Kosse Strip Coal

Equipment operator got off the piece of equipment to move a tractor pan wheel out of the roadway that had come off the pan, and in the process of lifting the wheel, the employee felt and heard a pop in their left shoulder. The employee was taken to an occupational medical clinic in College Station, Texas and diagnosed with a torn bicep muscle.

Employee fell on a seat arm rest on the shift change bus injuring their left rib area. Doctor diagnosed as first aid injury with no restrictions on 11-11-24. On 11-21-24 employee contacted safety indicating was still having discomfort. Employee seen by medical professional on 11-22-24 and placed on no work beginning on 11-23-24 until follow-up appointment on 12-3-24.

Employee was pulling up on a breaker bar trying to break loose the keeper on dragline bucket tooth and felt a pop in their right shoulder.

A Luminant employee received a laceration on their left hand while attempting to cut air fresheners off of the rearview mirror of their work truck. The laceration required stitches.

Employee was removing bolts on cylinder piston, cylinder was under pressure due to hand brake still being applied causing cylinder to shoot out pinning employee's arm between brake arm and cylinder. Employee received a pinching contusion to right arm and had to seek medical attention at nearby hospital.

Dozer operator was working on a hot spot at the Kosse mine coal barn when the machine caught fire causing the operator to sustain burns to their hands, face, buttocks and legs. EMS was called and the operator was taken off-site for medical treatment.

Employee was cutting and welding on the roof structure when EE stopped sweating due to the temperature of mid 90's and high humidity.

Motorgrade just finished getting fueled and caught fire.

While operating a haul truck, the operator hit a rough spot in roadway jarring their back. Did not report incident until 6-2-24. Evaluated by occupational doctor on 6-6-24 and given restrictions that cannot be accommodated. Became lost time injury on Friday, 6-7-24.

EMPLOYEE WAS MOVING METAL SHAVINGS FROM ONE ROOM TO ANOTHER ROOM WHEN THE SHAVINGS HUNGUP CAUSING THEM TO CUT THEIR RIGHT-HAND INDEX FINGER NEEDING FIVE STITCHES.

Employee was drilling 1-5/8" holes and when in the process of changing out the drill bits, the head released and pinned EE's Right-Hand little fingertip between the Jacobs chuck and the plate EE was drilling. This caused a partial amputation of the Right-Hand little fingertip.

Employee was grinding overhead and when they removed their hardhat and face shield some metal particles fell into their right eye.

Bearing/steady rest assembly weighing about 40-50 pounds, came off the end of the bar and landed on the employee's left pinky finger. The employee received a fracture to the finger.

Employee was stepping over #2 hoist coupling to inspect #1 hoist motor and their foot slipped causing them to lose their balance. The employee reached out to catch themselves from falling hitting their left hand causing a dislocation of their left ring finger.

Employee was setting up welding machine to make a weld. Employee used a retractable razor knife out of their tool box to strip wire to ground the machine. The retractable razor knife slipped cutting their left index finger resulting in a laceration requiring sutures to close.

Employee was working next to a pull box when EE bent over to pick up materials. When EE stood up, EE made contact with the corner of a recently removed panel cover, which caused a small laceration to EE's upper lip.

Production worker was working at the Coal Barn and became overheated.

Employee was walking around the back side of a wheel pump and lost footing due to a washout. As the employee stumbled, their left foot went downward into the washout. As their right foot came down to keep them from falling, the right side of their knee came in contact with a piece of angle iron on the wheel pump causing laceration on right knee.

during routine maintenance on dump block assembly portion of rigging, dump block moved and pinned contract miner up against the upper spreader bar. The contract miner was life flighted.

Employee was power washing a piece of heavy equipment in the wash bay and slipped on the pavement that resulted in a fracture of their right ankle.

Employee claims they hurt their back while pulling up a post. The mine operator has not conclusively determined whether the employee's injury is an occupational injury. The injury remains under review by workers comp carrier.

