Worker was cutting with a torch. Employee had on safety glasses but was not wearing a face shield. When the metal was heated a piece of rust came loose and went under the workers safety glasses and into their left eye. The rust particle caused a corneal abrasion in their left eye.
Employee was working under a truck and the socket fell off the handle and hit employee's mouth and broke a front tooth.
Employee was unloading a fork for an IT28 fork loader from the bed of a flatbed pickup truck. When EE slid the fork back and it cleared the end of the truck bed, the fork rolled and pinched the tip of EE's right pinky finger between the fork and the truck bed. Result was a simple fracture to the tip of EE's right pinky finger.
Employee was trimming a dam for a screen panel. The employee was cutting it with a utility knife. The knife slipped and cut employee's right leg.
Two employees were drilling a hole in a disconnect box. The hole saw slipped & made contact with one of the employees left hand. This resulted in 14 stitches in EE's left hand.
Employee stated that EE tripped over a 4 inch pipe and fell on EE's right knee. EE was provided First-Aid treatment at a local medical facility and returned to work on EE's next scheduled shift. The EE's condition was diagnosed on 6/24/2020 as being a work related injury and EE is unable to return to work at this time.
Employee was struck on the left leg below the knee by a slurry pipeline resulting in a laceration that required 4 sutures.
The employee stated that while replacing a mechanism beam on a coarse refuse screen, the became lodged against a screen side board. While trying to dislodge the beam left index finger was caught between the beam and sideboard resulting in a contusion and laceration requiring 7 sutures. EE next scheduled shift was 11/27/17.
Employee was trouble shooting an electrical transformer using a Fluke multi-meter when an electrical arc occurred resulting in second degree electrical burns to EE's right arm.
Employee has been diagnosed with Occupational Pneumoconiosis. The Safety Department was notified on 5/24/2016. Employee was terminated on 1/27/2016.
A clean coal truck pulled onto the scales, employee observed the truck bed was on fire. EE grabbed a nearby fire extinguisher and while going around the mechanics truck EE slipped in the snow catching self before falling. Operator was informed on 3/24/2016 that employee has a testicular hernia and will have surgery on 3/31/2016.
The employee was energizing a 30 amp circuit breaker. A circuit failure of undetermined origin occurred resulting in thermal burns to the employee's right arm, face and neck.
Claim for occupational hearing loss, due to exposure to noise.

Injured was replacing a 5 HP motor on an FMC Feeder, and got his hand pinched between the frame and motor tray.
Employee was trying to hook up the broken chain on the slope of the wash box, when the chain broke free causing employee to slide down the slope to the divider and the chain struck him in the lower back. Employee worked up to 10/4/2012 and missed the first shift scheduled back on 10/8/2012.
EE was cleaning the windshield of his articulating truck and slipped causing him to grab the hand rail. He stated his arm began hurting but finished his shift. He latter went to a Health Care Facility and was told to stay off work for a strained arm.
While taking a compressed spring out of a raw coal feeder, the EE suffered a fracture to his middle finger on his left hand when the spring released tension.
Employee was working in a muddy area lifting fifty pound coal samples one at a time into the back of the company truck. As he turned to put the sample into the bed of the truck, his foot slipped in the mud, causing his lower back to twist. Employee was diagnosed with sprain/strain to his lower back.
While using his hammer to knock ice off the belt roller, his hand slipped, causing it to become caught between the belt and the skirt board rubber on the belt.
EE was changing the ceramic lining on the plant cyclone, when the lining slipped, causing EE to pinch his right finger against the cyclone wall. EE received seven stitches to close the wound.
Changing belts and caught finger in mechanism. Skinned rt. little finger back midway between first knuckle and top of finger.
Washer bottom being replaced. When it got hung, employee pulled on it and it came loose and caught his finger between side and bottom.
Injured employee was holding a piece of channel while another employee was striking it with a sledge hammer. Struck right ring finger with sledge hammer.
Injured was straining while working under truck. Collapsed rt. lung.
INJURED WAS FINISHING SHIFT. TURNED TO BACK DOWN THE STEPS AND REACHED FOR THE HANDRAIL, MISSED IT AND FELL.
INJURED WAS GETTING OFF OF DOZER. KNEE MADE A POPPING NOISE. WHEN HE WOKE UP THE NEXT MORNING HE COULD NOT WALK.
MOVING TO LANDING AND STEPPED ON 1/2" STEEL SAFETY CABLE. CABLE BROKE UNDER HIS WEIGHT CAUSING HIM TO FALL BACKWARDS INTO PIPE WORK WITH LUGS. HIT BACK, SHOULDER, AND TAIL BONE.
EMPLOYEE WAS WELDING AND STOOD UP STICKING BACK AGAINST PIPE THAT WAS STICKING OUT OF SUMP.
TIGHTENING BOLTS ON MECHANISM BEAM. TORQU MACHINE WEDGE FELL OUT AND HIT RT TEMPLE. WAS UNAWARE ACCIDENT REPORTABLE UNTIL 7/16/02.
EE WAS USING PERSONAL PROTECTIVE EQUIPMENT AND HAD SAFETY HARNESS ON. EE WAS SHAPOING METAL TO FIT INSIDE OF SHUTE. A PENCIL LEAD SIZE FRAGMENT RECOCHETED BETWEEN SAFETY GLASSES AND IMPALED IN TO EYE. EE WENT TO HOSPITAL.
EMPLOYEE WAS REACHING INTO BACK OF PICK UP. HE TRIED TO MOVE A LOADED JOB BOX AND FELT MUSCLES PULL IN BACK.
MOVING PIECE OF METAL, IT SLIPPED IN HIS HAND CAUSING PAIN IN HIS BACK.
WELDING BENCH ON 1ST FLOOR PIECE OF RUST WENT INTO RIGHT EYE WHILE GRINDING PIPE.
CUTTING WITHT TORCH, FACE SWEATING, REMOVED GOGGLES TO WIPE FACE. METAL IN EYE WHEN RUBBING FACE.
EE, PLANT ELECTRICIAN, WAS SHOCKED WHILE CLEANING INSIDE OF CONTROL BOX OF DIESEL PUMP, IN WHICH HE THOUGHT WAS DE-ENERGIZED. HE WAS SPRAYING WITH CLEANER AND WIPING IT OUT, AND RECEIVED A SMA LL SHOCK 110V CAUSING HIM TO FALL BACKWARDS STRIKING HEAD AND SHOULDER ON PIPE.
