Employee was acting as a spotter to position a pipe for another employee operating a forklift to lift a section of tubing through the doorway on the 2nd fl, of mill. During the process the pipe contacted the metal stair handrail causing the pipe to swing around and contact the employee who received a small cut from the shield edge of his safety glasses.

Two millwrights were taking a hose sweep from the warehouse wall rack to the floor, so they could move it with a forklift. As they pulled it off the rack, one end at a time, one employee felt a pain in his back. Appeared to be a back sprain.

EMPLOYEE WAS INSTALLING WATER PUMP ON PL-E TRYING TO LINE UP BOLT HOLES, FELT SOMETHING PULL IN BACK OF NECK. EMPLOYEE TRATED HIMSELF UNTIL 8/27 THEN WE TOOK HIM TO THE DOCTOR.
EE WAS DOING NORMAL JOB OF BAGGING & LOADING OF 50# BAGS OF VERMICULITE CONCENTRATE FOR A SHIPMENT ORDER. EARLY IN HIS SHIFT HE COMPLAINEDOF A SORE BACK. HE TOLD HIS SUPERVISOR, ASKING FOR HEL P. LATER IN THE SHIFT HIS BACK WAS VERY SORE. DR'S ANALYSIS STATED A SLIGHT STRAIN/SPRAIN OF THE LOWER BACK.
EE WAS HELPING MOUNT AN SAFETY LADDER CAGE ON A STRAIGHT LADDER AT THE EXPANDER TO BE STABILIZED, ASSISTED BY 2 EMPLOYEES STANDING AT GROUND LEVEL. HE FELT A SHARP TWINGE BETWEEN HIS SHOULDER BLADES. HE WAS HOLDING THE UNIT HALFWAY FROM HISBODY. AFTER HE FELT A TWING, HE HELD THE POSITION TILL UNIT WAS SECURED.
