A contract employee was positioned near the rib, when the corner broke off and fell and struck a timber. The timber then fell and hit the employee in the left shoulder, nose, and mouth. This resulted in a bruised left shoulder and a cut to the lip requiring 4 sutures.

Employee was descending the small WA70 Loader and slipped due to wet conditions and fell to the ground. Employee started missing work 10/30/2020.

Employee tripped hitting EE's knee on mine floor. Employee was transported to get x-rays and they was negative. EE received a full release. Two shifts later EE went back and was referred to a specialist, on March 23rd EE was diagnosed with a torn meniscus. Due to covid-19 and furloughed we are just returning since March 20th.

condition was damp. area was 19 feet wide and 6 feet 10 inches high. EE, and 2 others, was stacking a brattice when another man threw a block into work area while EE was picking another block striking EE in the right index finger resulting in an impact laceration. EE finished the shift. Went later to the Emergency Department and received 5 sutures.

Employee was using a slate bar to remove a rock from the Right CM on 006 MMU. While pushing down on the slate bar to slide the rock off the slate bar slipped out of employee's hands causing the bar to hit employee in right eye.

Employee was lifting oil can off of maintenance ride and felt a sharp pain in lower back.

Move crew foreman trimming belt skirting out of section tail piece with a razor knife when the channel locks slipped causing both hands to cross resulting in a 2 inch laceration on top of left hand.

Employee was installing a conveyor motor on a shuttle car. While aligning the motor, the chain ratchet slipped causing motor to hit frame of shuttle car and roll out on employees right foot. Employee was diagnosed with a fracture to the right third toe.

Employee entered track motor and was sitting down to operate when EE felt a sharp pain in lower part of left back.

Employee was installing a supply insert to connect a sled to track motor, when guiding insert into holes, the insert kicked and pinched employee's right hand between insert and metal on motor. Employee received 5 sutures.

Employee was installing a rib bolt and slipped, which caused employee to fall off operator platform landing on the mine floor. Employee received bruising to lower back and pelvis.

Employee was straightening a bent 5' and 5/8" diameter resin bolt when EE felt a pop in the back of the neck and pain down the arm.

Employee was drilling the mine roof and was inserting another piece of drill steel, pushed it into the top and strained shoulder. Employee started missing work 3/6/2018.

Employee was using a hand grinder and got a piece of grit/metal in eye.

Employee was stacking a solid block brattice and as they bent over to get another block, the last block placed fell off and struck their left hand/ring finger.

Employee was on a ladder, building a stopping when the stopping fell and knocked ladder over causing employee to twist ankle and chipped a bone in the left ankle. Employee worked on it and later went to the Dr. and missed 2 days.

Employee was installing rib bolts and slipped and fell off bolter platform and hit back on drill mast. Bruised back.

Employee was using a slate bar to get a rock off of the continuous miner and ee slipped and caught finger between the bar and frame of miner. Sutures were required. No lost time.

Employee was pulling timber into position with a pick hammer and strained themselves. Has swelling in the groin area. Employee started missing work on 6/1/2017.

A piece of rock from a rib corner struck the employees forearm resulting in employee getting 10 sutures.

The continuous miner operator stepped across the (MRS)Mobile Roof Support trailing cable just as the MRS operator activated the cable reel and the trailing cable knocked the miner operator off his feet and he fell on his right side fracturing four (4) ribs.

Employee was making clean-up run in the # 4 right when a piece of draw rock fell striking the continuous miner and breaking up and striking the employee in the left ankle causing bruising.

Employee was preparing to pull a piece of draw rock when a piece of draw rock fell and struck the employee injuring his left wrist, and nose and causing fractures to his right leg, right hip and left heel

Roof Bolter was getting ready to bolt #4 left this was the 1st cut out of the break. Co-worker was using the inch tram to set up for a corner bolt when he activated the tram he ran over employee's left foot. He was transported to Mcdowell ARH hospital where he received x-rays which were negative for broken bones, but the toe nail of the 2nd toe was torn off.

Employee was walking between the # 3 and # 4 entries when stepped on an uneven surface and rolled his right ankle. he was transported to St. Joseph Martin Hospital he received X-rays that were negative for any broken bones, he was diagnosed with a sprain and released same date

Employee was carrying 3 rib angle brackets across his shoulder when he stumbled and fell. Employee was complaining with lower back pain, he was transported to ST. Joseph Martin Hospital he was given a cat scan which was negative. He was transferred to Pikeville medical Center and had an MRI and diagnosed with a hernia to the left testicle.

Employee was setting up to install rib bolts in the # 4 heading when he got off the drill station platform on to the mine floor he lowered the platform on top of his left foot causing bruising to the top and bottom of his left foot.

Employee was operating a Fairchild scoop to clean ribs in the # 9 entry when he said the scoop bucket caught the coal rib and caused him to be bounced into the operators canopy hitting his head. employee was treated and released at ST. Joseph martin hospital and was diagnosed with neck spasms.

Employee was moving the continuous miner cable out of the haul road with the help of shuttle car operator when he said he felt pain in the back of his left leg. Employee continued to work until 9-10-2014 when he said his leg was still bothering him and he sought medical attention at Williamson ARH Hospital where he was diagnosed with a pinched nerve.

Employee had pulled the bolter up to get the last row of bolts installed when a piece of rock fell between the two inside bolts landing on the A.T.R.S and then sliding to the left side of the A.T.R.S and striking employee on the top of the left forearm causing a laceration that required sutures.

Employee was bolting roof in the # 5 entry when the sleeves on the canopy slid down the post and striking the employee on the thumb of his left hand cutting the end of the thumb off.

