A ROOF FALL OCCURED IN AN INTERSECTION AT STATION #328 ON #1 BELT. THIS FALL WAS 20' LONG BY 18' WIDE BY 6' HIGH. THE AREA WAS REBOLTED, SCREENED AND CLEANED UP.

After bolting a cut through employee noticed a loose rock and proceeded pulling the rock down with a slate bar. In doing so he inadvertently dislodged another rock causing it to fall pinning him down. The rock was removed by other employees present at the time. He was then given first aid and transported to the surface where EMS personnel took over.

A roof fall occurred on a roof bolting machine in a fully supported crosscut. The fall was 80' long, 18.5' wide and 5' high. The area had been bolted with 5/8" x 4' fully grouted resin bolts. The fall was in crosscut 3-2 on the 0010-0 section.

A roof fall was discovered in #8 entry 15' inby sta #80 in the return on the 001-0 section. The fall was 16' wide, 70' long and 6' high. The area had been bolted with 5' resin bolts.

A roof fall was discovered in #9 entry 17' inby station #103 in the return of the 001-0 section. The fall was 50' long, 16' wide and 6' high.

While doing retreat mining on block #316, one block inby spad #1852, the continuous mining machine had a roof fall on it before setting it in the clear. It covered approx. 20 feet of the machine, 10 feet high.

Employee was tramming miner out of a cut and was struck at knee level with miner boom knocking him to pavement.

Weak rock strata in intersection where roof fall occurred.

Going across parking lot, jumped across a ditch & landed on side of left foot & felt it pop.

28' long X 18.5' wide X 6' high. Location: #7 heading 15' inby spad #1590. Initially reported to federal on 12-9-07 by phone.

Employee was prying on a pot on a continuous miner when he felt pain in his lower back and left hip.

70' long X 18' wide X 5' high. Reported by calling 800-746-1553 @ 2:42am.

MSHA inspector found while inspecting mines air courses. It was in #9 heading intake along #3 belt at spad #1014 5 south mains. The fall was 18' long, 18' wide and 5' high.

Roof fall found while firebossing the mine. It was in 5 South Mains No.5 & No.6 headings, spad #74 and spad #80. The fall was 80' long, 18' wide, and 5' high.

Employee was in a standing position when a piece of rock 4' x 2' x 1/2' fell from between roof bolts striking him on the right shoulder and lower neck.

Employee was in a standing position prying a rock off the continuous miner when a piece of rock broke off striking him in the right ring finger causing two lacerations.

Roof fall of #1 entry of Old Section, Spad No. 599

Employee was in a standing position using a mechanical lifting jack to spread the sides of the miner boom. As he operated the lever on the jack, the jack dislodged and struck him on the right knee.

While the fireboss was making his pre-shift examination, a roof fall was discovered in 4 North Mains. The fall was located between No. 5 and No. 6 entries in the crosscut near spad No. 832. The fall was located 500 feet about the working section and it did not affect travel or ventilation.

Employee was trimming a piece of conveyor belt being used as a skirt at the radial stacker tail when he accidently cut his left thumb with the utility knife blade.

Employee was taking the sequence switch from the top of the feeder when it slipped out of his hand striking his right index finger.

Employee was in a standing position lifting on a belt rope trying to level the belt when he slipped and fell causing pain in his right knee.

Employee was in a standing position blocking roof bolts when a piece of rock 3'x1 1/2'x6" fell from the top striking him on the side of his left leg.

Employee was walking to get a timber when a peice of rock 3'x5'x1' x 0 fell from the roof striking him on his left shoulder, back and left leg. Employee continued to work light duty until 10/25/2005, the date of his operation.

EE was attempting to lift high voltage cable into a sled when he began having trouble seeing out of his right eye.

Intake entry at #2 block along #2 belt between stations #58 and #64. Fall was 70' long, 18' wide and 4 1/2' high.

Belt entry in #1 block, 150' from portal, fall was 60' long, 18' wide and 5' high.

EMPLOYEE WAS USING A PICK HAMMER TO OPEN ON A BELT SPLICE. HE USED A SECOND HAMMER TO DRIVE THE TIP OF THE HAMMER INTO THE CLIP, WHEN A PIECE OF THE HAMMER BROKE OFF & LODGED IN HIS LEFT WRIST.

Employee was walking around #4 belt tail piece when he tripped over the anchor pin or turnbuckle causingm him to fall to the mine floor causing pain in his back. Employee quit working as of March 29, 2005.

