Employee threw an empty cardboard box into a fire. Employee did not check to ensure box was empty. Box contained empty aerosol can. Can ruptured causing flash fire Flash. Fire ignited employee's nylon jacket which caused burn to employee's cheek, chin, and nose.

Employee attempted to lift 150-200lb. steel plate without obtaining assistance. Employee felt pain in right bicep as they were lifting plate.

Employee was coming down form ladder when EE reached the ground EE rolled right ankle causing a sprain

Employee tripped and fell spraining left shoulder and left middle and ring fingers

Employee was lifting rock from walkway and felt a pinch in the left shoulder

Employee caught fingers on belt drive while inspecting belt clips, causing fracture to left ring finger and laceration to left middle finger.

Employee was changing screen with fellow miners when employee felt pain in neck/right shoulder area. Employee was diagnosed with a shoulder sprain.

Employee strained right hand while using a sledge hammer to remove a fork lift pin.

Employee was removing a hydraulic filter with a wrench when EE strained lower back, causing a lumbar strain.

Employee came to work on a Sunday morning to turn on water pump. As employee opened the quarry entrance gate, employee felt pain in lower back. Went to doctor on 1/4/16 and was diagnosed with degenerative disc herniation. EE returned to work on 1/5/16-no work limitations. As the back pain did not improve, employee had surgery on 5/25/16.

On 12/23/15, employee stated that on 11/2/15, he was removing a grease gun coupling. When the coupling released, his left elbow struck the steel frame of the wash plant causing a elbow strain and contusion.

Employee was using a steel bar to clean the sides of a hopper. His right ring finger was pinched between the bar "T" handle and the edge of the hopper side, resulting in a fracture.

Employee was struck by a stone on his right lower leg that fell off a locked out conveyor causing a contusion.

Employee struck his head on side window of haul truck he was operating when it came in contact with a front-end loader causing a neck sprain and forehead contusion.

While walking, employee stepped on a rock and twisted his ankle, causing a sprain.

Employee was ascending a concrete walkway along side of the Primary Crusher conveyor belt when he slipped and fell, striking his left index finger on the steel conveyor railing. Result-laceration requiring 4 stitches.

Employee stated that when he pressed on the foot brake on his 988F loader, he felt pain in his left ankle. This was not reported to Management until 8/6/13. Diagnosis: ankle sprain.

Employee was walking down stairs at portable crusher when he hit his elbow on the hand railing causing a contusion.

Employee was shoveling stone when he felt a pain in his lower back. Diagnosis: Lumbar strain/sprain.

Employee was walking on a pile of 4 inch stone when he slipped and twisted his right knee. Injury was not reported to the Company until 07/19/2012. He was diagnosed with a meniscus tear but elected to post pone surgery until winter shut down. His first day of missed time was 1/15/13.

Employee was cutting a piece of steel in half with a torch. The cut piece fell and hit employee's left knee & thigh causing a contusion.

Employee was tightening a "J" hook during a screen change using a wrench when he strained his left shoulder.

Employee was handing up 10Ft. length of chain to fellow employee when he felt a strain in his left shoulder.

Employee was sliding an electric motor from a loader bucket onto a truck bed when he pinched his right index finger between the motor and the truck bed causing a laceration.

EE was striking a wedge with a sledge hammer. He missed the wedge and the hand holding the hammer handle struck against the wedge causing a contusion to right index finger and nail.

Employee was helping remove a crusher when he tripped & fell resulting in a strain to lower back.

Employee was removing upper engine panel on 980 Loader. After removing 3 of the 4 bolts, the panel swung down, striking the employee's left forearm causing laceration requiring 9 stitches

Employee was adjusting a conveyor belt, his wrench hit the conveyor guard, dislodging it from its hook. The guard fell, hitting the employee finger causing a contusion.

Employee was removing a pump drive & hydraulic pump from a drill rig when it fell on his right foot causing a fracture to right 2nd toe & contusion to right foot.

While another employee was lowering a screen down it slipped and hit employee on left foot causing a contusion.

Employee received an insect bite on left fore arm. After several days, the arm became swollen and infected.

Employee was cleaning dirt build-up around a head pulley using a metal bar. The bar got caught in the pulley, causing the employees thumb to get pinched by pulley head.

Employee was kicking a breaker bar to loosen a drain plug when he slipped. Employee stated he was still feeling pain in October and was sent for an MRI on 10/8/2009 which revealed an ACL Tear.