Moving office furniture lifting and dragging a cabinet away from the wall when the cabinet suddenly stopped causing lacerations to three of EE's fingers due to location of the laceration on two fingers at the PIP Joint requiring stitches.

Mobile equipment operator of a 785D hauler fell through the access ladder approximately 4 1/2 to 5 feet from the platform to the ground while reaching to pick up their backpack and cooler off of the platform.

Hydraulic ram was placed on a lug mounted to the side of the dragline. The ram slid off the lug. The safety line attached to the ram caused it to swing and contact EE's left hand.

The employee was working in the warehouse rigging yard putting pins on a pallet. When EE rolled the pin into place, the pin moved faster than expected and smashed EE's left thumb in between another rigging pin.

Employee was operating a farm tractor when the engine began to overheat, the employee lowered the loader and shut the tractor down. While opening the hood, the employee stepped over the loader tilt linkage when employee's foot slipped causing left knee to strike the loader tilt linkage resulting in a laceration about 3 inches below the knee requiring stitches to close the wound.

Employee instinctively reached out to catch a coffee pot that had dropped while EE was in the process of cleaning it resulting in a laceration between the pinky and ring finger of EE's left hand. 3 stitches were required to close the wound, making this reportable to MSHA.

Employee stepped out of car in the employee parking lot prior to work shift and slipped on ice causing them to fall to the pavement. Employee reported soreness with hip, shoulder and bicep but did not request medical attention. Later on 4.8.21, employee started a series of physical therapy treatments which made this incident reportable to MSHA.

Employee was stepping up onto a dozer and felt a pop in their right knee. A few days later, the employee said that it was a pre-existing injury, but EE did re-aggravate knee while climbing onto the dozer at work. The employee was taken to the doctor on 2/23/21 and diagnosed with a meniscus tear.

Employee started to slip while carrying a load and grabbed a door jam to regain their stability. The employee heard a pop and felt immediate pain in their right arm and immediately reported the incident to their supervisor. The employee was taken to the ER and the physician diagnosed the employee with a right distal bicep tendon rupture that would require surgery.

Employee was standing by foreman's truck waiting for shift to begin and have temperature taken when they passed out for unknown reasons. When they passed out they fell to the ground and struck head on small gravel resulting in a laceration. EE received hospital medical treatment to correct a medication imbalance that caused them to pass out.

Employee was working inside a coal hauler (belly dump). While working the employee raised welding hood up and the grinder dust that had collected on the welding hood fell into eye.

Installing Reeving Ropes on dragline drag drum. Reeving rope secured to pro-locks on drum and was being lowered by crane from above via nylon rigging. Pace of drum rotation became greater that pace limit of crane causing nylon rigging to break and reeving rope to fall awkwardly below striking employee at right shoulder and left ankle.

Two mechanics were putting a tire on a rim. As one mechanic would pry a portion of the tire bead over the rim with a tire tool, the other mechanic would pry another portion of the tire bead over the rim. When the second mechanic started prying on the tire bead, the first mechanics tire tool slipped and the end of the tire tool hit the first mechanic in the lower lip.

The employee was hooking up sling chains to the D10T track grouser pad on the right rear side. As EE went to hoist up on the chain sling and using right hand to adjust chain position EE's glove hung between chain and grouser mashing the right thumb.

Employees were installing pins into the carrier for the drag drum on an 8750 dragline. During the hammer process the fingertips of the injured employee's right hand ring finger and little finger were caught between the pin and the hammer. The contact resulted in corrective surgery to repair the damage.

Employee was using a T-post driver when EE missed the T-post on one of the down strokes and the T-post struck the side hand-hold and smashed the tip end of EE's right pointer finger resulting in a fractured bone tip and stitches.

