Employee sustained a nondisplaced fracture of right thumb (Distal Phalanx) when employee was ascending the stairs to access the belt 2260CON001 and the safety bar came down and smashed the employees right thumb.
Liners fell unexpectedly, striking worker's leg. Causing bruising and soreness.
EE was organizing lower tire yard. While moving wheels to scrap, EE had stacked Wheel #@1 on top of wheel #2, when wheel#3 that was standing next to the stacked wheels fell into wheel#2 while EE's left hand was in the line of fire, resulting in a broken left ring finger above the first joint.
A D10T dozer component was being repaired and the component tipped over striking the employee. Prior to the event the component was removed from the machine and was placed on blocks and was being dissambled. As parts were removed, the component became off balance.
On 2/28/2022, a handrail reclined on the wall fell over, hitting the employee in the Left Shoulder (The employee declined medical evaluation). On 3/4/2022, the employee requested to be seen at the local clinic.
EE was assembling a 6" knife valve. The EE was rebuilding valve parts that were staged next to the valve that was being rebuilt. The valve, which was leaning against a wooden crate, slid over and pinched the ee's finger between the parts and valve. The ee sustained a fracture to EE's finger and was issued restrictions.
Employee's were placing spile bars against the wall for the mechanics to slide them into the chute. An employee was handling a bar to place against the wall. Another bar that was already against the wall slid off and bounced off a stack of liners landing on the employee's right foot.
An employee was going to pry off a pre-stripe cathode, the employee dropped the pry bar and while bent over to pick it up the copper cathode came loose and hit the employee's left shoulder. The left shoulder received two lacerations.
Mud fell from dove tail of haul landing between employee's safety glasses and eyes causing pain in left eye
A dirt clod fell from the right mud flap on the 618HT and struck an employee on the shoulder.
After first dipper was placed in haul truck, material rolled down from shovel bank, striking the filter housing, and jarring the driver. Driver initially refused treatment, but the next day prior to the shift, went to the ER and was taken off work until 2/16, a day off.
Employee had completed crane tasks, and was parking the crane, lifted the PAPR visor, and a drop of condensation or rain fell from the ceiling onto forehead and rolled under the safety glasses into eye, causing a burning sensation. Employee was taken to the ER and given prescription ointment, and returned to work.
Employee was performing pre-shift inspection on haul truck, checking under engine for leaks, when a pair of pliers fell from the engine compartment and hit the employee on the safety glasses. Initially employee refused treatment, but called while on scheduled days off, 6/14, complaining of neck pain. Ongoing.
EE was conducting post blast safety check when the material below him gave way. This resulted in the employee being engulfed with shot muck to waist. Mayday was called he was removed and transported to ER. He was diagnosed with a sprain to his knee. On going.
Employee parked water truck next to asphalt refuse pile. Asphalt section rolled down hill while employee was next to driver door area. Material hit employee on right leg. X-ray has revealed a hair-line fracture on bone of right ankle. Employee released for light duty.
Standing approx. 10' from hydro blaster when EE felt foreigh object cause irritation to his left eye.
Employee was working on a haul truck. Employee sustained a cut on the forehead when an impact wrench fell and struck him on the hard hat.
Checking the bank, a rock rolled out and hit the employee. He as approx 40' from the bank.
