing him to fall. While falling he extended his arms to brace his fall, this resulted in a fracture to one of the small bones in his right wrist.

Employee stated that while hanging roof bolter cable on 1 North Mains he felt pain in his right shoulder.

Employee stated that while removing a top roller and structure from the longwall belt he felt pain in his left elbow. Employee continued to work and did not begin to miss work until 9/18/2015 when he was diagnosed with a sprain/strain type injury to his elbow.

Williamstown contractor stated that while assisting other miners in placing a locomotive back on the track rails he sprained his left knee while using a lifting jack.

While attempting to install a roof bolt in the face of the #3 entry at 135 block on the 1 North Mains Section, a piece of rock fell from the top striking the employee's right arm resulting in a fracture.

Employee stated that while attempting to uncouple mine supply cars on the mine bottom he felt a pain in his left knee. Employee was diagnosed with a sprain/strain type injury.

A transformer caught fire on a bank of transformers along the mine access road just prior to the 6 Left Shaft hallow at the surface area of the Federal No. 2 Mine. The transformer fed power to a sedimont pond pump. The fire at all times was contained to inside of the transformer and allowed to burn itself out.

Injured employee stated that he felt pain in his left shoulder while hanging continuous miner cable and waterline on the 1 North Mains section.

Employee stated that while moving a bag of rock dust from a stack of bags to an air duster he felt pain in his groin area.

Employee stated that he slipped while attempting to level the monorail along the 10 Right belt striking the belt structure resulting in a bruised right knee.

Injured employee stated that while removing and carrying a belt stand along the 10 Left conveyor belt he slipped in wet rock dust causing him to stumble. He did not fall but this action caused him to feel pain in his lower back. He finished the shift but was later diagnosed with a strain to his lower back.

Employee stated that while carrying a bottom belt roller on his right shoulder he felt pain in his right shoulder and neck when his foot slipped in some mud.

Employee was struck in the mouth by a socket from an air impact gun resulting in a laceration to his upper lip and damage to his teeth.

Employee had hand placed on supply car when a crib was accidently thrown on it causing a laceration to his right hand middle finger, the laceration required 8 sutures to close.

Employee stated that he and fellow employees were attempting to drag the cable of the continuous miner to the power center by hand. While pulling, the cable took slack causing the injured employee to stumble forward and twist his right knee, resulting in a sprain/strain type injury.

Employee stated that while attempting to dismount the mini-trac at the belt trench for the 11 Left section, he struck his head on the canopy of the machine causing pain in the upper right side of his neck.

Injured employee sustained a laceration to his left thumb when it was struck by a piece of drill steel that fell from the mine roof while he was roof bolting. The wound required 11 sutures to close.

Employee stated that while lifting a set of overcast steps to unload from a rail mounted vehicle he felt pain in the lower left portion of his back. Employee was diagnosed with a strained back.

Employee stated that while lowering trolley wire during recovery on the 10 Right longwall section he felt a pain in his left shoulder. Employee was diagnosed with a sprain/strain type injury to his shoulder.

Employee stated that while tramming the continuous miner out of a place on the 1 North Mains section the boom of the miner contacted the rib. After shutting off the power on the machine, injured employee rested his hand on the boom to check the cable when a piece of the rib rolled off and struck his right thumb causing a fracture.

Employee was operating the shearer on the longwall face when his eyes became irritated by dust particles. Employee was wearing approved safety glasses at the time of the incident.

Employee has been diagnosed with repetitive motion type illness to his right shoulder.

Employee stated that he was unloading bags of rock dust from a supply car to be placed at an air duster along the 1 North haulage when he felt pain in his lower back.

Employee stated that while lifting a jack for a wire drill to load it into a rail mounted vehicle the jack dropped causing the jack support to strike the employee on the left jaw chipping a tooth in the right side of his mouth. The tooth was repaired on 4/14/2015.

Injured employee was standing next to a shield adjustment hose on the longwall recovery face. While the shield was in the process of lowering to pull, a return hose ruptured and sprayed the employee on the right side of his head and face resulting in a corneal abrasion. Safety glasses were being worn at the time of the incident.

Employee stated that while attempting to lift a roof bolter cable in order to re-hang it he sustained a strained left shoulder.

