2 workers were installing grating/plates around the service platform at the Raw Mill area while wearing full personal protective equipment, including anti-impact gloves. After disconnecting the grating from the rigging, they tried to fit it in place by hand. One of the workers smashed EE's finger. They underestimated the weight of the grating pieces.

A fire was observed in the coal mill after the control room operator saw a spike in temperature at the coal mill exhaust inlet. The control room operator stopped coal feed going to the mill to allow the fire to extinguish.

Maintenance Mech lacerated forearm when 6" grinder bound up in the chute that they were cutting. EE received 6 stitches, returned to normal job duties.

Employee was climbing down a fixed ladder when dust blew into their eyes from above. This cause corneal abrasions.

Employees were exposed to dust kicked up when a belt was turned in a micro dome. Injured employee removed their dust mask after panicking. They was assisted up the stairs and out of the micro dome. After they laid them down, they observed that they lost consciousness, then they regained consciousness after they poured water on them.

Two employees were moving a forklift fork inward on a reach-type forklift (telehandler). One employee lifted the fork slightly while the other pushed the fork over. When the fork was set down one employee's left pinky finger was pinched, resulting in a bruise. The employee went to a clinic and had their finger X-rayed. No break was detected but the doctor offered a splint.

Employee was using a rock bar to pry loose hardened material buildup. When the material broke free the bar slipped from the employee's hand and bounced off of an adjacent handrail. When bar bounced back up it hit employee in the chin causing a cut that required stitches.

Employee was climbing down a steel ladder, after welding, slipped of the round rung and when they tried to catch themselves they struck their left hand on the ladder. After observation was taken to the clinic for x-ray and they fractured the fifth metacarpal and is pending procedure on 7/14/26

While grinding in the RM area, the employee felt a particle in EE's eye; however, EE was wearing proper PPE, including glasses and a face shield. Safety officers removed a small particle from the eye and, to ensure the employee was taken to the clinic, used vibrating tools to remove the rust. The employee returned to work after the clinic.

Cement powder flushed from ¾" pipe opening and into employees eye as EE was attempting to install and air line into the top of a hopper

The employee was working on the cone combo under belt at the dry plant and putting flexcos on to patch a hole. While moving to another area on the under belt to patch it, the employee caught their right leg on a flexco causing a laceration below the knee.

Employee was riding up in the elevator when it stopped on the 4th floor. The doors would not open. The elevator company was notified, and in the meantime other employees were able to eventually get the doors opened from the outside and the employee got out safely.

Contractor was puttting beams in clinker cooler when rigging slipped causing beam to slip and and smash EE's left pinky

Employee has transferring material in a wheelbarrow when EE made contact with a stationary steel fan shroud. EE's left pinkie finger tip was pinched between the left wheelbarrow handle and the fan housing edge.

employee was unplugging a product line with a vacuum truck when the product freed up and pushed the hose out of their hand causing product to hit them in their face

EE was talking when EE's coworker struck a column with a broken piece of aluminum handle when EE lifted EE's hand into the swinging area. EE received a laceration to EE's left middle finger, took EE to clinic for stiches.

Miner was refueling a haul truck, using a ladder to access the fuel receptacle. While descending the ladder, the minor lost footing and missed a rung and landed on the right ankle causing a sprain.

While grinding there was paint and metal particles that blew into EE's eye, EE was wearing foam lined glasses and also a face shield. Wind from the west 21MPH at the time of the incident.

The employee was working at the base of the crusher ladder and placed their left hand on the frame and started turning the pin with their right hand to collapse the ladder. The employee failed to notice that their hand was placed in a pinch point area and when the ladder folded in, it crushed their left hand.

An unresponsive truck driver was found in the driver seat of their haul truck. 9-1-1 was notified at 12:51P on Friday, March 6, 2026.

EE was opening a box of welding rods using a self-retracting blade box cutter. While cutting the thick protective plastic covering, EE cut in a direction toward self. During the cutting motion, the knife got caught in the plastic, it became free the pulling motion caused a laceration to the middle joint (knuckle) of EE's left thumb.

Cement dust in eyes while troubleshooting a finish mill grinding circuit

EE was attempting to cut a zip tie with a razor knife. Knife was in right hand and EE was holding zip tie with left. The knife slipped and lacerated the EE's left palm.

Employee suffered minute hand fracture (diagnosed via x-ray at urgent care) on Feb 12, 2026, ~10AM CST when loose bearing fell on left hand while working on IBC4 pulley at batch plant. No meds prescribed; first aid only.

a newly hired laborer for the Dayton pit went to the pit supervisor and informed them that EE had hurt EEs wrist shoveling from the previous day. The employee said that EE was using a shovel to clean off a concrete pad that had build-up on it and the shovel lip caught crack in the concrete, which caused the shovel to stop moving abruptly, which is when EE said the pain occurred.

Maintenance Mechanic's middle left finger was pinched between an hoist cable and block's sheave, resulting in a laceration and fractured finger tip.

