Employee was placing a bench hammer in a shaft bucket and smashed finger. Laceration required stitches.

Attempting to lift an 80 lb. bag of gunite when employee felt a sharp pain in his lower back.

While employee was hanging miner cable he felt sharp pain in his lower back.

While he was building a stopping he was getting a block off the pallet and one fell off hitting his right thumb. (He returned to work on his next shift 7-20-09. This was a medical report only.)

He was attempting to throw a set of steps over a hand rail, the steps came back and struck him on the right wrist, causing a laceration that required 8 sutures.

Injured and co-worker were unloading 8 inch aluminum pipe from a scoop. As the co-worker picked an end of the pipe up injured's left middle finger was caught between the pipe and scoop smashing the finger tip. He sustained a left distal phalanx fracture and began losing time from work on 07/02/09.

Injured began losing time when he was diagnosed with a torn left rotator cuff tear and surgery was necessary to repair it. He is an underground mechanic and the repetitive motion necessary to perform tasks for this job contributed to it. He began missing time on 7/27/09.

Four men were hanging miner, loader, bolter cables and 1 1/2" hose, when he felt pain in left shoulder. He continued to work at his regular duties, but also continued to have pain. He eventually was taken off work for left shoulder surgery on 11/13/2009.

While the employee was putting belt structure in on a belt move, he felt something pull in his right groin area.** Employee started missing work on 6/3/09.

He was tying a rope to the loader cable to pull it up when his foot became stuck in the mud. When he attempted to free his foot he caused a strain to the left buttock hamstring.

Lifting a piece of 80# rail that had a guard rail attached to it with a co-worker. He strained his lower back. Injured was off work from 4/30/09 until 5/6/09.

Crossing overcast thru 32x32 door & into return entry with 250' 3" poly line. Poly wedged up, freed up, wedged up again causing injury/pain to employee hip from pulling the line.

He was carrying bags of rockdust thru a mandoor. As he was stepping up over the door jamb he felt pain in his right knee. He was taken off work for a right knee strain.

He was attempting to uncouple a diesel mantrip, when he lifted the bar he felt pain in his right shoulder. He continued to work at his regular duties but continued to have pain. He was removed from work on 4/23/2009 due to a possible left rotator cuff tear.

EE was changing the ceramic lining on the plant cyclone, when the lining slipped, causing EE to pinch his right finger against the cyclone wall. EE received seven stitches to close the wound.

While blocking the trammable tailpiece, the stab jack slipped off the rail and his left forefinger was caught between the frame of the belt tail and a crib block causing a fracture to the tip of his left forefinger and a laceration requiring 4 sutures.

Injured was removing cable from a shuttlecar cable snub. He caught his left index finger in the shock absorber cylinder of the anchor fracturing it. Injured was off work from 2/26/09 until 3/2/09. He returned to his normal job at that time.

Injured was traveling through a mandoor and felt pain in his lower back and left leg.

He was carrying crib blocks and placing them in the bucket of a scoop and felt pain in his right shoulder. He continued to work at his regular duties until February 26, 2009 when he was removed from work for a Rhomboid tear of the right shoulder.

Employee pulled a broken measuring tape from the muck pile and cut his finger.

Victim was taking down ventilation tubing and was struck on the right side of the face by a piece of slate, 4" long X 2" wide that fell from the roof. This resulted in a laceration that required six sutures to close. No time was missed from work.

Employee smashed finger in muck bucket.

Individual was lifting a body pin into the truck pivot bore and strained his back. Incident occurred on January 29, but individual did not seek treatment until February 2nd, at which time he visited a chiropractor.

He was walking under cm cable and water hose in hangers when the water hose coupler became stuck. When it let loose, the hose struck him in the head causing pain in the neck and head. He continued to work at his regular duties but also continued to have pain. He was taken off work due to head and neck pain on February 21, 2009.

He was unloading rails from a car, when his left index finger was causght between tow rails, causing a fracture and laceration requiring 8 sutures.

While performing an inspection of the conveyor belt, EE's index finger contacted the moving belt, resulting in a laceration that required medical treatment.

While collecting samples for plant tech EE opened Sample System Cabinet doors and coal dust flew into her eye

EE was assisting pulling a belt togather and he stepped over the pull cable, the chain on a sheave broke the cable striking him in the lower leg causing a break to the leg.

He was throwing a track switch, the handle on the throw, flew back and struck him on the inside of the left knee, causing a contusion of the left knee.

Employee was building cribs; had right hand on scoop bucket and crib rolled off on his right finger. Finger is broken. Splint on right pinkie and ring finger.

Injured was unloading six inch aluminum pipe from a scoop bucket onto a flat car. He was pushing the pipe and felt pain in the left shoulder. He was off work on contractual time until 12/8/2008 when he saw a physician. He was taken off work at that time. The MRI report we were informed of on 12/15/2008 showed a tear in his left shoulder.

He was attempting to straighten the top tray covers on the stageloader to put cribs on. While moving the trays he felt pain in his lower right side. This happened on 11/21/2008. He bagan missing work on 11/25/2008 for lower back strain.

Employee states, injury and other ee were loading the roof bolter, when he cut the bands on the bolts the band srung back striking him on the left hand. He was wearing gloves.

lifting an emulsion pump hurt back.

He was loading belt structure onto a jeep when he caught his left index finger between two pieces of Structure causing a hairline fracture to the tip of the index finger.

He was in the process of moving the emergency post sled when one of the posts struck him in the left knee causing a contusion to the left knee area.

Lifted ATE box to put on top of vacuum breaker and received an umbilical hernia. Injured continued to work until 10/31/08.

Employee tripped laying out a mat of rebar.

Caught his foot between lift of roof straps and scoop bucket while loading supplies.

Employee was loading a drill steel into drill steel holder and lacerated finger.

Taking down a metal stopping and a 24"x10" filler piece fell from 12 feet above and struck him on the right forearm causing a severe laceration.

Placing the bottom half of a screw clamp under a conveyor belt and caught his left index finger between the clamp and a crib. He received a comminuted fracture of the left index finger. This is a medical treatment incident. No time was missed.

Turned on a high pressure line to water down tram roads and the hose and shut off valve came out of his hands striking him on the left side of the head and the right hand lacerating both areas.

He was putting posts thru a mandoor when he felt pain in his abdomen. He went to a physician and was diagnosed with a hernia and removed from work on 9/18/08 for surgery.

Changing scoop tire, it started to fall over, he tried to stop it and felt pain in lower back. Note: Employee continued to work until 8/27/08 at which time he was taken off work by a physician.

He was adjusting the blade on a bow saw when his right hand came in contact with the blade and caused a laceration requiring 6 sutures.

Trying to pry belt clamp loose from top structure, structure belt broke. Belt and C clamp struck employee in the face, near the right eye.

Lifting blocks, to build brattice and pulled his groin. He began missing work on 10/10/08. He had surgery.

Putting conveyor chains together, smashed right ring and middle finger.

EE was cleaning off the top of the shearer, when he pushed against a rock he felt pain in his right shoulder, he continued to work at his regular duties but contined to have pain. He was removed from work on 12/3/08 for surgery to the right shoulder to repair a torn rotator cuff.
