IW leaned 36" pipe wrench against a pipe. IW placed left hand on the floor to support themselves as they repositioned a brace. Pipe wrench fell over striking employee's hand. IW sustained fractures to the left middle and ring fingers. IW finished their shift and received no lost time.

One employee was in the separator changing blades and a small amount of cement that was left on the separator table and walls dislodged and fell onto an air-slide below the separator. The material that landed on the air-slide then slid into the feed chute of the mill body where another employee was welding; the dislodged cement dust splashed onto his head and eyes.
