Mechanics were attempting to pull Sag feed cart from Sag due to plugged cart. A shackle pin snapped and struck employee in right shin, causing a fracture. Employee was taken to ER, referred to orthopedic. Currently ONGOING.

Shovel mechanic was working on the drill mast and when stepping down they rolled their right ankle. The employee was placed on restrictions on 08/09/2026.

Employee was stripping/waxing a floor when some excess chemical from the stripper came into contact with EE's knee causing a reaction on EE's skin

Employee was grinding surface area for welder. Grinder slipped and cut the employee's left middle finger. Employee received four stitches.

Employee was helping to remove chains from a tire on the loader. EE swung a brass hammer to remove a cut metal chain link. The link struck on the left cheek causing a laceration.

Following harvest activities in the Tank house, the employee was assigned to pull samples. While attaching a single pull to a cathode, the employee's left hand was caught while engaging the locking mechanism, resulting in an injury to the left hand.

A mechanic reportedly got something in their eye when washing a Haul Truck. The mechanic stated they did not feel any discomfort until they got home. After some time, the eye discomfort became pain, and the employee sought medical attention. The event went reportable 08/01/2026.

Mine Ops Employee caught on/struck by chain going under locked Mine Shop gate.

Operator of a water truck started to pull themself up the ladder when they lost grip and fell backward. Event went reportable 07/30/2026.

Tech was installing a bull gear into the final drive of a 793 haul truck. The gear hung from a crane hook by a strap; an installer bar was in the drive. While positioning the gear, the boom-up button was pressed instead of winch-up, raising the boom and shifting the gear, pinching the tech's finger against the installer bar.

The CMV Driver was walking towards the semi-trailer when their right toe caught something on the ground causing them to trip and they landed on their right side with their arm caught underneath them. They impacted their right wrist and shoulder, their arm made contact with their right upper ribs when they hit the ground.

Mechanic performing routine maintenance on a haul truck changing out the park brake valve. While using their wrench, employee slipped on ladder and caught themselves with their left hand by grabbing onto a hose on the truck. This resulted in the employee feeling a sharp pain in their left shoulder. The employee was given restrictions starting 08/03/2026.

Employee was moving boxes with shirts in them and as they were lifting a box, they felt a pop. Event went reportable on 07/29/2026.

Employee started shoveling and realized their back was in pain in the beginning of their shift. Event went reportable 07/30/2026.

Was putting discharge cap back on trailer after offloading trailer. When EE pushed the cap with EE's right thumb, EE applied extra pressure and it bent EE's thumb back which resulted in a sprain.

While pulling forward with the bed raised to plug out, the front wheels lifted off the ground. When the front end came back down, the operator began to feel pain in their back. Went reportable 07/26/2026.

Employee was experiencing allergic reaction due to the environment inside the EW Tankhouse. Employee was experiencing redness and swelling on their face.

Employee was walking and fell resulting in hurt right ankle and scraped their left elbow/arm. Injury went reportable 07/23/2026.

Employee was opening a flush water ball valve to the tails launder in the mill basement. The valve was difficult to operate, so the employee used a wheel chock to strike the valve in an attempt to open it. During the task, the employee sustained an injury to the left shoulder.

Employee lost their balance and instinctively placed their hand on a job box to stabilize themselves. The movement caused the job box lid to close, striking the back of their right hand. Went reportable on 07/23/2026.

Employee was helping to carry a 14-foot wooden frame down a ramp. Their foot slipped and they fell forward striking their face on the frame, causing them to loose their left incisor and cutting the inside of their left lower lip.

Employee was performing routine work and bent over to connect strongback. As employee stood up, they felt a sharp pain in their lower back. Event went reportable 07/22/2026.

Hydromet Maintenance employee attempted to adjust a belt mop tray While repositioning the tray, the tray edge causing a laceration to the right index finger. Injury went reportable on 7/16/2026.

