Employee was fire bossing at 72 brk. 2 South seal pump when he tripped over a 6 inch discharge line and fell into a sand prop that was set around the seal wing wall injuring right knee.
An unintentional roof fall above anchored occurred at approximately 8:20 AM. The fall occurred on the slope belt at break 12 in the intersection. They were no injuries nor was ventilation impeded. The fall measured approximately 35' wide X 40' long X 6' high. A plan was submitted for clean-up of the fall.
Employee was using a slate bar to remove a rock from on top of the continuous mining machine. In the process of prying the rock off of the continuous mining machine, the rock slid down the slate bar and smashed the employees little finger on his left hand.
Employee was walking toward face from the power center on the 6 West section. After the employee got past the power center about a break, he slipped on the wet roadway and twisted his lower back.
Employee was pulling a shield in on the longwall face. In the process of pulling in the shield his foot caught got under the pontoon of the shield and it caused him to smash his right foot. It broke the tips of three of his toes and the employee had to receive four stitches in one of them.
Employee was helping to move the power on the longwall section. The monorail became hung up and the injured employee was feeling a joint of it looking for a rough spot. In the process of feeling the monorail it broke free and smashed the employees' right hand. This caused two of the employees fingertips to be mashed.
Employee was going to help to unload vulcanize equipment at 3 South Head. In the process of stepping out of the battery motor he stepped on a rock, twisting his left knee, and fell face first to the ground.
Employee was helping set timbers at 13 brk on 3 South belt line. He was using a pick hammer to set the cab wedges. He swang to hit the cap wedge with his hammer. When he did the hammer bounced back and the sharp end of his pick hammer struck him in the face.
Employee was walking around the scoop that another employee was operating up on 17 Headgate. They had just loaded a winch power box into the scoop bucket and they were going to take to the mouth of the section. In the process of walking around the scoop three rocks fell from the mine roof and drove the employee down to ground beside of the scoop and into the mine rib.
An unintentional roof fall occurred at approximately 1:15 AM. The fall occurred at the long bottom seal at the intersection of the No. 3 seal. No injuries occurred nor was ventilation impeded. The travel way to the No 3 seal was blocked. A plan has been submitted for the clean-up of the fall. The fall measured approximately 15' long X 15' wide X 3' high.
Truck he was riding in overheated - he and another employee got out to check radiator. The other employee checked hose, hose burst & antifreeze sprayed in face.
Driving his truck on mine property - truck over heated, employee got out to check radiator, hose burst, sprayed antifreeze in face.
Employee was helping change battery cells out of the #523 scoop on the 18 headgate longwall section. In the process of loading one of the battery cells into the #765 Duckbill Mantrip, the employee felt pain in his right arm.
Employee was helping unload supplies on the 6 West section at the supply doors. They had hooked up the pull chain to a pilot of bulk dust and the scoop so they could pull the dust into the scoop. In the process of pulling the dust into the scoop, the pull chain broke and came back and hit the employee who was standing beside the flatcar, in the left hand and leg.
Employee suffered a back strain when he slipped/tripped and fell onto the draw head of a low boy.
Employee was scooping gob out of a break on the long wall when a cable bolt flipped up out of the gob striking the employee in the back of the head causing his forehead to strike the value chest in the operators compartment.
An unintentional roof fall above anchored occurred at approximately 12:30AM. The fall occurred at the Long Bottom Seals between seal No 2 and seal no 3. No injuries occurred nor was ventilation impeded. The travel way to the no 3 seal was blocked. A plan has been submitted for clean-up of the fall. The fall measured approximately 75' long X 20' wide X 6' high.
Employee was using a pair of chain hoists to pull in belt on 4 South. In the process of operating the chain hoist the clamp came loose off of the belt and caused the employee to fall off the "A" frame where the employee was standing beside of the rock box. This caused the employee to strain his right knee, ankle, and put a small bruise on his face.
Employee was walking in the #3 belt entry to 55 break on 19 head gate when a rock that measured 5 1/2 foot long, by 2 1/2 foot wide, by 1/2 to 3 inches thick fell between two bolts striking the employee in the head.
Employee was helping to move some shuttle car cable up on the 6 West Section. In the process of pulling on the shuttle car cable, the employee tripped on a rock causing him to fall against the rib, striking his right shoulder.
