A mechanic was trying to remove an engine oil filter from a CAT D9T Dozer. While struggling to loosen the oil filter, the mechanic felt an immediate pain in lower back. The employee was taken to a local medical center where diagnosed with a lower back strain.

Wind blew the employee's hard hat off of head, as employee tried to catch the hard hat they slammed the hard hat against left ear causing a hematoma. Dr. lanced and drained the area affected on the Left Ear, and a couple of sutures were applied.

Mobile Equipment Specialist called into work on the morning of 3/12 and informed EE's supervisor that EE's back was too sore to come into work. EE said EE's back was hurting from performing pumping activities the day prior. EE was taken to the medical clinic on Mar. 14th where EE was diagnosed with a lower back strain. Restrictions and medications made this a recordable injury.

Electrician stripping wire in well field felt particle enter his right eye. Employee rubbed his eye to remove the particle. After the end of the work shift, the employee experienced pain and irritation in the eye. The employee was taken to the local optometrist where a prescription was given for eye irritation.

Employee was cleaning C 1 sump pump area and thought he had the hose turned off. Turned water on and hose kicked back while holding nozzle spraying water under his face shield and safety glasses into his right eye

EE was lifting a final drive with his crane when it swung back and hit right knee, pinning his knee between the push arm of the dozer and the final drive.

Employee had completed an installation of an hydraulic hose on an end dump when he felt a sharp pain on the right side of his abdomen. After eating lunch he discovered a knot in his abdomen. On 08/05/15 he was diagnosed with a work related umbilical hernia. He does plan on having surgery.

Employee was standing in and working concrete on an 85x85' pad. The pour amount became excessive and concrete entered the top of his knee-high rubber boots. Employee washed the exposed area but apparently not well enough to prevent chemical burns from the concrete onto both his lower legs from occurring hours later.

Injured had backed his vehicle over a farm T-post. While trying to dislodge the post, his finger was caught in between the T-post and attached 1/8" steel cable.

Employee and company were unaware of an occupational injury. On 06-02-15 employee stated he recalled picking up a choker on 05-28-15 with no issue. Matter turned over to Workers Comp. On 06-30-15 employee diagnosed with bulging disc and began losing time. Upon investigation and WC acceptance of the injury on 07-14-15, it was concluded this should be reported.

On 2-18-2015, employee was lifting a hydraulic jack ram when he felt pain in his right shoulder. Employee continued to work full duty with no medical treatment. On 6-24-2015, employee had surgery on his shoulder for a torn bicep and rotator cuff tear making this injury reportable to MSHA.

As employee was depositing a bag of trash into a dumpster, he felt a small strain in his lower back.

Employee arrived for work and was entering the shift change building. He did not raise his foot high enough for the next step and stumbled but did not fall. He was using the handrails while ascending the steps and caught himself from falling. When he caught himself from falling his lower back began to hurt.

Employee strained back while taking off coveralls on an end dump. The back strain prevents him from performing his regular duties.

Employee was attempting to build a replacement hydraulic hose. The employee had just cut the new hose to the proper length and when he attempted to remove the hose from the clamp, the stored energy caused the hose to snap back and struck him in the bottom lip. The employee received stitches to close the laceration to the lip.

A mechanic was moving a piece of 1/2 inch plate from the metal rack to a welding table in the shop. He leaned the plate up aginst the table and when he let go of the plate it slid and fell on his right foot. The plate weight was 125 LB

Employee was installing a suction hose on a 6" water pump in the Wright Branch pit in D4 area when he experienced pain in his lower back on left side. He was taken to ETMC Henderson where he was diagnosed with a back muscle strain. He was instructed by the doctor not to work the next day.

Employee was trying to connect a pull chain from a skid mounted light plant to a D9T Dozer hitch. The employee was trying to move the fuel quick connect to access the hitch pin area. While trying to move the quick connect out of the way, the bracket slipped from his right hand and contacted his left hand causing a laceration to his left ring finger.

Employee was approximately 3.5 hours into shift and bent to pick up a water hose while cleaning.

A miner was performing a pre-operation inspection on truck/vehicle. While getting into truck he had to sneeze and when he turned his head to sneeze he hit his right eyebrow on the door of the truck causing a laceration above his right eye, on the eyebrow, which required 3 stitches. Miner had on all PPE when injury occurred.

