Employee was changing hyd. hose on continuous miner when panel he had opened fell catching his fingers.

The employee stated that a roof bolt bundle steel band flipped back and struck his right hand.

Employee bent over pulling vent tube onto respirable dust sampler and felt pain in rib.

The employee attempted to move a tool box. He reportedly suffered a double hernia. Surgery to be scheduled.

On 04/21/08 the employee reported that he had suffered a hernia on 04/02/08. He believes the hernia resulted from lifting himself out of a scoop operator cab. Surgery is scheduled for 04/24/08.

The employee attempted to lift a barrel set in the mine floor. A sharp edge cut through his glove and finger.

While moving equipment parts through a mandoor, the shaft slipped and pinched the employee's right index finger.

Pouring concrete roadway, concrete seeped into boots, and pants, causing irritation.

Employee was draging a section of dust line. As he pulled on dust line his shoulder poped out of place.

The employee was removing a shuttle car tram motor, when it slipped and pinched his left middle finger.

Lifting lid on battery pack felt a pop in chest

Was installing 90 deg. steel fitting. When trying to couple flange, the fitting slipped pinching his finger between fitting & pipe.

The employee lifted a kennedy panel to put it in place. The inside portion slid out, striking the right foot.

Strained his back while helping two other employees change a tire on the joy buggy.

On 5/15/06 a medical exam of the employee revealed a right side hernia. Employees stated that on 5/13/06 he had been lifting belt structure during a belt move. Surgery to be scheduled

The employee rolled a piece of rock and pinched his left middle finger between a water line and the rock

On 3/27/06 the employee notified the mine operator of lost time due to an injury sustained on 12/27/05. The employee attempted to lift a crusher inspection door and strained his right arm, torn tendon.

Employee was lifting a guard on a conveyor belt take-up, and felt a pop in his lower back (lower back & leg strain). Employee did not seek medical treatment at the time. His injury was later diagnosed as a disc herniation, which eventually required surgery.

Employee was stepping down off of a prell truck when his right hand caught the corner of a sign and cut it receiving stitches.

Employee was checking for a chipped tooth on the bucket on the shovel. He pushed a rock out of the way which in turn fell on his left hand & fingers.

Employee was servicing truck and the truck had broken down, employee came around the truck and caught right ear on lunch bucket rope hook, hook caught on earing and lacerated right ear lobe, requiring a few stiches

Mechanic was moving the jack pad on the drill when he started feeling pain in his lower back. Pulled muscle in lower back. Missed work from 9/6 thru 9/10/2005.

Employee states that he thinks original injury occurred on 6-1-05. He didn't report it until 10-14-05 when he felt a sharp in lower left groin area. He went to doctor and was diagnosed with a hernia. Surgery to repair is scheduled for 11-4-05.

Victim picked up a drill bit for highwall drill and caused an existing hernia to be reinjured. Missed one days work.

EE was tugging on the tire on a mechanics truck trying to remove it from the truck. He felt a pain in his lower abdomen/groin area.He is still off work due to hernia.

Employee was pulling rock off track on trackhoe. Rock hooked on cuff of glove. Pulled Arm causing injury to Left shoulder.

EE was installing a blade on a Caterpillar 16-G Grader. The chain holding the blade upright slipped off and the blade fell forward. The top edge of the blade struck his left foot just behind the steel toe. He incurred a deep laceration to the top of his foot and fractured three metatarsus bones (big toe and the next two). He is still off from work.

EE says he rolled a small rock off a push arm on a dozer. He then proceeded to another piece of equipment and his back started hurting.

EE was lying on the ground on his back installing cutting edges on the blade of a 16-G grader when he says he had a sharp pain in his lower back going down his right leg.

Victim states that he initially hurt his back on 9/16/04 but never reported it to anyone. On 9/21/04 he aggravated the injury while performing repairs on a truck. Reported to supervisors and went to doctor this date. Strained muscle in lower back.

Victim was hooking up a power connection (cathead) on a highwall miner and lacerated the middle finger on his right hand. Six stitches to close the wound.

Victim struck his head on a cross member of the Anfo Bin while preparing to load a truck with Anfo. Six staples to close the wound. Employee was a summer employee and scheduled for 08/20/2004 to be his last day of work before returning to school. He just asked to be terminated one day prior to schedule. No lost time involved and voluntary termination.

Victim removed the radiator cap from a D-9 dozer while engine was still hot. The hot anti-freeze blew out and hit him on the forearms, chest and stomach resulting in 1st and 2nd degree burns.

EE WAS HANGING VENT TUBE AND STRAINED HIS BACK.

Victim was working on a dozer when he slipped & hurt his back. He is being treated for a herniated disk in his back. He continued to work through June 21, 2004 and began missing work on June 22, 2004 and is still off work.

Operator got something in his eye while in the cab of the loader. Stayed home one day from work on doctors recommendation.

EE stated that he went to a tool box to get a can of WD-40. He lifted the tool box lid and reached into the tool box with his right hand and unkowingly put his left hand on the edge of the tool box and when the lid fell, it struck his hand between the lid of the tool box and the edge of the tool and caused a cut to the left little finger.

Employee was installing new corner bits bolts in dozer blade. He had all old bolts removed and put a bar in bolt hole to hold bit in place. The bar slipped and the corner bit fell striking his left foot at the base of his toes. Three toes were surgically removed, the big toe and the next two.

SETTING VALVE DOWN ON WATER TANK, WAS GOING INTO PLACE & CAUGHT FINGER BETWEEN VALVE & FLANGE, CATCHING FINGER.

Employee is a mechanic's helper - was helping the mechanic drop a belly pan on a D9N dozer for repairs - the chain slipped - the belly pan came to the ground & caught both hands

PULLED A CONTROL VALVE FROM A CRANE THROUGH A SMALL HOLE. STRAINED LOWER BACK.

While dozer was being serviced by greasers, the mechanic began doing maintenance on the track rollers. The fuel cap rolled off the machine and hit him in the head. Required fifteen (15) clips/staples to close.

HANGING A PIECE OF FLAT BAR, MATERIAL SLIPPED AND FELL STRIKING FINGER AND TWISTING FINGER ON LADDER.

LIFTING 5 GALLON BUCKETS OUT OF A VAULT.

EMPLOYEE WAS PULLING ELECTRICAL WIRE WHEN HIS HAND SLIPPED AND LACERATED INDEX FINGER ON RIGHT HAND. FIVE STITCHES TO CLOSE.

VICTIM WAS REMOVING ROLLERS FROM FRAME. ONE FELL ON HIS RIGHT ARM. POSSIBLE NERVE DAMAGE. PHYSCIAL THERAPY ONGOING.

EE INJURIED HIS BACK WHILE LIFTING A PART TO CAT 5230 BUCKET.

EMPLOYEE STATES THAT HE BUMPED HIS RIGHT ELBOW WHILE DISMOUNTING DRILL. DOCTOR DIAGNOSED AS POSSIBLE BURST BURSEL SAC WITH NERVE DAMAGE.

EE WAS DRAGGING A PULL CABLE AND PULLED A MUSCLE IN HIS BACK.