Employee was cutting and removing the rock box off of the 1 south #2 tailpiece. Employee slipped and fell off of the tailpiece striking his right on the ground.

While lifting a vent tube. Strain muscle in chest. Employee complained of pain in chest area to supervisor immediately following task. Employee worked the remainder of the shift and the following shift.

Employee stated that while attempting to lift an aluminum pan pusher into a scoop bucket he felt pain in his lower back.

Injured employee was attempting to drag a ventilation tube measuring 18" in diameter by 10' long to the face when he stepped on a lump of coal and twisted his right ankle.

Employee was walking along the rib line on the long wall headgate area, the rib rolled out and pinned him against the corner of the head drive.

Williamstown contractor employee sustained a laceration to his right upper thigh while using a hand held grinder to cut a section of the impact bed of the 1 North #2 belt when the grinder kicked back and struck his leg. Sutures were required to close the wound.

Injured employee sustained a hernia while lifting a bottom belt roller on the 1 North Mains conveyor belt.

Injured employee was attempting to release a chain hoist after completing a belt splice. The employee left his left forth finger on the chain and as the hoist suddenly released his finger was caught between the chain and the belt resulting in a fracture.

crew was pulling rail, employee was using a slate bar to pry the moving rail away from the existing rails when the slate bar came back and struck the employee in the face.

Employee was cleaning air line to free it of ice and water. While attempting to reconnect the 2" line he dropped it causing the valve to open and the line to strike his left arm resulting in a bruise.

While rib bolting on the miner on the 10 right section, the employee pinched his left hand finger between two drill steels while adding an additional steel to finish drilling his rib bolt hole.

Injured employee sustained a laceration to his right middle finger. While attempting to hang the loading machine cable on a rib strap he tripped and fell, while falling he grabbed the rib strap resulting in the laceration. Sutures were required to close the wound.

An accident occurred at the Preparation Plant involving a dozer approaching a feeder on the coal stockpile.

Employee stated that while walking from the personnel hoist through the waiting room to the personnel carriers on the main line track he struck his head on the gunited mine roof causing a strain to his neck.

While employee was pulling a joint of rock dust hose through a man door he slipped and fell injuring his lower back and right side.

Injured employee sustained a laceration to his left middle finger while assisting in moving the 10 Right section conveyor belt. While attempting to position a bottom roller he placed his hand on a bed rail. A fellow employee was attempting to bend the tab on the bed rail and inadvertently struck the injured's hand with a hammer.

Employee was lifting a box and felt pain in their back.

Employee stated that while throwing trash bags through a man door in a stopping, he felt pain in his left shoulder.

Employee pressurized the pneumatic drill rig and a fitting came apart, striking him in the chin.

At approximately 4:25 PM the head gate shearer operator on the 9 Right longwall located at 102 shield noticed a possible ignition (circular arc of sparks or flame) at the face lasting approximately 15 to 30 seconds.

Injured employee stated that while riding out of the mine on the A Shaft elevator at the completion of his shift the elevator knocked power causing an abrupt stop resulting in pain to the lower portion of his back.

Injured employee sustained a laceration to his left hand while cutting metal bands from a cube of block. Three sutures were required to close the wound.

Injured employee stated that while dismounting the company number 71 jeep at 107 block of the 1 North Mains he stepped in a hole and felt pain in his right hip and back.

After connecting a move car in the 5 Right spur, employee sustained a sprained knee when he tripped over a crib block placed along the track to level the track.

Injured employee struck his right knee on a cover for an electrical box on the mini-trac while walking around the machine in the right return entry of the 1 North Mains section resulting in a bruise.

Employee stated that he slipped from the edge of a scoop bucket while supplying the continuous miner with roof bolt material and twisted his left ankle. Employee was diagnosed with a sprained ankle.

An unplanned fall of the mine roof blocking the passage of people occurred on the bottom track haulage between the rock dust hole switch and the south end spur switch. The fall measured approximately 20' wide x 4-5' high, rib to rib.

Injured employee was lifting a loading machine cable to place it on the gathering pan when his right hand contacted a bad spot in the cable resulting in an electrical shock. Employee finished the shift following the incident.

