Operator trammed over a rock, causing dozer to shift. Causing pain in lower back.
Employee was investigating a truck fuel leak when his left knee contacted the turbo causing a burn. Employee did not report incident until 08/25/2014 and refused medical attention. Because of no medical attention or restricted duty, it was considered a non-reportable. Employee requested medical attention on 09/04/2014 and was taken to the doctor on 09/05/2014.
Mechanic and Foreman were attempting to change out a 992G tooth. While striking the tooth to free it, a shard of metal broke free from the hammer and struck the fueler in the neck.
Operator stated he had neck pain and numbness in his left arm.
Welder was working on tracks for a D10 dozer when a seal blew out of track pin and oil struck welder in the face
Caterpillar D11 Dozer was engulfed in flames. No injuries to Operator
Parts runner was attempting to remove parts from bed of pickup and strained his shoulder.

Foreman started a D11 dozer prior to the start of the shift to warm up the equipment for the operator. When he returned with the fueler, the equipment was in flames.

Mechanic was climbing onto a Cat D11 Dozer when his foot slipped off of the foot step and he fell landing on his right foot slanted inward.
While opening the magazine door the top lock stuck EE pushed on the top of the door and bumped the bottom of the door with his left foot at the same time He then felt a pain(burning) in his lower abdomen/groin area and it began to swell The hospital released him back to work and had a checkup appt. on 9/1/11 at which it was determined to be a Hernia requiring surgery to repair

Operator was pushing with a Cat D9 dozer when smoke started from the engine compartment. Operator turned off machine, called for assistance from foreman. Both tried to extinguish the fire but could not. Local fire department called to extinguish fire.
Foreman was struck by a metal shard in the cheek and eye when aligning and hammering a hardbar pin on a dozer.
Operator/Fueler was attempting to put antifreeze into equipment, when he missed the opening for the radiator and the antifreeze splashed on him.
Operator was operating a loader on the upper stockton coal pit, when he became light headed and hot. The operator stopped the loader and stepped out onto the cat walk to get some fresh air. At this time he briefly lost consciousness and fell, striking his lip and cheek on the railing. Operator was taken to Logan Hospital for Dr. evaluation.
Operator was climbing down a ladder on the side of his loader when he felt a strong pull in his leg. Pain continued and operator was transported to the hospital for an examination and Dr. review.
Operator was pulling coal from pit floor when the loader bucket came into contact with solid ground causing the operator to lunge forward. Operator complained of neck and back pain and was transported to the hospital for doctor evaluation.
