Setting a transmission in a 777 - boom kicked out, extended boom so it would work then transmission started to swing and while trying to keep boom from swinging with right hand felt pain in right shoulder - strain to shoulder.

Employee pulled a muscle in his back while lifting his hood during pre shift inspection. Note: Filing solely to meet deadline regulations. Incident is under investigation.

Employee was investigating a truck fuel leak when his left knee contacted the turbo causing a burn. Employee did not report incident until 08/25/2014 and refused medical attention. Because of no medical attention or restricted duty, it was considered a non-reportable. Employee requested medical attention on 09/04/2014 and was taken to the doctor on 09/05/2014.

Welder was working on tracks for a D10 dozer when a seal blew out of track pin and oil struck welder in the face

Miner was removing a sensor above his head when he felt a pain in his neck

Parts runner was attempting to remove parts from bed of pickup and strained his shoulder.

Mechanic was replacing hydraulic pump on bulk truck, pump slid off of pto shaft striking employee on right forearm requiring 6 stitches.

Victim felt a pain in his left shoulder while dropping miner cable. He was taken to a local ER where he received an X-ray and was released. He hasn't returned to work as of 5-3-13.

Employee was changing fuel filters on 980C End loader when diesel fuel splashed into his eye.

Employee was helping remove a hydraulic cylinder from a storage rack and as he reached to remove a block of wood from in front of the cylinder, the cylinder shifted & slid on the forks of the forklift and struck him behind the left knee causing laceration & bruising.

Employee attempted to lift a piece of metal that was obstructing his path to his tool box. In doing so, he strained his back and finished his shift. He continued to work for the next couple of days before deciding to seek medical treatment and is under care of his family physician and anticipates returning to work within the next few days.

A piece of metal slid against victims right ankle, fracturing his lower leg, just above his ankle. He was treated and released at a local hospital and is scheduled for surgery in the near future and is expected to make a full recovery.

Mechanic was removing the wheel cover from a loader when the cover slipped contacting the middle and ring fingers on his right hand and causing a laceration requiring 6 stitches on his middle finger.

Employee received a cut to his right hand from a nylon rope hanger while dropping the miner cable. Stitching was required.

Mechanic replaced valve hoses of lifting jacks on rock truck. Lifting jacks on rock truck not completely filled with oil and released truck bed when the mechanic attempted to lower it.

While setting up the wet hole trailer, I lifted a crib block & threw it under the trailer.

Miner pulling cable across beltline when end piece cathead caught on belt. When end piece release it struck him in the face causing bruising, fracture on nose and laceration requiring two staples.

Foreman was attempting to move track beam (uninstalled) from roadway. Track slid into another track beam and caught his index finger between the two beams. Received four stitches from pinch.

Roof Bolter was trying to pull a bent roof bolt from the roof when it suddenly came loose and contacted his mouth. He was taken to the hospital, recieved three stitches and returned to work.

Mechanic was installing a new heater hose on a Cat D9R dozer. While moving the hose through a tight spot he received a laceration on the back of his left hand. He was taken to the hospital where it was glued back together.

While opening the magazine door the top lock stuck EE pushed on the top of the door and bumped the bottom of the door with his left foot at the same time He then felt a pain(burning) in his lower abdomen/groin area and it began to swell The hospital released him back to work and had a checkup appt. on 9/1/11 at which it was determined to be a Hernia requiring surgery to repair

Outside man attempting to hook tow straps on the fork of a fork lift when his right thumb became pinched between the strap and fork causing a pressure tear.

Shuttle Car operator assisting in maintenance on CMMU 001 pinched little finger on right hand while lowering the CMMU 001 cover. Received two stitches and returned to work the following day.

Mechanic moving pins for a loader into storage, picked up a pin (approx 45 lbs) and strained his back while moving. Sought medical attention from family physician.

Operator/Fueler was attempting to put antifreeze into equipment, when he missed the opening for the radiator and the antifreeze splashed on him.

Operator was dismounting and mounting a Volvo articulating truck throughout his scheduled shift. Later in the evening, EE noticed pain in his knee and notified supervisor via text message that evening and sought medical tratment the following day.

Operator was climbing down a ladder on the side of his loader when he felt a strong pull in his leg. Pain continued and operator was transported to the hospital for an examination and Dr. review.