Employee and Co-worker were handing up a 95lb. screen to another emplyee. He indicated afterward that he felt pain in left shoulder.

Employee stated he had lower back pain he had a previous work related back injury in February 2008.

Employee was climbing down the counter weight ladder of a front end loader. He missed the last step and fell to the ground causing a bruised sacrum.

Employee was shoveling stone, the shovel hit something solid causing sprain to right wrist & forearm.

Employee was sitting on a bench in locker room, when he bent over to put on his work boots, he felt a pain in his lower back.

Employee was cleaning area around a feed plate using a steel bar. The bar slipped causing employee'e right hand to strike a dust suppressor fitting causing a fracture to his right ring finger.

Employee was holding a metal pipe under a rock hammer as it was being lowered into a box for shipping. As the hammer fell into box, the pipe was jerked downward causing back pain to employee.

Employee stated that when welding with MAP gas, he inadvertantly touched his shirt. He tried to put out flame with his hand resulting in burn to chest & hand.

Employee was freeing a jammed stone feeder with a pole when a 6 inch core stone was dislodged and struck the top of the employee's left hand causing an abrasion / contusion.

Employee stated that while operating front end loader he experienced back pain

Employee was driving a load of dirt up a ramp. The truck stalled and started to roll back causing the trailer to pitch sideways. The employee hit his left knee on cab door.

Employee was walking from the locker room to his work station when he stepped on a piece of core stone, twisting his right ankle.

Employee was working on a drilling crew when he complained of dizziness.

Employee was handling steel pipe when he felt a pull in his lower back.

Employee was checking circle drive on a grader that was not working properly. As the operator moved the drive from left to right, the yoke broke free and the PTO drive shaft struck the EE above his left eye causing a laceration requiring 6 stitches.

While working on a bulldozer wind blew dirt into EE's right eye causing a corneal abrasion. EE wears contact lenses.

Employee stated the use of a jackhammer and or climbing a ladder caused back strain.

While climbing into a crusher, employee's left foot slipped off the feed distributor and he struck his left knee against the side of the crusher causing a contusion to knee.

Employee was standing on a 5 gallon pail that he was using as a platform to do some welding work. He fell off causing bruising and contusion to his lower back and right knee.

Employee stated he strained his lower back while shoveling.

Employee was cleaning dirt off of drill rig skirt when dirt got into his eye.

Employee struck his elbow on a protruding bolt on a side plate frame while tightening another bolt.

EE was using a sledge hammer while changing a shaker screen. His right index finger was crushed between shaker hopper and hammer when he missed the hit pin. He received a blackened nail.

EMPLOYEE WAS CUTTING A PIECE OF STEEL WITH A TORCH. THE PIECE BROKE OFF & FELL, STRIKING EE'S FOOT. IT CAUSED A FRACTURE JUST ABOVE HIS RT. BIG TOE PAST THE STEEL TIP OF HIS BOOT.

EMPLOYEE WAS USING TWO PORT-A-POWERS AS SPACERS BETWEEN CRUSHER WALL AND SHEAVE. THE PORT-A-POWER BROKE LOOSE AND STRUCK EMPLOYEE'S HARD HAT VISOR WHICH PUSHED HIS SAFETY GLASSES INTO BRIDGE OF EMPLOYEE'S NOSE CAUSING LACERATION REQUIRING 5 STITCHES.

WHILE EMPLOYEE WAS WORKING ON A SHAKER, A STONE FELL DOWN ONTO THE SHAKER, BOUNCED UP AND STRUCK EMPLOYEE ON RIGHT SHOULDER BLADE CAUSING A CONTUSION.

EMPLOYEE WAS HOLDING A CRUSHER WEDGE DOWN WITH A PRY BAR WHEN HE WAS HIT IN THE ABDOMEN WITH A SLEDGE HAMMER BY A CO-WORKER WHO WAS HITTING THE WEDGE. THE HAMMER GLANCED OFF THE WEDGE & HIT HIM CAUSING A STOMACH CONTUSION.

While setting a steel plate into proper position, employee pushed from an awkward angle and felt a twinge in his lower back. He was diagnosed to have a lower back strain.

Employee slipped and fell on icy walkway, causing a pulled muscle in his lower back.

Ee was hit in mouth by a come-a-long handle that he was standing near, which shifted causing ee to cut his lip & chip his front teeth.

