IW leaned 36" pipe wrench against a pipe. IW placed left hand on the floor to support themselves as they repositioned a brace. Pipe wrench fell over striking employee's hand. IW sustained fractures to the left middle and ring fingers. IW finished their shift and received no lost time.

High wind and or weak uncompleted fall/collapse of structure.

Muddy materials pushed EE to hand rail causing thigh sprain. EE felt it would get better and continue working until 6/26/2015 when he got scheduled to attend physical therapy 3x/wk * 3wks

One employee was in the separator changing blades and a small amount of cement that was left on the separator table and walls dislodged and fell onto an air-slide below the separator. The material that landed on the air-slide then slid into the feed chute of the mill body where another employee was welding; the dislodged cement dust splashed onto his head and eyes.

EMPLOYEE WAS STANDING WITH HIS HAND ON APIECE OF CONDUIT - A BOLT WAS DROPPED FROM ABOVE AND LANDED ON HIS HAND.

WORKING ON SCAFFOLDING IN TOWER AND DROPPED LIGHT DOWN TO LOWER SCAFFOLD IN TOWER.EMPL DESCENDED TO LOWER SCAFFOLD TO RETRIEVE LIGHT.BENT OVER TO PICK UP LIGHT AN OBJECT FELL AND HIT HIM ON B ACK OF HEAD CAUSING LACERATION TO HEAD.
