Employee was installing a tail pulley on portable screening unit when pulley slipped and pinched their fingers on their left hand.

Employee attempted to lift 150-200lb. steel plate without obtaining assistance. Employee felt pain in right bicep as they were lifting plate.

Employee received a scrape on right calf area. Employee initially refused medical evaluation. The scrape became infected which resulted in medical treatment which resulted in 3 days lost time.

Employee bent over to lift a chain from the ground when ee felt a pull on right side-groin area. EE was diagnosed with a groin sprain.

Employee was struck by a screen deck which was being lowered to the ground. The screen slid and hit the employee in the left thigh, causing a laceration requiring 5 stitches.

Employee was reaching for a piece of steel on a conveyor belt when employee felt a "pop" in right calf.

Employee stated that ee was pulling a cable when ee felt pain in right bicep. This injury was not reported to management until 10/5/16.

Employee was pulling wire thru a conduit when EE strained left elbow.

Employee came to work on a Sunday morning to turn on water pump. As employee opened the quarry entrance gate, employee felt pain in lower back. Went to doctor on 1/4/16 and was diagnosed with degenerative disc herniation. EE returned to work on 1/5/16-no work limitations. As the back pain did not improve, employee had surgery on 5/25/16.

On 12/23/15, employee stated that on 11/2/15, he was removing a grease gun coupling. When the coupling released, his left elbow struck the steel frame of the wash plant causing a elbow strain and contusion.

Employee was struck by a stone on his right lower leg that fell off a locked out conveyor causing a contusion.

Employee was cleaning build-up from pulley when his right arm was caught between the pulley and the conveyor belt causing fracture to left hand fingers, arm and scapula.

Employee was handling a 5 gallon pail of stone when he felt pain in his right elbow. He later developed an infection in his right arm.

Employee was shoveling stone when he felt a pain in his lower back. Diagnosis: Lumbar strain/sprain.
