In the B-8 Headgate section at cc 26 a roof fall occurred in supported roof. The fall was a 20 ft. wide, 20 ft long, and 7 ft. high. A thin coal ryder seam was located at the anchorage zone.

Employee was changing hyd. hose on continuous miner when panel he had opened fell catching his fingers.

The employee stated that a roof bolt bundle steel band flipped back and struck his right hand.

Employee bent over pulling vent tube onto respirable dust sampler and felt pain in rib.

The employee attempted to move a tool box. He reportedly suffered a double hernia. Surgery to be scheduled.

The section foreman on 2nd East Mains discovered a fall in the #5 entry between crosscut #4 and #5. The fall was approximately 10 feet wide, 25 feet long and 10 feet high.

Employee stated that he slipped stepping into ramcar and twisted his left ankle.

Was shoveling spillage around belt line stepping wrong and twisting his knee.

On 04/21/08 the employee reported that he had suffered a hernia on 04/02/08. He believes the hernia resulted from lifting himself out of a scoop operator cab. Surgery is scheduled for 04/24/08.

Was operating a pickup truck on slick roads and slid thru intersection and into irrigation canal, wrecking truck and fracturing collar bone.

The employee attempted to lift a barrel set in the mine floor. A sharp edge cut through his glove and finger.

On 2/1/08 the operator was informed by MSHA of the employee's black lung condition.

Transporting materials on forks of bobcat skidster, losing a crib block, stopping to pick it up he stopped behind another material hauler. That operator did not see the skid steer behind him and backed up pinning employee between the two pieces of equipment.

A bounce occurred on the First East Mains section with floor heave and rib sloughage in several areas across the section.

Employee was miner helper; while cutting coal a rib sloughed off landing on his right foot.

On 10/31/07, MSHA determined that a floor heave incident on 10/20/07 was a reportable "bounce". Floor heave occurred in the First East Mains #4 Entry Crosscut 14, #3 Entry Crosscut 14 to 15.

While installing a roof bolt, the bolt wrench broke and struck the employee in the face.

An event occurred which created floor heave and rib spalling in the First Mains section in the No. 2 entry between crosscuts 15 and 16.

The employee attempted to move a loop of miner cable when the rib fell, knocking him down.

Cutting a tree limb to clear pad site when chainsaw kicked back and cut knee.

While moving equipment parts through a mandoor, the shaft slipped and pinched the employee's right index finger.

Pouring concrete roadway, concrete seeped into boots, and pants, causing irritation.

EE was unloading trash from back of p/u truck into dumpster. Lost balance falling from back of truck onto ground.

The employee raised the roof bolter drill box and bent the drill steel which pinched the right middle finger.

On 4/22/07 at approximately 10:30 pm the preshift examiner discovered a roof fall in North Mains #6 entry cc 33 to 34. The fall was 20'x100'7'. Several small roof coal falls were discovered in adjacent entries.

On 4/10/07 the employee reported that on 4/8/07 while operating a skid steer loader he ran over a windrow of coal. The cab hooked a power cable hung from the roof. The cab raised up and then came down on his foot.

Employee was draging a section of dust line. As he pulled on dust line his shoulder poped out of place.

Miner Operator was struck by rib roll while cutting crosscut with continuous mining machine.

The employee was removing a shuttle car tram motor, when it slipped and pinched his left middle finger.

On 12/12/06 Bowie was notified of the diagnosis of carpal tunnel syndrome of the employee.

The employee traveled over construction material. When he slipped his arm was cut on the edge of a steel plate.

Lifting lid on battery pack felt a pop in chest

While operating a wagner scoop the employee hit a pot hole in the roadway. He struck his head on the cab. His safety glasses cut his face.

The employee attempted to travel through a mandoor at Crosscut 14 West Mains 4-5 entry B-Seam when he stepped through the door he fractured his right ankle. The mandoor closed on his left left leg. He was found at 6:00 a.m. on 8/17/06.

Was installing 90 deg. steel fitting. When trying to couple flange, the fitting slipped pinching his finger between fitting & pipe.

BUCKET OF SCOOP WAS DROPPED ON FOOT.

The employee lifted a kennedy panel to put it in place. The inside portion slid out, striking the right foot.

Slipped off miner and injured hip.

Strained his back while helping two other employees change a tire on the joy buggy.

WAS IN FRONT OF BOLT MACHINE BEATING DRILL STEEL WITH WRENCH. AS HE STEPPED OVER TO HIT STEEL HIS FOOT SLIPPED WHILE SWINGING STRIKING FINGER BETWEEN STEEL & WRENCH.

The employee turned a corner with a shuttle car. Bolting wrenches stored in the rib mesh were flipped into the operator's cab. The wrenches struck the employee in the right ear.

On 5/15/06 a medical exam of the employee revealed a right side hernia. Employees stated that on 5/13/06 he had been lifting belt structure during a belt move. Surgery to be scheduled

On 4/5/06 Bowie was informed of a fracture to the employee's finger. The employee was attempting to remove a bit from a miner head when she struck her finger with a hammer.

The employee was helping to load explosives in a rock in the face conveyor when a piece of rock 2'thick 3' high and 10' long flushed off of the face and struck the employee.

On 3/6/06 the mine operator was informed that the employee was diagnosed with occupational-caused carpal tunnel syndrome.

The employee rolled a piece of rock and pinched his left middle finger between a water line and the rock

On 2/1/06 Bowie Resources was informed that the employee was diagnosed with carpal tunnel syndrome of both wrists.

Employee was lifting on a slate bar while trying to hook up a chain link on a road grader and strained his lower back. Employee did not report accident until 1-10-2006. Employee missed one day of work
