Employee was struck in the head by a water bottle that fell from above.
While walking on the Longwall setup face a roof bolt head popped off and struck employee on the right shoulder. Employee worked all scheduled shifts until being taken off of work on 7/19/18.
Employee was walking on third floor of plant 2b when a screen center strap fell through a hole chase from above striking the employee in the hard hat and right shoulder. Center strap weight approximately 10 lbs and fell approximately 1 1/2 feet. Muscle pain was the only thing found at the hospital for the injured employee.
Three inch metal water line slid down the slope, knocking the injured to the slope floor. Injured suffered bruises to the back and shoulder. He also had abrasions on his left ear that required 3 sutures.
A chute was lifted by overhead crane with wire chokers & clevis's, chute was lowered back to the bottom to reattach chokers to adjust angle. The employee was inside the chute passing the chokers across, the chute slid down the slope appro. 16 ' stopping at the slope bottom. The employee attempted to move with the chute but it came in contact with his right foot and leg.
Person was installing grout on a silo wall and a stone size piece of concrete fell from the roof striking him on the hand resulting in a fractured bone in the hand.
Employee was standing to the front of the trailer to the side of the tongue, no work was being done or any adjusting of the jack. As he was standing to the side of the tongue the jack broke and bent to the side shifting the trailer and sliding it causing the tongue to fall on his right foot. He sustained two fractures on his right foot and dislocated 3rd right toe.
