The miner was performing maintenance on a crusher removing liners. While lifting one of the liners employee felt right elbow pop.

Miner was helping clean out a scale pit by removing buckets of mud and dirt material. The lifting caused a back strain they realized later that night.

Clearing a rock chute when a rock fell smashing their finger causing a laceration.

Repairing a water truck when a tire fell on left foot fracturing it.

In the process of final fabrication of heavy metal object, object tipped, striking employee's foot causing a compression injury to the right foot.

During installation of new screens in the screening plant, one screen did not set in properly. The employee was adjusting the screen and cut EE's finger on the new screen edge causing a laceration. The laceration required 3 stitches.

Employee was securing a chain to a piece of sheet metal. As EE was pulling on the chain, the piece of sheet metal tipped over striking the employee on the left ear causing a laceration to EE's left ear. The injury required stitches.

While assisting in the changing of the shocks and sway bar, employee was under the blocked equipment and hit head on the rear differential of the articulated truck. EE received a filet laceration on the top of the head requiring 8 stitches.

While connecting discharge water pipeline together employee pulled the collar from the bottom of the pipe around to secure the lock straining shoulder.

Employee and witness were completing repairs using a hand-held hydraulic jack. Prior to its use, the jack was placed on a ladder. The employee bumped into the ladder, and the jack fell off the ladder striking the employee's foot. Employee suffered fracture of great toe. Employee was wearing protective shoes.

Employee was performing maintenance on rotor and wedges of crusher when pulling on a multiplier he felt two snaps in his upper right arm

With supt removing flywheel off crusher, spacer pins buckled pinching employee's hand. Employee refused treatment; had full mobility of finger & hand. 6-2-15 Employee reported everything was fine just sore & bruising. 6-9-15 employee complained of lack of full extension of pinky finger went to dr for eval & sent to specialist 6-10-15 specialist determined fracture & put in cast

Part of a crew replacing the belt on a feed conveyor (C-397). He was working in a manlift. Attempted to push the conveyor belt into the center of the troughing roller. He felt a twinge/pull in his left shoulder. Pushed a second time on the belt, the pain was more severe and he had a hard time moving his left shoulder, i.e. the pain worsens with movement.

Operating a pit truck going to dump in hopper from muck pile, a 150-200# boulder fell off load when making turn to setup backing to hopper. Boulder was in travel portion of haul road. After dumping went to move boulder out of way manually, turned over twice and 3rd time his arm went pop, pop. Thought pulled muscle, MRI reveals tear in right bicep. Didn't report til shift end (6PM).

Employee was assisting with the replacement setting of the catwalk on the primary crusher. He stood from a crouch position to a standing position striking his back against the catwalk framework.

While doing maintenance work on the Secondary Impact Crusher Equip. ID#P-035 employee was attempting to manually roll hammers from a Svedala S250DC Crusher into the bucket of a skid steer to take to the scrap dumpster when he caused injury to his right shoulder.

While attempting to dislodge a rock from the tailgate of a Cline Haul Truck; company number H-43, the tailgate came down striking/smashing the employee's right index finger.

While walking from the Corporate Office to the dumpster a shard of glass fell out of a garbage bag striking the employee on his left lower leg (calf) causing a laceration requiring 25 stiches to close

At the surface shop our mechanic EE was removing an hydraulic pump from a haul truck (belonging to a subsidiary of River Product), when the pump slipped and struck the 5th finger (pinky finger) of his right hand, requiring three (3) sutures.

While lifting the tongue on a dolly so it could be chained up, the tongue was let down and the employees 3rd and 4th fingers on his left hand were smashed. The 4th finger required stitches.

While removing a hydrolic cylinder rod on a Tamrock DHA 107B Drill with air, the rod dislodged and exited the cylinder lacerating the thumb on the right hand of the employee; requiring six (6) stitches.

While repairing the guard on a conveyor tail pulley a C-Clamp being used to secure the guard came loose. The tension in the guard threw the clamp striking the employee just above the right eye. This resulted in a laceration about 2.8 cm in length that required stitches to close.

While disassembling portable sand plant for relocation. Employee's hand was pinched/crushed between pipe and angle iron on plant structure. Fracture of 5th metacarpal of right hand.