Employee was cleaning C 1 sump pump area and thought he had the hose turned off. Turned water on and hose kicked back while holding nozzle spraying water under his face shield and safety glasses into his right eye

EE was lifting a final drive with his crane when it swung back and hit right knee, pinning his knee between the push arm of the dozer and the final drive.

Using a pressure washer to clean drag line boom when employee inadvertently made contact with top of left foot while pressure was fully engaged.

Employee had completed an installation of an hydraulic hose on an end dump when he felt a sharp pain on the right side of his abdomen. After eating lunch he discovered a knot in his abdomen. On 08/05/15 he was diagnosed with a work related umbilical hernia. He does plan on having surgery.

Employee stepped into an uneven ground surface as he was dismounting his Excavator and sprained his left ankle.

Employee was standing in and working concrete on an 85x85' pad. The pour amount became excessive and concrete entered the top of his knee-high rubber boots. Employee washed the exposed area but apparently not well enough to prevent chemical burns from the concrete onto both his lower legs from occurring hours later.

Employee and company were unaware of an occupational injury. On 06-02-15 employee stated he recalled picking up a choker on 05-28-15 with no issue. Matter turned over to Workers Comp. On 06-30-15 employee diagnosed with bulging disc and began losing time. Upon investigation and WC acceptance of the injury on 07-14-15, it was concluded this should be reported.

On 2-18-2015, employee was lifting a hydraulic jack ram when he felt pain in his right shoulder. Employee continued to work full duty with no medical treatment. On 6-24-2015, employee had surgery on his shoulder for a torn bicep and rotator cuff tear making this injury reportable to MSHA.

Employee transporting coal to crusher along a haul road when the Hauler contacted a pot hole, causing a muscle strain to the employee's back.

Employee was attempting to mount bulldozer when his foot slipped, causing him to hit his head.

Employees were removing belt clamp from C3 belt. When the first belt clamp keeper was removed, the bottom bar of the clamp shifted, catching the employees finger.

Employee attempted to assist another employee in removing a piece of equipment from the mud. After completing the task, the employee was removing the winch from the end dump. When the winch released, a burr penetrated the employee's glove and cut his finger, requiring sutures.

An employee was attempting to remove a wellhead from it's position by cutting through the casing with a cutting torch. The employee cut several little openings in the casing and the wellhead fell over and struck the employee due to a lack of support of the wellhead by a backhoe that was being used to complete the job.

Employee was assisting crew which was installing wooden transmission utility pole. Ground was frozen due to 19 degree temperature. When he attempted to roll the pole with a cant hook - his foot slipped on hard ground and he fell. He was taken to ER and X-ray determined that he fractured his fibula.

Employee was using a utility knife to trim out door facing when knife slipped and cut palm of left hand.

The employee was using a 6' pry bar to adjust the brake rigging on a railcar. As the employee was pulling back on the bar it slipped out causing him to fall back and hit his head which resulted in a laceration that required sutures.

On Friday July 19, 2013, dragline D52 was out of service on a schedules preventative maintenance day. The employee at the machine was not feeling well and started to experience a medical issue. EMS was requested and the employee passed away at the Groesbeck Hospital emergency room due to a Myocardial Infarction.

While cleaning and shoveling coal spillage in the C3 tunnel, the employee felt a pain in his lower back.

Employee was sampling at a coal pile, felt hot and dizzy. Went inside to A/C room for 45 minutes to cool off. Still dizzy, an ambulance was called and took him to emergency room. He was treated by the ER doctor with an IV and released with a one day restriction of no work in hot conditions. Employee was diagnosed with heat exhaustion. The temp was 100 degrees.

While walking on drag line catwalk, employee experienced a personal medical illness causing the employee to fall (same level) striking his head. A laceration was noticed on the back of the employee's head requiring 8 staples.

Employee was reinstalling a 992G loader motor when the motor lowered onto the employee's fingers.

Employee was changing brake shoes on #96194 rail car. As he was reaching to remove the old brake shoe, he felt pain in his lower back.