While assisting in removing hot coal from silo the employee was burned when there was an unexpected spill.

Empl was working in Pit when Shell cracked off face of Highwall and fell causing water & mud to rush over berm. After the incident, empl went to the back of truck got a hammer and began hitting his arm with a hammer. Empl claimed he was struck by flying debris from incident. Co-worker witnessed the Empl using the hammer and placing it in the truck once they were ready to leave.

Employee was working on D35 dragline # 3 swing case. When he put on a welding hood a foreign body got in his right eye. He was given a description medication for his eye.

Employees were removing belt clamp from C3 belt. When the first belt clamp keeper was removed, the bottom bar of the clamp shifted, catching the employees finger.

On March 4, 2014, the electrician reported that he strained his back the previous day. He did not wish to seek medical attention at that time. On March 11th, he asked to see a physician. The physician diagnosed him with a lumbar strain with sciatica. The employee was given restricted duty and referred for physical therapy.

Employee attempted to assist another employee in removing a piece of equipment from the mud. After completing the task, the employee was removing the winch from the end dump. When the winch released, a burr penetrated the employee's glove and cut his finger, requiring sutures.

The mechanic ruptured or detached the proximal tendon of his right bicep muscle while performing a bucket changeout on a Marion 8750 Dragline. The mechanic did not report the injury for four days. On February 10th, an orthopedic surgeon issued restrictions which made this a recordable injury. Physical therapy has also been approved and will be scheduled.

Mechanic was installing a hydraulic hose on D10R Cat Dozer and over-exerted his left arm, causing him to feel sudden discomfort in his shoulder. On 11/08/2013 the injury became recordable.

Employee was pulling a roll of flex line on an elevated bed of a pump truck with a 4" strap. Another employee was pushing the roll. As the roll broke over the edge, the employee stopped it briefly and then let it go but attempted to slow the momentum of the fall and therefore the weight of the roll jerked his wrist from the weight causing pain.

When installing axle in final drive housing on left front of 815 cat compactor employee was pushing down with right hand sliding axle into place when right hand became pinched between snap ring and sun gear on axle shaft causing injury to right hand.

While cleaning and shoveling coal spillage in the C3 tunnel, the employee felt a pain in his lower back.

Injured employee was walking on uneven terrain with another employee unrolling barb wire and the wire slid on the support bar cutting the employees left forearm. The employee was taken to the ER for a Tetanus shot and the ER Doctor decided to close the wound with glue and two steri-strips for support .

While running wire through conduit workers legs gave out and had a sharp pain in lower back area.

Employee was reinstalling a 992G loader motor when the motor lowered onto the employee's fingers.

Employee was changing brake shoes on #96194 rail car. As he was reaching to remove the old brake shoe, he felt pain in his lower back.

Two electricians were moving a section of dragline trail cable when it became snagged on a stump. The stump broke free causing the trail cable to sling forward striking the right ankle of the electrician working on the ground. He was diagnosed with a sprained right ankle at the emergency room.

Mechanic was replacing #2 Swing retainer plate on 16001 Bucyrus Erie 1500 Dragline. As the ring was being held in place with a piece of angle iron, the ring shifted to one side and fell, catching the mechanic's left middle finger tip between the plate and the housing. Employee's injury was secured by fellow workers and employee was taken immediately to Emergency Room.

Employee was assisting in installing a dragline drag chain repair link when the employee's left hand ring finger was caught between the drag chain and repair link.

A railroad mechanic was repairing a hitch cushioning device on a caboose when stored energy was released causing the mechanic's wrist to contact an axle. The mechanic was diagnosed with an avulsion fracture of a bone in his left wrist.

Employee was handling a piece of sheet metal when it shifted. He grabbed the piece and a sharp edge cut through his gloves and cut his left thumb requiring 5 stitches.

Employee felt pop in right knee when boarding the right shoe on a BE 2570W dragline.

While moving furniture to conference room employee attempted to move a hearing booth by himself with a dolly. The booth was to heavy and fell back on his right leg/knee. Employee was then taken to clinic and determined he had sprained his knee.

Employee was working on a pickup truck replacing the front hubs. The truck was on an automotive rack and the employee was under it. While working on the truck an object got into his left eye. He was taken to the doctor and small object was removed with the help of a swab. An antibiotic prescription was given to the employee.