WHILE CLEANING CONVEYOR, REPLACING CLEANOUT DOOR HIS 5TH FINGER ON LEFT HAND WAS PINCHED BY THE CLEANOUT DOOR DURING REPLACEMENT. THE FINGER WAS CRUSHED (TUFT FRACTURE REQUIRING SPLINT).

EE WAS DOING LOADER REPAIR, WHILE PUSHING PINS OUT OF BUCKET HYDRAULIC RAM LET LOOSE ALLOWING METAL SPACER TO FALL STRIKING HIS HEAD. THIS RESULTED IN A LACERATION THAT REQUIRED EIGHT STITCHES TO CLOSE. EE RETURNED TO WORK SAME DAY.

THE EMPLOYEES WERE WORKING IN THE SURFACE SHOP (MAINTENANCE ONLY) WITH A PIECE OF 1/2" THICK PLATING WEIGHING APPROXIMATELY 120 LBS. WHILE PICKING UP THE PLATE, THE EMPLOYEE'S FINGER WAS PINC

WHILE CHANGING DOWN A LOAD ON FLATBED TRAILER WITH A BREAKOVER BINDER, WAS RE-CHECKING ALL THE DOWNS. WHEN BREAKOVER BINDER CAME LOOSE (POOSIBLE MECHANIC'S FAILURE) STRIKING EMPLOYEE ON THE PA LM OF HIS RIGHT HAND.

WHILE WORKING ON DRILL IN QUARRY SHOP, EE WAS CHANGING STEELS IN THE CAROUSEL. HE WAS HOLDING A STEEL WHEN THE JAWS WERE OPENED. THE LOCK RELEASED PINCHING HIS FINGER, RESULTING IN A CUT TO TH E MIDDLE FINGER OF RIGHT HAND, REQUIRING SIX STITCHES TO CLOSE.

WHILE MOVING A FORK ON THE FORKLIFT EE STRAINED HIS LOWER BACK.

THE EMPLOYEE WAS INSTALLING A GUARD DISTATED BY AN MSHA INSPECTION ON 3-16-99. WHILE INSTALLING THE GUARD THE EMPLOYEE MOVED HIS HAND DOWN THE GUARD EDGE AND CAUGHT HIS FINGE WHERE THE EDGE W AS ROUGH.

WHILE MANUALLY LIFTING THE HOOD OFF LOADER, THE HOOD SLIPPED AND PINCHED THE PINKY FINGER.

A 2" DIAMETER STEEL POST WAS INSTALLED WHO THE EARTH TO MAKE A BURIED AS LINE, THE STEEL POST (TRAINING A ROOF THE SURFACE WAS APPROX 2' A PLAN AND PERSON WALKING IN THE AREA CONTACTED TO TOP PORTION OF THE POST WITH THE UPPER THIGH CAUSING A LACEERATION REQUIRING SUTURES.

EE WAS DRIVING THE STOCKPILE TRUCK FROM THE PLANT TO THE STOCKPILE ARE WHEN HE NOTICED THAT THE TEMPERATURE GAUGE SHOWED A HIGHER READING THEN NORMAL. HE DROVE TO THE SHOP AREA & RAISED THE CO VERING TO RELEASE THE RADIATOR CAP & RELIEVE BUILT UP PRESSURE. WHILE RELEASING THE CAP THAT WASUNDER PRESSURE, WHEN THE SEAL RELEASED, THE CAP BLEW OFF IN HIS HAND & STEAM & RADIATOR FLUID S

INJURED WAS CLEANING OFF BUILD UP MATERIAL ON WALKWAY WHEN HE HEARD SOEMTHING BEHIND HIM. HE TURNED TWISTED SUDDENL TO LOOK BEHIND HIM AND HURT HIS BACK.

INJURED WAS CHANGING LOCATIONS IN ORDERTO FLIP MATERIAL FLOW GATES WHEN HE HIT HIS HEAD ON A HANDRAIL RESUTLING INA STRAIN TO HIS NECK REGION.

HE WENT UNDER R-50 FOR AN AIRHOSE, STOOD UP UNDER TRUCK WITHOUT ENOUGH HEAD CLEARANCE, STRIKING HIS HEAD ON THE UNDERCARRIAGE OF TRUCK CAUSING A LACERATION REQUIRING 4 STITCHES.