Employee injured his back loading posts by hand into the bucket of a scoop. He finished the shift and continued to work every day until 6/30/05. On 6/30/05 he was operated on for a lumbar disc herniation and/or lumbar stenosis.

Roof Fall in intersection of #9 entry of 1 Left off Northeast Mains, one block inby spad #869. Fall was 18' long, 12' wide and 8' high. Fall Was discovered by fireboss prior to working on Sunday night.

Employee broke a drill steel off in the top, leaving approximately 8 inches of the steel handing out. After drilling another hole for the roof bolt, he caught his left index finger between the roof plate and the broken drill steel while installing a roof bolt.

ROOF FALL IN #6 ENTRY OF NE MAINS ON BELT ONE BLOCK INBY SPAD #1711. FALL WAS 38'LONG X 18'WIDE X 5'HIGH. FALL WAS DISCOVERED BY FIREBOSS PRIOR TO WORKING ON SUNDAY NIGHT.

EE WAS TAKING A CAP OFF THE HEAD OF A MINER IN THE SHOP, WHEN IT SLIPPED OFF & PULLED ON HIS LEFT ARM. HE DID NOT MISS ANY WORK UNTIL 10/25/04 WHEN THEY OPERATED ON A TORN MUSCLE IN LEFT ARM

EE WAS TRAVELING THE RETURN AIR COURSE, A ROOF FALL WAS ENCOUNTERED. IT WAS LOCATED IN THE INTERSECTION OF #1 ENTRY OF NORTHEAST MAINS IN THE REDSTONE SEAM SPAD #1727. IT WAS APPROX. 18'WIDE , 30'LONG, AND AT LEAST 6'HIGH. NO INJURIES.

ROOF FALL OCCURRED IN #3 ENTRY AND INTERSECTION, TWO BLOCKS LEFT OF SPAD #257 ON THE 1 LEFT SECTION. THE FALL WAS APPROX. 80 FT LONG, 12 FT WIDE, AND 6 FT HIGH.

EE WAS STANDING IN THE INTERSECTION AS THE SCOOP WAS TRAMMING OUTBY WHEN THE SCOOP SLID INTO A HOLE IN THE ROADWAY & CAUGHT HIS RT.FOOT BETWEEN SCOOP BUCKET & ROADWAY PAVEMENT.

WHILE WORKING ON #4 BELT, 2 WORKERS WERE LETTING OFF THE COME-A-LONG WHEN THE INJURED GOT HIS FINGER CAUGHT BY THE COME-A-LONG.

FALLS IN BLEEDER ENTRIES OF BARRIER BETWEEN TOP END OF 7 LEFT OFF 1 RIGHT AND 6 LEFT OFF 1 RIGHT SECTION.

EE WAS HELPING UNROLL A 700' ROLL OF MINER CABLE THAT WAS SUPPORTED WITH A 5' PIECE OF PIPE LOCATED BETWEEN THE FORKS OF A FORK LIFT. THE PIPE FELL OFF THE FORK CAUSING THE SPOOL TO FALL AGAI NST THE LEFT FORK WHERE EE'S RIGHT INDEX FINGER WAS LOCATED.

EE WAS ASSISTING THE CREW IN THE CHANGING OF THE GEARCASE ON THE JOY 14CM10 CONTINUOUS MINER WITH REMOTE IN THE #5 ENTRY, SPAD #1722, OF EAST MAINS WHEN HIS RIGHT FOOT WAS PINCHED BETWEEN THE RIPPERVEYOR SPROCKET AND GEARCASE OF THE MINER, RESULTING IN TWO BROKEN BONES IN HIS RIGHT FOOT.

WHILE MAKING THE WEEKLY EXAM OF THE BLEEDER ENTRY IN BLEEDER ENTRY IN 5 LEFT OFF 1RIGHT, A ROOF FALL WAS ENCOUNTERED IN #1, #2, AND #3 INTERSECTION NEAR STATION #1532. APPROXIMATE LENGTH: 125' , WIDTH: 18' AND HEIGHT: 6'.

EE WAS SWINGING A BOLTER WRENCH WITH HIS RIGHT HAND STRIKING AGAINST A PLUGGED UP BOLTER STEEL, WHEN IT GLANCED OFF AND HE STRUCK HIMSELF WITH A WRENCH ABOVE HIS RIGHT EYE.