Employee was handling a piece of sheet metal when it shifted. He grabbed the piece and a sharp edge cut through his gloves and cut his left thumb requiring 5 stitches.

Employee placed his right knee onto a shelf to reach something on a higher shelf. He felt a sting in his knee. On 09/10/12 a doctor placed him on RD. On 09/12/12 he had surgery due to a staph infection.

The mechanic had chest pains and left shoulder and left arm pain while he was attempting to locate a leak on a Caterpillar 349E excavator. He was transported to the hospital and diagnosed with a heart attack. MSHA inspector was on site and informed at the time.

Employee received cervical strain when shift change vehicle hit bump in road. On 07/31/2012, employee did not work full shift and was prescribed physical therapy treatments causing injury to be reportable.

EE was driving t-post for fence. While driving t-post he lifted t-post driver too high causing him to miss the t-post on the way down and caught his finger between t-post driver and t-post. Has to have stitches in left index finger.

While cutting hay, the cutter blade hit a metal object causing a spark which ignited a grass fire; 2-3 acres estimated to be damaged by fire. Fire burned approximately 30 min.

Employee was cleaning coal in the D-3 pit area. As he was backing up with the loader bucket in the air, the left back tire rolled off the ramp edge causing the machine to slowly roll onto its left side. The operator sustained bruising to his left shoulder, elbow, and shin. He was taken in for medical evaluation and placed on restricted duty.

While gathering tools to begin job, the involved employee lifted a roll of fence wire and experienced pain in his lower back.

Employee was working underneath a Cat D6 Dozer in process of removing a hydraulic hose. While maneuvering up and underneath the dozer getting into position to loosen the bolts holding the hydraulic hose, employee twisted into a awkward position which caused a strain on his lower back.

Employee in process of removing brake filter housing received laceration to upper lip. Employee was using a ratchet wrench on the bottom nut of the filter housing, pulling towards employee to break housing free. Once housing became free, the open end of the wrench came in contact with the employees right upper lip.

Grass fire started in non active area of mine, due to tree contacting high line wires during high winds. Approximately two acres affected.

While operating water truck to D3 area, employee noticed fire behind cab. The truck was stopped and fire extinguished after approximately fifty minutes. Off site fire assistance was requested. Damage was noted to the brake system, cab, tires and drive train of equipment.

Employee stumbled up stairs and hit his chin on landing edge. As a result the employee received stitches.

Employee was moving trail cable in with a CAT 930H. The equipment went over a deep dozer track. The back tires dropped into the dozer track. As a result the employee suffered a lower back strain.

Employee was in the process of removing a wiring harness on a D10 dozer and dropped his allen wrenches. When the employee reached down and grabbed the wrenches he cut his left thumb on a hose clamp as he pulled his hand back up.

The employee was pushing coal off of the tepee and pushing back to the east. When the employee was making a push the coal gave away beneath the left side of the dozer causing it to slide down into a void. There was no injury or equipment damage.

Supervisor was observing employee changing out teeth on the dragline line bucket. When the employee struck the tooth with a sledge hammer a metal sliver flew off and struck the supervisor in his right hand.

Pier hole was drilled for high mast lights. Driller was examining drill tower and walked off into hole falling 45 feet to bottom. Compound fracture to left lower leg and bruise to right leg.

Employee was raising a jack stand extension and it slipped pinching his left ring finger between the jack stand and the extension. The employee received a small laceration to his left ring finger that required 6 stitches.

EE was removing a pin when the cylinder slipped causing a laceration to the left thumb.

Employee closed a door on injured employees right hand, causing a non displaced fracture to employees right hand, middle finger.

Employee was trying to roll the front motor generator set #2 with a 5 ft. prybar. The bar slipped and the employee fell backwards against the rear motor generator set #1 mounting base striking his left back area. Employee sustained an occult rib fracture.

