EE WAS HELPING UNROLL A 700' ROLL OF MINER CABLE THAT WAS SUPPORTED WITH A 5' PIECE OF PIPE LOCATED BETWEEN THE FORKS OF A FORK LIFT. THE PIPE FELL OFF THE FORK CAUSING THE SPOOL TO FALL AGAI NST THE LEFT FORK WHERE EE'S RIGHT INDEX FINGER WAS LOCATED.
EE WAS ASSISTING THE CREW IN THE CHANGING OF THE GEARCASE ON THE JOY 14CM10 CONTINUOUS MINER WITH REMOTE IN THE #5 ENTRY, SPAD #1722, OF EAST MAINS WHEN HIS RIGHT FOOT WAS PINCHED BETWEEN THE RIPPERVEYOR SPROCKET AND GEARCASE OF THE MINER, RESULTING IN TWO BROKEN BONES IN HIS RIGHT FOOT.
AS THE CONTINUOUS MINER WAS REMOVING A PILLAR, A PIECE OF DRAWSLATE FELL ON THE FRONT. THE FOREMAN HAD THE MINER OPERATOR TRAM THE MINER BACK OUT & HE & THE OPERATOR REMOVED THE ROCK OFF THE HEAD OF THE MINER. THE DRAWROCK HAD BROKEN INTO THREE OR FOUR PIECES & AS FOREMAN WAS SLIDING 1 ROCK OFF MINER HE HIT ANOTHER PIECE OF ROCK THATHAD A SHARP EDGE CAUSING A LACERATION TO FINGER.
EMPLOYEE WAS DISCONNECTING 6" WATER PIPE AS SECTION IS RETREATING AND A SHE WAS PULLING ON TEH PIPE HE FELT SHARP PAIN IN HIS BACK. HE CONTINUED TO WROK BUT ON 5-10-00 HE WENT TO DR AND THEY PUT HIM IN THE HOSPITAL FOR TRACTION.