EMPLOYEE WAS LIFTING A DRILL BIT OUT FROM A SHARPENING HOLDER. HE LIFTED THE BIT FREE & ITS MOMENTUM CAUSED IT TO STRIKE EE IN THE CHIN, RESULTING IN A LACERATION (3 STITCHES).

EMPLOYEE WAS KNOCKING PISTON OUT OF AN ENGINE WITH A HAMMER. THE PISTON CAME OUT AND PINCHED EMPLOYEE'S FINGER BETWEEN PISTON AND EDGE OF CRANKSHAFT CAUSING LACERATION REQUIRING 3 STITCHES.

EMPLOYEE PINCHED FINGER BETWEEN THE TWO HALVES OF A MOTOR BASE. HE HAD JUST CHANGED THE V BELTS ON THE NO 1 SHAKER AND WAS PIVOTING THE MOTOR BASE BACK IN PLACE. HE RECEIVED 7 STITCHES.

EMPLOYEE WAS PICKING METAL / GARBAGE OFF A CONVEYOR BELT. HE TOOK HIS SAFETY GLASSES OFF TO CLEAN THEM AND FELT SOMETHING GO INTO HIS EYE.

EMPLOYEE WAS CHIPPING SLAG OFF A WELD WHEN A HOT PIECE OF SLAG GOT INTO HIS EYE. EMPLOYEE STATED HE WAS WEARING A FACE SHIELD.

EE WAS HOSING OFF WINDSHIELD OF DRILLING MACHINE. A PARTICLE OF STEELE THAT MUST HAVE COME FROM EITHER THE DRILL BIT OR STEELE CASING BLEW INTO HIS EYE CAUSING IRRITATION.

WHILE CLIMBING DOWN FROM THE BACK OF THE WELDING TRUCK, THE EE'S FOOT LANDED ON A SMALL ROCK, TWISTING HIS RIGHT ANKLE.

WHILE CUTTING SPRING BOLTS, EMPLOYEE WAS STRUCK BY A SPRING BASE WHEN THE BOLT CAME OFF.

EMPLOYEE WAS WALKING TO HIS CAR WHEN HE SLIPPED ON ICE.

WHILE KNOCKING OUT A BOWL LINER WITH A STEEL SHAFT, THE SHAFT SLIPPED AND PINCHED THE EMPLOYEE'S FINGER BETWEEN THE SHAFT AND THE BOWL CAUSING A LACERATION REQUIRING 5 STITCHES.

EE WAS CUTTING OFF A BOLT FROM A HEEL PLATE WHEN A SPARK GOT BEHIND HIS WELDING GOGGLES, BURNING HIS RIGHT EYE.

WHILE CUTTING A MANTLE WITH A TORCH, A PIECE OF HOT SLAG GOT INTO WELDER'S GLOVE, BURNING HIS HAND.

AFTER TWO DAYS OF SHOVELING EMPLOYEE STATED HE FELT PAIN IN LOWER BACK.

WHILE CUTTING METAL BLOCK OFF A TRUCK CAB, A PIECE OF HOT SLAG BURNT THRU EE'S PANT LEG AND DROPPED DOWN INTO HIS WORK BOOT CAUSING BURN TO ANKLE.

EE WAS USING A SLEDGE HAMMER TO REMOVE A HYDRAULIC CYLINDER WHEN HE STRUCK HIS HAND WITH HAMMER.

WHILE SHOVLING SCREENING MATERIAL, EE FELT PAIN IN LOWER BACK.

EE WAS USING A CHISEL & HAMMER TO CUT A BROKEN BOLT. A PIECE OF THE BOLT HEAD BROKE OFF, STRIKING EE IN LT. FOREARM.

WHILE CLEANING DIRT OFF SHAKER EMPLOYEE SLIPPED STRIKING HIS SIDE AGAINST SHAKER SCREEN.

WHILE EMPLOYEE WAS PULLING A NEW PULLEY UP THE 11/2" CONVEYOR HE SLIPPED DOWN TWO STEPS AND FELL AGAINST THE HAND RAIL.

WHILE REMOVING TAIL PULLEY #4 CONVEYOR, EMPLOYEE HAD HIS RIGHT HAND PINCHED BETWEEN PULLEY SHAFT AND ANGLE IRON.