Injured employee was operating a shuttle car from the #4 entry to the feeder in the #2 entry. While tramming off a bridge placed on the mine bottom, the shuttle car struck the mine bottom causing a jarring of the employees lower back.

Injured employee sustained a hernia while adjusting the forks on a forklift.

Injured employee was unloading a power converter off of #68 bus when he strained his lower back.

Injured employee was walking in the #2 entry of 7 Right outby the power center when he tripped and fell, fracturing his right leg.

Injured employee stated that while operating a shuttle car, he struck a hole in the roadway causing pain in his back. Later in the shift while attempting to lift a miner cable he felt increased pain in his back. Employee was diagnosed with a strained lower back.

Injured employee was rib bolting on the 9 Right section when a piece of material 4" wide x 3" long x 2" thick fell from the top, glanced off the ventilation tubing and struck the left side of his face resulting in a broken tooth. Employee received dental treatment.

Individual was carrying cable bolting supplies along the 8 right belt at 5 block when the inby corner rolled out and struck his upper chest and right side of his head. The individual allegedly lost consciousness momentarily. The incident was immediately reported to MSHA and WVOMHST.

An unplanned fall of the mine roof above the anchorage horizon occurred at 12 block, #2 entry on the 8 Right longwall section. This fall was immediately reported to MSHA.

Employee was bent over to lift a floor plate from drainage ditch. Pulled lower right side of back.

Employee was on tail piece crossover when he stepped down off and twisted his left ankle.

While carrying two, five gallon buckets of garbage from the Motor Barn to the Mainline track, the employee rolled his ankle at the de-rail. Employee sprained his right ankle causing him to fall. The employee continued to work until 06/25/2014, he then missed three scheduled shifts.

Individual was removing a lid from an electrical box when the lid slipped and caught his left thumb resulting in a laceration requiring sutures to close. The operator was not notified of this incident until 7/2/2014

Employee was assisting fellow employees in removing belt structure on the 8 right longwall. While lifting a bed rail for the conveyor belt he felt pain in his back.

Employee stated that he was operating #37 jeep coming out of the rock dust hole on the mine bottom when the trolley pole came off the wire. As a fellow employee placed the trolley pole back on the wire coal dust and rock dust from the mine roof entered the employee's left eye.

Individual was installing ventilation tubing to the face of the 9 right section. While bending to lift the tubing he felt sharp pain in his right groin area. A hernia resulted from this incident. The individual has had surgery to repair the hernia. The operator was not notified of the incident until 7/3/14

Employee stated that the motor crew was picking up garbage along 9right track. At 5 block employee was pulling on a piece of curtain about 10 ft. long. He felt pain in his lower back.

Over exposure to coal dust resulting in lung disease.

Employee tripped while pulling rock dust hose down the 1 North belt line causing him to fall landing on his right arm and back.

Employee stated when lifting and taking down mono-rail he felt something pull and pain in his right groin area. Pain existed the entire remainder of the shift. Employee was diagnosed with a hernia. Employee finished the shift and did not begin to miss work until 05/28.

Employee stated that while roof bolting on the left side of the Sandvik continuous miner on the 9 Right section the roof bolt that he was inserting into the hole started to bend. Employee then attempted to grab the roof bolt and caught his left hand between the roof bolt and the mast of the bolter causing a laceration and a fracture to his left forth finger.

Contractor was laying a block stopping, he was knelt down reaching for another block out of the cube when the band from the cube cut his arm/wrist. The laceration required five sutures to close.

Employee was operating the roof/rib bolter on the Sandvik miner on the 1 North Mains Left section in the crosscut #4-5. As he retracted the rib protector a large flat piece of rock from behind the rib protector struck his left leg causing bruising.

Hearing loss caused by over-exposure to noise.

At approximately 6:12 AM the 8 Right Longwall intersected a gas well at 135 shield. MSHA and WVOMHST were notified.

Employee was riding out on a mantrip, the mantrip derailed at 9 block. Employee was getting out of the bus and stepped in a hole. Employee was complaining of soreness and stiffness in his lower back. Employee continued to work one day after incident before missing work.

Injured employee was in the process of connecting a finishing steel to a starter steel while roof bolting when a fellow employee at the center of the roof bolting machine attempted to reposition the head. This caused the drill steels to move suddenly and strike the injured emplyee's right hand resulting in a bruise.

