Employee finished splicing 39-B belt and began training the belt for proper alignment. As they were adjusting the structure, they pulled on a piece of top structure and they slipped on the wet mine floor. When they slipped and fell, they twisted their back and felt pain in mid to upper back.

Employee was opening airlock doors and felt pain down EE's back
EE was helping to move a MRS that was stuck in bad mine floor conditions. EE, along with several crew members, were assembling and spacing the metal slats and clevises to connect the C/M to the MRS. While handling and positioning the slats and clevises EE suffered a strained back. EE could not remember the exact time of the event.
EE was standing beside the scoop hanging the 12" waterline from a rib bolt with a chain when the chain slipped out of the eye of the plate causing the waterline to fall and strike EE in the top of the left ankle.
Worker was aligning two pieces of pipe on a pump in order to bolt them together. One piece of pipe fell; and as a result, smashed and cut open the pinky finger on worker's right hand. Received outpatient treatment: 13 stitches to close wound and support brace applied to finger.
Employee was working on the 4 Right Section and EE was helping move the C/M and as EE was hanging the cable EE felt pain in lower back.
Employee was adjusting the forks on a forklift and they felt a pop in the chest area.

Employee was moving a water hose for a C/M from mud, when the employee felt pain in lower back. The injury occurred in wet & muddy conditions. X-Rays were negative and employee was diagnosed with a strain.
EE was injured after completing installation of a row of roof bolts. After completing the row, EE moved back to the center of the bolting machine. After moving back, EE inadvertently placed EE's left hand on a frame plate near the ATRS, resulting in hand placement in the pinch point of the ATRS. EE suffered soft tissue injuries only.
Hydraulic tank cover was removed with crane. The cover was suspended with lifting brackets. The technician mistakenly removed the retainer bolts for the sight glass guard allowing the cover to fall onto EE's left. The cover impacted the employees foot just behind the steel toe of EE's boot.
It was determined through a Part 50 Audit that this miner took a vacation day on 4/18/2019, that would have been reportable. We submit this in protest and to abate citation #9222852. Employee was hanging the miner cable & felt a strain in groin area.
EE was assigned to clean up a caustic soda spill. EE had EE's PPE on and was using a 1" water hose to wash the spill to a drain. In the process of washing the spilled materials, EE dropped the hose into the area, and EE was splashed on the arm and cheek. EE suffered a 1st degree burn to EE's cheek.
Employee was laying on their back replacing a fuel filter on a pick up truck. They loosed the line and gas spilled out onto their face. A small amount of gas got under their glasses and into the left eye. The eye was flushed & washed, they suffered no damage to the eye. Employee was prescribed medication that makes this event reportable.
It was determined through a Part 50 Audit that this miner had physical therapy, that would have been reportable. We submit this to abate citation #9222850. Employee was lifting a rub rail on the #4 C/M & felt pain in lower back.
It was determined through a Part 50 Audit that this miner took a vacation day on 10/2/2018, that would have been reportable. We submit this in protest and to abate citation #9222849. Employee was replacing belt rollers when the roller slipped from employee's hand and fell into mud. Mud was splashed into employee's face.
Employee was unloading overcast panels off a track car as another employee cut the banding strap and two panels caught EE's left hand resulting in a laceration that required stitches.
Employee was constructing a stopping on 5A section and as ee was applying cinder block the stopping rolled out and a block hit ee on the left leg. EE saw a Dr. and was placed restricted duty on 8/23/18.

EE was climbing down the ladder of the rock truck, when ee missed a step, causing the weight shift to right ankle on the following step.
Employee was removing a hydraulic hose from a threaded coupler joint, when the hose sprung back striking the employee in the right maxillary central incisor tooth, making the tooth loose.
While coupling two pipes together, one piece of the pipe sprung back striking the worker on the lip. Injury resulted in five sutures to the outer lip and two sutures to the inner part of the lip and one incisor tooth chipped from the top end.

Employees were attempting to break down a power screen for transport. Employee was helping to hold a cylinder up out of it's pocket and as he flagged the loader man to raise the screen, the screen slipped. It fell off of the bucket and smashed his hand between the cylinder and it's pin point.
Technician was working under a 992G. Raised up and struck his head on the hydraulic tank, causing his hard hat to drive his safety glasses into his forehead causing a laceration. Required 6 stitches to close the laceration on the bridge of his nose.
Employee was moving a piece of metal when it slipped and struck his right index finger.