Engineer gathering information about the structure underneath HBF2 (Horizontal Belt Filter) conveyor was standing below the conveyor in the containment area when liquid splashed onto their arm and into their right eye despite wearing gasketed PPE eyeglasses. Under investigation.

Employee was washing boots off with portable hot pressure washer. While doing so a stream of hot water entered EE's boot causing burns to top of left foot.

Flushing out lines on the 9 dump, while attempting to adjust the wobbler the wobbler tip broke off and raffinate solution sprayed the employees head/face area. The employee used their hands to wipe their eyes and got solution in their eyes.

SW-EW Employee sustained laceration and fracture injury to finger working with pipe/fusing machine.

Dozer D11-5 Operator was walking across drill pattern when they tripped/fell on uneven terrain and sustained a laceration injury to their forearm.

Employee descended a crane ladder and missed their grip causing them to fall onto their back. Event went reportable 7/8/2026.

Mine Tech Employee felt shoulder pain at the end of shift following repetitive lifting of 5-gallon buckets into pickup truck and emptying 100-pound sandbags into a bin. Late reported on 7/8/2026.

Employee attempted to close the door of a wet scrubber after inspection; However, door slammed shut, catching the employee's left ring finger above the fingernail and resulting in an injury to the fingertip. Event went reportable 07/06/2026

Was attempting to remove a pneumatic hose to reposition it. The fitting was slightly bent and difficult to remove. EE applied extra force and the hose disconnected and struck EE in the chin. As a result EE received 3 stitches on the chin and was released to regular duty

Rear-end collision involving two haul trucks on a haul road.

Haul Truck operator was prepping truck prior to operation and bent their thumb backwards, hyperextending it, when adjusting their dispatch screen. Employee was immobilizing brace.

Employee was laying a 4-inch pipe; when moving the pipe employees' pants leg was caught on a piece of PVC pipe, cause the employee to trip and fall forward landing on their forearms. Felt pain in their back a short time later. Employee was seen at clinic.

While using a hammer and brass plug to remove drill splits from a core innertube, the hammer struck the tube rack/frame and deflected, causing it to strike the employee's left thumb instead of the brass plug, resulting in an injury to the thumb.

Employee was installing the eye flush piping at the filter building. While tightening a bolt the swivel adapter on the impact broke. The pin from the swivel adapter cut the employee between the index and middle finger.

Lube operator was doing their 2nd round of lube and did not feel good due to heat. Event went reportable on 6/24/2026.

Hand of employee was caught between truck bed and davit arm assembly when loading equipment into bed.

A Mechanic injured right bicep during unplanned repair. They felt/heard a "pop" while lifting and aligning fender with pry bar. Continued work briefly; pain worsened. The injury went reportable on 6/24/2026.

Employee was working on a haul truck. When they completed working on the inside of the haul truck the employee stepped down and rolled their left ankle. The injury went reportable on 6/23/2026.

Employee was using a 4-pound hammer to tap in a snubber pin on a shovel dipper. A piece of fly metal from the pin struck the employee on the right cheek and caused a laceration.

At shift change, employee was riding in the back of the man van when the driver hit a pothole and employee was jostled in their seat, claiming bruises to the head and leg. Employee was taken to the ER and currently off work. ONGOING

Employee was opening an air valve for a tower mill flush water when the valve got stuck. They used their hand to wipe the blade of the knife gate when the knife gate slammed open, catching their middle finger, pinching it. Employee was taken to the clinic, diagnosed with a distal fracture and referred to ortho. CURRENTLY ONGOING.

Employee stepped on uneven ground, resulting in a trip and forward stumble. The employee landed on their right knee. Injury went reportable on 6/20/2026.

Employee tripped while walking between the motor and handrail to assist a coworker and fell onto the grating, resulting in a pinky finger injury.

Maintenance employee performing routine rounds in the Morenci canyon crushing plant. The employee began to feel lightheaded and was attempting to sit down when suddenly they lost awareness. The employee quickly recovered and got back up.