While working as a Greaser, tossed brake drum and something flew into his right eye.

While cleaning up maintenance yard, Injury lifted a piece of metal and felt pain in his groin area. No medical treatment or missed time. It was reported as an incident only. However, it appears that a hernia has resulted in this strain and he just sought medical treatment on 10/3/09.

Employee tripped entering dozer and caught himself by grabbing handrail to keep from falling. He twisted his left arm.

EE WAS REMOVING DEBRIS (BIG ROCK) FORM THE ROADWAY, WHEN HE FELT PAIN TO HIS LOWER BACK.

TAKIN GPIN OUT OF EXCAVATOR PIN DROPPED CAUGHT EE'S FINGER.

PULLING CABLE OFF THE WENCH ON THE DOZER. PAIN TO LOWER BACK.

PUT FAN BELT ON ROCK TRUCK. HAND SLIPPED CUT THUMB ON BELT PULLEY.

EMP WAS LIFTING A TRACK SEGMENT WHILE REPAIRING A D11 DOZER WHICH INJURED LOWER BACK. MISSED 3 DAYS THEN REPORTED BACK TO WORK ON 3-29-97. LEFT AGAIN WITH BACK PAIN ON 4-26-97.

EE WAS LIFTING A TRACK SEGMENT. WHILE REPAIRING A D-11 DOZER, AND INJURED HIS LOWER BACK.

UNDER TRUCK, KNOCKED LOOSE MATERIAL INTO EYE. EE CLEANED EYE OUT. WENT TO DOC. SAT 11/30/96. GOT TWO PIECES OF METAL OUT OF EYE. WENT TO ER TO REMOVE PATCH. WENT TO EYE SPECIALIST 12/2/96, REM OVER RUST FROM EYE.

EE WAS CLEANING THE THREADS ON A 1 1/4" BOLT, WITH A RAG AND GOT A BURR FROM THE BOLT THREADS LODGED IN THE PALM OF HIS LEFT HAND.

THE AUGER CREW WAS RETRACTING AUGER FLIGHTS END EE WAS STACKING FLIGHTS THE STACK BECAME UNEVEN & THE FLIGHTS BEGAN TO FALL AN AUGER FLIGHT CAME FROM THE MIDDLE OF THE PIE STRICKING HIM ON THE FOOT HE WAS THEN TRANSPORTED TO THE HOSPITAL WHERE THEY TOOK XRAYS OF HIS FOOT DETERMING IT WAS BROKE

UNHOOKING THE CABLE FROM THE MACHINE WHEN HIS HAND CAME ACROSS THE CABLE AND CUT THE INSIDE OF THE RIGHT HAND.

BENT OVER TO PICK UP PUSH BEAM LOCKS & HURT BACK

EMPLOYEE WAS LIFTING ON A SECTION OF PIPE & STRAINED HIS LOWER BACK

HE WAS OPERATING A 45 DRILLER WHEN ONE OF THE OUTRIGGER PADS FELL OFF, WHICH WEIGHED ABOUT 100 LB. HE WAS ROLLING IT OUT OF THE WAY. HE SAID HE PULLED SOMETHING OUT IN HIS BACK. HE NOTIFIED HI S FOREMAN ABOUT 3 HR LATER HE HAD HURT HIS BACK.

EMPLOYEE WAS MOVING PARTIAL BARREL OF FLOCCULANT (55 GAL) AWAY FROM THE FLOCCULENT TANK.

EMPLOYEE WAS HELPING LIFT A ROLL OF BELT UP INTO HANGERS TO CHANGE OUT BELT. WHILE LIFTING BELT IN PLACE, STRAINED BACK.

EMPL PUTTING DRIVE CHAIN ON PRIMARY WASHER.MOVING SLACK UP ON DRIVE SPROCKET,DRIVE CHAIN CAUGHT FINGER BETWEEN SPROCKET AND CHAIN 100-2 CHAIN.

EMPL SHOVELING & CLEANING TAILPIECE ON REFUSE BELT.

SHOVELING REFUSE TAIL PEICE. WHILE SHOVELING REFUSE STRUCK TO SHOVEL WHILE TURNING TO DUMP, STRAING BACK.

EXITING DOZER PUT TO MUCH PRESSURE ON HIS WRIST AND STRAINED IT
