Employee was attempting to widen the forks on a telehandler. During the process, one of the forks slipped from EE's hand and pinched EE's left hand between the fork and the mast, resulting in a laceration to EE's thumb and forefinger which required sutures.
***summarized from attached letter*** Employee was lifting rebar in conjunction with other employees. In lifting and/or moving the rebar, the employee either injured or re-injured their back. Employee was running the miner on #6 Section in the #4 LOXC, when a rock EE loaded on the car rolled off the coal in the shuttle car and struck EE's right foot causing a fracture to the top of EE's foot.
Accident was not observed by any witness and injured employee does not remember circumstances of the accident. Injured employee has a broken nose, received stitches and staples and had bruises.

The employee was in the process of unstacking wall girts when the incident occurred. As EE was lifting one off the stack, EE rolled it over and the sharp edge of the girt lacerated EE's right arm, 4 inches below the wrist. The EE received first aid treatment and was taken to a local Urgent Care where EE was evaluated, treated, and released to return to work with no restrictions.
Employee was sliding on EE's back to exit shield #145 after finishing repairs when EE's feet got caught on the pontoon and the weight of EE's body while sliding pushed EE's knees under EE causing a sprain to EE's right knee. After further evaluation, EE had a torn posterior horn medial meniscus which required surgery on 9/30/2024. (Started losing time 9/30/2024)
Employee was carrying oxygen bottle when they tripped on a piece of rail, twisting their right ankle causing injury.
Employee was moving a rock off the handrail at Shield #175 when EE's hand was caught with another rock, resulting in a fracture to left middle finger and sutures to left hand.

REPORTABLE ONLY DUE TO SUTURES - Employee was washing down on the offside of the slope belt, when EE picked up a rock to move out of the way and caught the right index finger between the rock and rib causing laceration that required sutures.
REPORTABLE DUE TO FRACTURE - Employee was replacing a top roller when the roller rolled off the belt and smashed EE's right thumb between the two rollers causing a fracture to EE's thumb. (Employee did not receive medical treatment until 5/2/2022)
Employee was struck by slider rail when the slider rail that was hung from a red hanger off the ground came loose and employee tried to catch the rail causing a contusion/hyperextension to right shoulder. (Started losing time on 3/8/2022)
Employee was lowering the cover down over the engine compartment of a Lo-Trac when EE felt a sharp pain in EE's left shoulder. EMPLOYEE HAS BEEN WORKING SINCE THE DATE OF ACCIDENT UNTIL 1/31/22, WHEN EE WAS TAKEN OFF WORK DUE TO CONTINUED PAIN IN EE'S LEFT SHOULDER.
REPORTABLE ONLY DUE TO SUTURES - Employee was working on East C belt header. When EE went to remove dirt from receiving end of bracket, the dirt became dislodged and the wiper fell catching EE's left middle finger between the ITC blade and drip pan causing laceration and fracture to tip of finger, requiring sutures.