EMPLOYEE WAS WELDING NEXT TO ANOTHER WELDER. WHEN FIRST EMPLOYEE REMOVED HIS SHIELD HE WAS FLASHED BY ANOTHER WELDER, RECEIVING FLASH BURN TO BOTH EYES.

EMPLOYEE WAS REMOVING BOLTS FROM A D8 BLADE. HE WAS HOLDING THE BOLT HEAD WITH HIS LEFT HAND WHILE USING AN IMPACT WRENCH ON THE NUT. THE BOLT HEAD SPUN CUTTING EMPLOYEE'S LEFT PALM THROUGH WORK GLOVES, 4 STITCHES PLUS TETANUS SHOT.

EMPLOYEE WAS HOLDING A LARGE WRENCH WHILE FELLOWWORKER STRUCK IT WITH A SLEDGE HAMMER TO LOOSEN A BOLT. THE FELLOW EMPLOYEE MISSED THE WRENCH AND HIT EMPLOYEE IN THE HIP AND ABDOMEN CAUSING CONTUSION AND BRUISES.

EMPLOYEE WAS PUSHING A PIECE OF STEEL RAFTER AFTER HE HAD CUT IT WHEN HE FELT A POP IN HIS LEFT ELBOW.

WHILE CLOSING DOOR ON EXCAVATOR EMPLOYEE'S FINGER WAS CAUGHT BETWEEN DOOR AND DOOR FRAME, SMASHING FINGER.

WHILE REMOVING A PIN FROM A LOADER BUCKET, THE PIN PINCHED EMPLOYEE'S LEFT HAND BETWEEN PIN ANDPIN HOLE.

WHILE EMPLOYEE WAS LIFTING A 45LB. STICK OF EXPLOSIVE INTO THE BLASTING TRUCK HE FELT PAIN IN HIS LEFT ARM.

WHILE OPERATING JACK HAMMER, THE EMPLOYEE'S HANDSLIPPED AND WAS CUT BY WIRE COUPLING HOLDING THEAIR HOSE TO THE JACK HAMMER.

WHILE DRIVING DOWN HILL IN THE KINGSTON QUARRY, THE CAB OVER PITCHED FORWARD CAUSING EMPLOYEE'S PELVIC AREA TO STRIKE THE STEERING WHEEL.

WHILE ATTACHING A CABLE TO A BUCKET, EMPLOYEE WAS STRUCK ON ARM WHEN BUCKET MOVED. EMPLOYEE HAD GIVEN THE WRONG HAND SIGNAL.

WHILE REMOVING IDLER ON PRIMARY CRUSHER CONVEYER, IDLER FELL OVER, PINCHING FINGER BETWEEN IDLER AND CONVEYER.

WHILE ADJUSTING A CRUSHER, EMPLOYEE PINCHED THUMB BETWEEN WRENCH AND SIDE OF CRUSHER.

EMPLOYEE WAS MOVING A REFRIGERATOR ON A HAND TRUCK WHEN THE REFRIGERATOR STARTED TO FALL. EMPLOYEE GRABBED SIDE AND IT CUT HAND ON EDGE CAUSING LACERATION TO LEFT MIDDLE FINGER CAUSING 14 STITCHES.

WHILE EMPLOYEE WAS PUTTING A PIN IN A SCREEN, THE SCREEN SHIFTED, PINCHING FINGER.

EMPLOYEE STATED THAT WHILE DRIVING A HAUL TRUCK HE FELT BACK PAIN.

WHILE INSTALLING A SHAKER BEARING, EMPLOYEE WAS HIT IN THUMB WITH A PIECE OF BROKEN BEARING.

WHILE TIGHTENING A 4 FT CRUSHER, EMPLOYEE STRUCK THUMB WITH SLEDGE HAMMER.

WHILE CLEANING STONE OFF CONVEYOR BELT, EMPLOYEE FELT TWINGE IN RIGHT HIP AREA.

EE WAS COILING WIRE CABLE WHEN THE ROLL STARTED TO TIP OVER, EE TRIED TO KEEP IT FROM FALLING OVER.

EE SLIPPED ON ICE.

WHILE PULLING A HYDRAULIC JACK, EE SLIPPED ON ICE.

WHILE RE-ASSEMBLING ENGINE ON 245 BACK HOE THE WIND BLEW THE ENGINE COMPARTMENT DOOR SHUT, PINCHING EE'S FINGER BETWEEN DOOR AND DOOR FRAME.