Employee was dismounting backhoe. As he was turnng to descend the ladder, the cuff of his left glove caught on the ladder actuator device causing him to lose his balance. He fell to the ground landing on his right side injuring his right hip, shoulder, and lower lumbar. Employee was hospitalized and diagnosed with deep bruise to hip and shoulder.

Employee mashed his right hand while loading scaffolding components onto a flat bed pickup truck. He was diagnosed with a contusion.

Employee tripped on wire and fell, causing fracture of fibula in right foot and required surgery.

On 1/7/09 employee was painting inside area known as TP; on 1/8/09 he began coughing up blood; on 1/9/09 he was taken to Limestone Medical Center where he was diagnosed with chemical burns & pneumonia. That facility was not able to treat & he was taken to Hillcrest Baptist Hosp., Waco, TX. Employee is a smoker.

Installing fender brace using sling chain. Brace was on ground standing on its end, chain attached w/slack. Employee holding chain in L hand, pushed brace down to ground. Brace fell and chain wrapped around L index finger and pulled tight.

Lifting 4" pipe with other workers, suffered strain in lower left abdominal area.

EE was grinding w/ 7" grinder on 1-1/2 plate...hit edge of plate & kicked back striking Lt leg resulting in small cut which took 5 stitches.

Employee was eating lunch & drinking a soda from a can. He put down the can, picked it back up to drink & was stung on upper lip by a bee. He is allergic to bee stings but did not have medication with him.

Lifting a piece of 3" pipe and strained abdomen.

Welding pipe, hot slag went into boot & burned right ankle.

He was attempting to move a piece of grating. He was tied off properly. When he moved the grating slipped pinning his thumb between it and the steel.

2" metal plate slipped off employee hand and smashed left hand pinky finger.

Employee was opening a can of welding rods, when the metal band came loose and struck his left thumb causing a laceration, requiring 5 sutures.

Sorting steel and was bitten by spider on left forearm.

Lifting forms and pallets, strained abdomen/groin. Note: EE was laid off due to lack of work.

Employee was turning to sit down in a shift change bus seat. As he was turning the bus took off causing him to twist in the direction he was already turning. He felt pain in his left hip. He has an existing condition of lower back problems causing his left sciatic nerve to be aggravated. The incident caused his existing condition to "flare up".

Lack of communication: I was guiding a piece of steel down attached by the crane and a co-worker struck the steel with the hammer as I reached with hand to straighten it. He accidently hit my fingertip in the process.

EE was entering #2 swing compartment using flashlight to set-up shop light. Bumped into 2x4 wooden block which had shipping staple and lacerated left upper ear.

Conditions were wet from a sudden rain event. The Employee was trying to cover work area with plastic to protect from water and slipped on edge of trench. He struck a 1-1/2" pipe and cut his lower right leg. The cut required 10 sutures to close the wound. (Note: the work schedule is Monday through Friday)

Installing channel stringers & caught arm between stringer and wall. Sprained left wrist.

EE was using a chain block attached to a pad eye. Weld broke, causing pad eye to strike EE on knee & forearm.

While climbing on to dozer lost his grip and lost footing causing him to twist his right knee.

EE was working in Rev Fr of 8200 DL. Entering compartment slipped & bruised left thigh.

EE was grinding in Rev Fr when felt something in lt eye. Washed eyes...thought was gone. Later discovered still in eye. We could not find anything-nor could the 1st MD. Finally 2nd Eye Dr found & removed.

EE was welding on ballast section of 8200 from manlift. He turned & felt a sharp pain in middle of back.

EE was walking around front of truck to get lunchbox...foot slipped and EE felt pain in rt knee.

Thr foreman walked into Rev Fr to check on crack being welded. Welder stepped from 3' ladder causing air gouger to fall to floor hitting trigger...sending debris from the floor into the air and into the foreman's left eye.

EE got hot while pouring an inlet structure. Instead of stopping when he began to feel bad, he kept working thinking it would pass. First indication was he "threw-up".