Employee was installing a sieve wheel to the mine roof to pull in the belt on 6 West. As the employee started to hang the sieve, a rock on the right corner rib rolled out smashing the employee's right thumb between the rock and the belt structure.
Employee was backing the continuous miner out of a cut when a rock fell out between the rib and outside bolts striking the employee on the left foot.
An unintentional roof fall above anchored occurred at approximately 4:50AM. The fall occurred on 5 West B-Beltbreak No 13 in the track entry. There were no injuries nor was ventilation impeded. The fall measured 20' wide x 6' high. The area was dangered off.
EE was assisting another employee carry a 60" top roller to a scoop insert. In the process of carrying the roller the EE stepped into the scoop insert to stack the top roller on top of another one. When the employee stepped in the insert he slipped on a 60" bottom roller causing him to fall, twist his right ankle, and to drop the 60" top roller on top of him.
Employee was diagnose with permanent partial hearing loss.
Employee was removing a worn out piece of metal liner from the raw coal feeder. The employee removed the metal from the feeder and went to throw the piece of metal out of the way. When the employee threw the metal it cut his left index finger.
Stepped on step of fuel tank on truck, falling resulting in broken leg.
Employee was operating scoop near the bottom of the slope when he lost brakes suddenly and struck a concrete pad stopping abruptly. Employee continued to work until 4/24/15 when he was taken off work under Doctors order. The accident was deemed reportable by Workers Comp. on May 12, 2015.
Employee was walking around the power center on 19 HG Section. In the process of doing this, employees light cored got caught around the disconnect handle on the power center. This caused the employee to trip, striking his face on the power center. Employees tooth was knocked out.
Injured employee stated he was kneeling down talking to another employee when a rock fell out between bolts striking him on top of the head causing him to jam his neck causing strain.
Employee was dragging shuttle car cable by hand when he tripped over a piece of belt laying on the mine floor causing him to twist his left knee and ankle.
Employee was diagnosed with a Branchospasm.
EE was attempting to repair the column feed pump that was located in the basement of the prep plant. Another EE pushed the large sprocket & belt while EE pulled the belt toward him. While doing this, he looked away from the belt & got his finger caught in between the cogged V belt & drive pulley. He immediately jerked his hand out when he felt his finger become pinched.
Employee was standing beside the C/M operator while he was cutting out a belt channel on 16 Headgate. When the C/M operator was cutting some top, a piece of a roof bolt came back and struck the employee in the mouth.
Employee was operating a 9 man Brookville track mounted mantrip traveling outby on 5 west. The employee came around a curve when the mantrip de-railed causing the employee to strike his left shoulder on the frame of the mantrip.
Employee was diagnosed with permanent hearing loss.
EE was hanging bolter cable across entry when he twisted his right knee. EE continued to work and was taken off work on 4/1/2015 to repair a torn ACL to his right knee.
Employee was operating a mantrip when he struck a 4 foot strap that was sticking out from a rib bolt
Employee was servicing the longwall shearer when a rock fell from the top striking the employee on his left leg. On 1/21/2015, employee started physical therapy on his left leg.

Employee was helping hang a MRS cable in #7 entry. Employee was holding the cable up when a rock fell from the mine roof striking the employee on his right forearm causing a laceration
Employee was carrying a bag of B-Bond to a stopping when he twisted his right ankle on an airline.
Employee was installing roof bolts down the face of the long wall in preparation of moving the long wall. Employee was trying to position the roof bolt plate between the shield and ATRS. The bolt was pushed up, smashing the tip of his left point finger between the roof bolt plated and mine roof.
Employee was hanging a piece of line curtain when a section of rib rolled out striking his back and landing on his left foot.
Employee was moving miner cable out of the roadway. Employee picked up the cable and was walking backwards pulling the cable into a break through. The employee tripped and fell over a pressure pump striking the right side of his lower back on the corner of the pressure pump.
Employee was operating a 740 Caterpillar lizard truck on the refuse dump when he slid into a rut causing the operator to be thrown to the left then back to the right injuring his lower back and right thigh. Note: Employee did not start missing work until 9/26/2014.
Employee slipped on a rock twisting his right knee. Employee did not began restricted duty until 10/2/14.