While moving guard employee pinched left ring finger, causing the nail to come off. The finger was smashed between the guard and the railing.

The fire was called in when detected, through MSHA's Emergency hotline number on the morning of 01/13/2026. A 103(K) Order (No. 5056976) was issued for the electrical rooms affected by the fire, on 01/22/26. On 04/24/2026 A Denver office inspector was on-site for other purposes and mentioned a 7000-1 needed to be filed for the fire; this submittal is fulfilling that request.

An employee was riding the elevator down from the 10th floor and the elevator stopped at the 2nd floor. The outer door would open but the inner door would not. The elevator company was notified, and the employee was eventually able to pry the inner door open and successfully get out.

A maintenance crew was working on separator rotor cage. While lifting it with steel chokers, one of the steel chokers failed. Consequently, the other choker broke causing the cage to drop approximately 3 feet. As the cage fell, a maintenance repair got EE's fingers between the cage and the fan shroud where EE's hand was sitting, causing severe lacerations to 3 fingers

While cutting and welding on a manlift, an employee developed lower back pain. After notifying their foreman and resting, they sought medical attention. A safety representative escorted them to a clinic, where a physician diagnosed a lower back sprain. Employee was prescribed medication and stretching exercises

employee was helping step Eco blocks up for new crusher and got EE's left leg caught between two blocks and hurt both left knee and ankle.

A process attendant clearing material under the East/West CCDC screw reached into a leak hole, where their finger contacted the moving screw, amputating its tip. They reported the incident immediately, received first aid, and was taken to the ER for a tetanus shot and further treatment.

While loading coal on the pile, the operator backed down ramp and turn too soon causing loader to lay over on its side. Employee refused treatment initially, but was taken for evaluation the next shift. Operator was diagnosed with a concussion and given medication.

Employee was washing haul truck when lost balance and struck right side of chest on scaffold planking fracturing two ribs and injuring knee.

As employee was poking bar in to silo to free up material EE felt a pop in EE's shoulder.

EE was giving signals by radio to the crane operator, gave command to "hold it" 3 different time and the crane continued to boom up. The brace that was rigged and to be connected had too much pressure, was being held with spud wrench, flung the brace around and struck EE inside the right forearm causing a 5"2" laceration.

EE was lubing up a "Pile Shackle" as preventative maintenance, the pin was in the "out" position and the pin was dislodged. EE held the pile shackle with EE's left hand to keep it stable and was greasing with EE's right hand, the pin pinched EE's nail off and also part of the tip of the pointer finger.

Employee was moving too fast for the conditions. Slipped with EE's left foot and fell to left buttock. Caused swelling and the employee was taken to the ER. Was admitted over night for observation and prescribed time off work until 10-1-2025

Employee using cutters punctured themselves in the hand with the cutters leading to a laceration of their left index finger.

11:40pm a fire was discovered on hot belt 3 due to leak-by from the clinker gate. When the material was moved outside, there was no visual verification it was still going to the belt. The area was evacuated, 911 was called, and fire trucks arrived shortly after. Power was shut off and locked out for safety. The fire was out by 12:13am.

Employee brushed some dirt of a conveyor belt, and their hand slid over a self tapping screw.

While diverting material to another chute, a process attendant manually assisted a gate closure using a rock bar. During the attempt, attendant was using a strong amount of force to hit the gate. Their hand shifted and left pinky was caught between the bar and equipment, causing a laceration and fractured fingertip. Attendant was transferred to the local ER and received 7 stitches.

During hot work on a coal pipe, slag fell into the burner pipe and ignited a combustible source, causing a flashover into the kiln. A backflash returned to the coal mill, where three other mechanics were working. The three mechanics were taken to the emergency room and were given diagnosis and work restrictions

A mechanic was performing adjustments on the coal and was injured after slag from nearby hot work ignited a gas leak in a burner pipe, causing a flashover and backflash through the kiln. The force caused the mechanic to fall back and hit EE's thigh on a nearby handrail resulting in a contusion. The mechanic was given restrictions.

Process attendant showed signs of heat exhaustion, including feeling hot and a headache when trying to unplug the kiln feed elevator with an air lance. They came down to the ground and their symptoms worsened, prompting EMS response. Employee was treated with an IV, diagnosed with heat exhaustion and released the same day.

EMPLOYEE WAS USING A SPUD WRENCH TO BREAK A BUTTERFLY VALVE LOOSE WHEN IT MOVED UNEXPECTEDLY CAUSING THE EMPLOYEE TO CATCH A FINGER BETWEEN THE SPUD WRENCH AND THE HOUSING OF THE RSV. THIS SPLIT THE FINGER OPEN AND IT REQUIRED STITCHES

Mechanic was using a pry bar to straighten a bearing on a dog bone on a CAT 777 when the pry bar slipped and the dog bone fell cutting EE's finger.

Employee was shoveling dust with other employees in a tight location when they were hit on side of head by another employee causing their goggles to become dislodged. When this happened, injured employee was exposed to dust that irritated their eyes.
