Employee was walking around the scoop that another employee was operating up on 17 Headgate. They had just loaded a winch power box into the scoop bucket and they were going to take to the mouth of the section. In the process of walking around the scoop three rocks fell from the mine roof and drove the employee down to ground beside of the scoop and into the mine rib.

Employee was walking in the #3 belt entry to 55 break on 19 head gate when a rock that measured 5 1/2 foot long, by 2 1/2 foot wide, by 1/2 to 3 inches thick fell between two bolts striking the employee in the head.

Injured employee stated he was kneeling down talking to another employee when a rock fell out between bolts striking him on top of the head causing him to jam his neck causing strain.

Employee was servicing the longwall shearer when a rock fell from the top striking the employee on his left leg. On 1/21/2015, employee started physical therapy on his left leg.

Employee was helping hang a MRS cable in #7 entry. Employee was holding the cable up when a rock fell from the mine roof striking the employee on his right forearm causing a laceration

EE was walking in locked and tagged out face conveyor chain and a piece of rock measuring approx. 132" l. x 60" w. x 4" t. fell in front of shield tips, bounced off coal face and into conveyor chain striking employee.

Employee was installing supply doors when a piece of rock fell from the mine roof and caught employee's right thumb between rock and supply doors.

Employee was plugging up cathead to miner when a piece of rock fell stricking employee on the shoulder.

Putting car on track, piece of rock fell out cutting left arm.

EMPLOYEE WAS REPAIRING THE TRAILING CABLE ON ROOF BOLTER WHEN A PIECE OF DRAW ROCK FROM THE MINE ROOF FELL STRIKING HIM IN THE BACK RESULTING IN FRACTURES TO NOSE AND FACE.

INJURED ALLEGED A PIECE OF DRAW ROCK FELL FROM THE ROCK AND STRUCK HIM WHILE RIDING A 3-WHEEL MANTRIP. THIS RESULTED IN A BRUISED NECK AND BACK. THIS INJURY WAS NOT REPORTED TO HIS SUPERVISOR. POLICY AT THIS MINE STATES THAT ALL INJURIES MUST BE REPORTED PRIOR TO LEAVING PROPERTY.