WHILE PAINTING ON A CONVEYOR FROM THE UNDERNEATH SIDE HE LOOKED UP AND A DROPLET OF PAINT FELL INTO HIS EYE.

WHILE CHANGING HAMMERS ON P-22 (IMPACTII) PIECES OF METAL CHIPPED OFF A SHOE THAT HOLDS HAMMER IN PLACE ONE PIECE BECAME IMBEDDED IN EE ABODEMEN EE REMOVED THE OBJECT WENT TO DOC REQUIRED 4 ST ITCHES & A TETNUS SHOT

WHILE CHANGING HAMMERS ON P-22 (IMPACT11) PIECES OF METAL CHIPPED OFF A SHOE THAT HOLDS HAMMER IN PLACE ONE PIECE OF METAL HIT EE LEFT KNEE CAUSING A LACERATION RT ABOVE THE KNEE IT REQUIRED 2 STITCHES & A TETNUS SHOT

WHILE CHANGING GRATES IN HAMMERMILL, A GRATE SLIPPED AND FELL ON INJURED'S HAND. HE PULLED TENDONS AND BRUISED HIS HAND.

I NAME WAS REPAIRING A SCREEN SUB FRAME A PIECE OF STEEL 80# ANGLE IRON SLIPPED HITTING I NAME ON HIS LEFT FOOT THIS BLOW CAUSED IMMEDIATE SWELLING OF THE INSTEP I NAME WAS TAKEN TO MERCY HOSP ITAL FOR EXAMINATION SEVERE BRUSIING WAS THE INJURY

INJURED WAS USING THE SHOVEL AT THE PLANT LOCATION REMOVING ROCK FINES FROM UNDER THE BELT.WHILE DOING THIS HE PULLED THE MUSCLES IN HIS LOWER BACK.

EMPLOYEE WAS GETTING INTO A FELLOW EMPLOYEE'S VEHICLE TO GO TO THE SCALEHOUSE AT THE END OF THE DAY'S SHIFT TO CHECK OUT ON THE TIME CLOCK. HE RAISED HIS LEFT ARM UP TO PUT IT ON THE BACK OF THE SEAT WHEN HE BUMPED THE CO-WORKER'S ELBOW. HE SUSTAINED A RECURRENT DISLOCATION OF HIS LEFTSHOULDER (SEE PHYSCIAN'S REPORT ATTACHED).

WHILE WASHING DOWN THE SCALE DECK,IT REQUIRED THAT YOU MOVE THE FIRE HOSE TO VARIOUS LOCATIONS;DURING ONE MOVE THE HOSE BECAME CAUGHT AND WHEN TRYING TO MOVE THE HOSE THE INJURED PULLED A LOWE R BACK MUSCLE.

RAISING THE LAN8ING GEAR ONT HE SCREENINGPLANT TO I NSTALL CRIBBING UNDER THE PLANT. WHILE TURNING THE HANDLE ON THE LANDING GEAR, I NAME SPRAINED HIS BACK

EMPLOYEE WAS CH5NGING OIL ON MOTORGRADER HE CAME OUT FROM UNDER MACHINE & RAISED UP HITTING HIS HEAD ON RIPPER BAR HE HAD H IS HARD H AT ON HE COMPLAINED OF A STIFF NECK LATER IN THE DAY BUT R EFUSED MEDICAL 5TTENTION AT THAT TIME LATER THAT EVENING HE WENT TO MERCY H OSPITAL WHERE INJURY WAS DIAGNOSED AS A JAMMED NECKSIMULAR TO A WHIP LASH

EMPLOYEE WAS ST5NDING ON A FLAT SURFACE. HE WAS ATTEMPTING TO STAIGHTEN OUT SOME CHAINS LAYING ON A 3-DECK SCREEN USING A PIECE OF WIRE WITH A HOOK IN THE END OF IT. THE CHAIN WAS SNAGGED AND THE EMPLOYEE PU5LED, THE CHAIN CAME LOOSE. THE EMPLOYEE OVERBALANCED AND FELL 7' TO 9'. HE LANDED ON HIS FOREHEAD, STRIKING THE GROUND.
