Employee believes employee wiped something into eyes when wiping the sweat away from eyes. Employee was wearing Safety glasses and also had a welding hood on while grinding. Employee had irritation to both eyes and received a prescription while there was no particles or abrasions to the eyes.
The injured employee was reconnecting a 8 inch aluminum vacuum pipe that had been clogged. When connecting the pipe the stored energy cause by the 6 inch flex attached to the 8 inch pipe spun out of the employee's hand. When the pipe left EE's hand it cut EE's right middle finger.
EE was performing maintenance when left thumb was caught between a metal screen cloth and a handrail, sustaining a laceration.
Employee had right forearm mashed between two pieces of metal during plant maintenance causing a contusion.
Employee felt pain in lower back while using a water hose to wash out spilled material during cleanup.

While breaking the studs off of a belt fastener the worker sustained a laceration to the top part of wrist requiring 4 stitches.
While loosening the fill plug on the final drive of an excavator, the hot drive oil sprayed on the employees right hand resulting in burns that required medical treatment.
While grabbing onto a metal screen to remove it from the forks of the lull the screen shifted and its edge cut through gloves and caused a laceration into the palm of right hand requiring 4 stitches.
Employee was collecting tools and rolling up welding leads onto the maintenance truck when work was completed. The automatic reel began to speed up and the employee instead of letting go of the cable, grabbed the lead near the end causing the quick connect end to whip back and strike EE on the right wrist. It was confirmed that a bone in EE's right wrist had been fractured.
While lifting a spool of wire into the bed of a pickup the employee felt a pop/burn feeling in left forearm.
EE and 2 other employees were in the bone yard cutting a section of belt for splice. End of belt was buried and as the 3 employees pulled the belt, the EE felt a strain in stomach. EE did not seek medical treatment until 3/15/21. EE was diagnosed with a hernia. Injury is being reported within 10 days of EE seeking medical treatment and being diagnosed with hernia.
Injured employee was assisting with changing shim plates in the primary jaw crusher. As EE lifted the shim plate EE strained back. EE was taken for medical evaluation where EE was diagnosed with a thoracic strain and placed on restricted duty.
EE WAS ASSISTING IN THE MOVING OF A METAL SIGN POST EMBEDDED INTO A 2'X 2'X 2' CONCRETE BLOCK WHEN IT WAS TIPPED FORWARD FROM A SKID STEER LOADER BUCKET STRIKING THE EE IN THE FACE CAUSING A LACERATION/AVULSION TO NOSE.