Employee stated that as he got on the A.L. Lee mini trac he felt a twinge in his back but proceeded to load water line onto a flat car with the mini trac and as he was exiting the operators compartment he felt more pain in his lower back that was really bad. He received a CT scan at ST. Joseph Martin Hospital and was diagnosed with bulging disc's in his lower back.

Employee stated that he was backing the continuous miner out of the # 9 heading with his back to the miner when the cats either slipped or he split the cats, it was at this time the tail boom of the miner struck him in the middle of the back knocking the wind out of him. He was complaining of soreness in his back and a tingling sensation in his right leg and foot.

Employee was riding # 1 section mantrip when the man-trip passed over an uneven section of track which caused the man-trip to shift suddenly from side to side causing the employee to be shifted suddenly as well and he felt sharp pain in his lower back and numbness in his right leg, employee finished the shift.

Employee was cutting cable with a utility knife when the knife slipped out of the outer jacket of cable insulation cutting the index finger of his left hand just above the first knuckle. He was treated at Hospital at Martin, Ky. he received four sutures and was released and returned to work.

Employee was helping set the continuous miner up for the next cut, he bent down to pull the miner cable out of the roadway he then felt a sharp pain in his genital area. He finished his shift and sought medical treatment the next day and was treated for a strain.

Employee stated that he turned the #4 belt conveyor off by the remote switch and climbed onto the #4 belt to remove a rock from the #5 head drive and the # 4 belt started up and he was propelled into the belt crossover which is located outby the rock box of #5 head drive, which resulted in bruising to the left shoulder and abrasions to the back.

Employee claims to have hurt his back moving a miner cable from one rib to another. He finished that shift without reporting (the 8th). He worked his next shift without incident (9th). He missed work on the 10th. He returned on the 11th to report his injury to the Safety Department. He has not returned to work since.

Employee was trouble shooting the tram switch on a rubber-tired mantrip when it moved unexpectingly causing injury to his right arm.

Employee lost his footing and fell onto a metal rib strap resulting in a cut that required 5 stitches to repair.

Victim felt a pain in his left shoulder while dropping miner cable. He was taken to a local ER where he received an X-ray and was released. He hasn't returned to work as of 5-3-13.

An unplanned roof fall occurred 40ft. inby spad 1393 in the #3 entry, along the 001 Section North East Mains. Measured approximately 20ft. wide by 16ft. long by 6ft. high in an area that had already been "dangered off", pulling 2 rows of fully grouted roof support. Found by examiner around 10:10pm and reported to MSHA's hotline immediately thereafter.

Employee was reaching for a roof bolt plate when a piece of draw rock fell, striking his left fore finger. He was taken to ST. Joseph Martin Hospital and received X-rays. It was determined that a small piece of metal from the plate had penetrated his finger and that he would receive surgery later to have it removed.

Employee attempted to lift a piece of metal that was obstructing his path to his tool box. In doing so, he strained his back and finished his shift. He continued to work for the next couple of days before deciding to seek medical treatment and is under care of his family physician and anticipates returning to work within the next few days.

Employee cut his left, middle finger while making a belt splice. Received stitches and released to return to work his next shift.

Employee was struck by rib material on the #2 section. He was seen by a local hospital and treated for minor bruising. He was released to return to work on 11/08/2012 at full capacity.

A piece of metal slid against victims right ankle, fracturing his lower leg, just above his ankle. He was treated and released at a local hospital and is scheduled for surgery in the near future and is expected to make a full recovery.

While continuous miner operator was loading S/C, a loose portion of mine rib fell, striking him in the back, knocking him into the S/C. He was transported to a local helipad via ambulance and flown to St. Marys hospital in Huntington WV where he was treated for various injuries. Victim is in stable condition and doctors expect him to make a full recovery.

Victim attempted to remove a nylon string from a turning stuck roller while belt was in operation. Roller began to turn, string caught victims right index finger resulting in a cut which required stitches. Victim finished shift and missed no work.

Employee lost footing in a mud puddle and fell backwards onto right hand. Finished shift, missed next day to have hand examined. Bruise was determined to be only injury.

Victim attempted to hold on to bolter wrench while reversing rotation in order to remove a bolt he bent during its installation. The wrench/bolt snagged his glove and pulled him into the steel fracturing his left arm. He was released from hospital same day and is scheduled for surgery later.

Victim (Examiner) claims to have been struck between the shoulders with a piece of rock. He was taken to a local hospital where he received x-rays and was released to return to work the next d ay. He hasn't returned to work as of 6-29-12 and cannot be reached for an explanation.

Employee received a cut to his right hand from a nylon rope hanger while dropping the miner cable. Stitching was required.

Miner was struck in the forehead by a slate bar while attempting to move a rockdusting motor

Bolter operator working underneath his canopy had a 1'long x 1.5'wide x 1" thick piece of rock deflect off of the canopy and strike him on his back

A Piece of draw rock fell from in between the first row of bolts and the rib in # 7 right section and struck victim in the leg.

Miner pulling cable across beltline when end piece cathead caught on belt. When end piece release it struck him in the face causing bruising, fracture on nose and laceration requiring two staples.

Foreman was attempting to move track beam (uninstalled) from roadway. Track slid into another track beam and caught his index finger between the two beams. Received four stitches from pinch.

Outside man attempting to hook tow straps on the fork of a fork lift when his right thumb became pinched between the strap and fork causing a pressure tear.

Shuttle Car operator assisting in maintenance on CMMU 001 pinched little finger on right hand while lowering the CMMU 001 cover. Received two stitches and returned to work the following day.