Exposure to dust.

Injured employee was walking from the continuous miner on the section to the auxiliary fan when the ventilation tubing fell striking his head and neck.

Injured employee was struck on the knee by a shuttle car cable. The shuttle car was stuck in the mud on the section and the scoop was being used to attempt to pull it out. The employee was standing behind the shuttle car and as the pump motor was activated the slack was pulled up and the cable struck his knee. Employee did not begin missing work until 04/21/2014.

Contractor/mechanic was putting covers on a miner when one of the covers slipped smashing his finger on his right hand.

Employee was operating a shuttle car and while attempting to turn into a left crosscut he struck his left arm on the rib causing a sprain/strain type injury to his left shoulder.

Injured employee stated that while walking by the loading machine he tripped and fell on coal spillage causing him to strike his right knee on the loading machine resulting in a bruise.

Injured employee was roof bolting on an overcast ramp when a piece of rock fell from the top and struck the 4-hole strap to be installed causing the injured employee to fall into the roof bolting machine resulting in two fractures to the small bones of a vertebrae.

Employee stated that as he parked and attempted to dismount company #73 jeep just inby the rock dust hole switch he struck his head on an installed roof bolt causing him to fall from the jeep resulting in pain in his neck.

Injured employee and hourly mechanics were working around a continuous miner. While walking around the machine, injured employee struck knees with another employee causing him to fall. He continued to work but is now off work and scheduled for surgery on 8/5/14 for a torn meniscus.

Employee was walking from the 1 North Elevator to the 1 North Spur track when he struck his head on the door frame of the second regulator in the walkway causing pain in his neck.

Injured employee sustained a laceration to the left side of his face while using a hand-held grinder to cut a roof strap along the 8 Right belt when the grinder kicked back and struck his face. The laceration required sutures to close.

Injured employee was carrying a drill feed cylinder for approximately 4 blocks on the 1 North Mains section when he felt pain in his lower back. Employee continued to work until calling off under doctor's care on 3/30/2014.

Employee was diagnosed with occupational hearing loss.

Injured employee was assisting fellow employees in carrying a tram speed reducer for a scoop from the bottom area of the mine to the track so it could be loaded onto a jeep when he sustained a strained groin. Employee did not begin missing work until 3/25/2014.

Injured employee was attempting to lift a 6" waterline off a scoop loaded with belt structure when the structure fell from the blade of the scoop and struck his right foot resulting in a sprained ankle.

Injured employee stated he struck his head on the mine roof while walking to a portal bus at the start of the shift resulting in a strained neck. Later in the shift, the employee again struck his head, on a continuous miner cable on the section.

Injured employee was unloading 2" x 50' hoses from a supply car. While standing on the possum belly type car and pulling on a roll of hose his hand slipped from the roll causing him to fall to the mine floor and land on a wooden crate resulting in bruising to his back and upper chest.

Injured employee stated that while walking out of the motor barn on the mine bottom he stepped in a hole in the mine floor created by a track derail causing him to trip and fall, twisting his right ankle and right wrist.

Injured employee states that a contractor physically pulled him from a portal bus causing him to strike his head against the top of the bus resulting in a strained neck.

Employee stated that he was working over head removing belt structure and when he woke up the next morning (03/13/2014) his left shoulder and arm were hurting.

Employee was raising the canopy on a roof bolter and felt a strain in his lower back.

An unplanned fall of the mine roof at or above the anchorage zone of the installed roof support occurred on the 8 Right longwall set-up face. The fall measured approximately 133' long x 23' wide. The fall was immediately reported to MSHA and WVOMHST.

Injured employee stated that as he was walking into the Motor Chute on the mine bottom he struck his head on a roof bolt and header board causing pain in his neck. Employee was diagnosed with a bruised head and strained neck.

Injured employee stated that he noticed material falling from the mine roof conditions of the roof beginning to deteriorate as he was walking behind a scoop. While attempting egress from the area he tripped and fell into the scoop striking his head and right arm.

Employee stated that as he stepped down from a walk through roof bolter on 1 North Mains while loading roof bolting supplies he sprained his right knee.

Injured employee was working in the #9 heading of 1 North Mains when a piece of rock fell from a rib and struck his left foot resulting in a fracture.
