Employee was cleaning out a toolbox in the shop. A utility knife with the blade open was in the bottom which EE cut EE's finger on. EE was taken to Concentra walk-in clinic where they medically glued the cut closed. No impairment and no lost time.

Laying out future blast holes when rock and debris began falling from the above bench area.

Employee states while shoveling rock they hurt their shoulder. Employee notified the supervisor on Friday 5/5/2017 that the injury occurred on 4/27/2017. Employee currently off work due to restricted activity.

Working at bottom of feeder pan, chip down to bare steel to find plug hole; burn out the weld to move the plate-rust & fine dust on plate-immediately tried to wipe from eye and flushed eye-trouble with eye on way home; awoke to bad pain at 1:30 am; 5:00 am eye swollen shut. Medical attention sought 1/26/2017.

Injured employee was helping another employee get a screen loose on the pep screen. They pulled on the screen and when it came loose, injured employee's right hand was mashed between the lever and the pep screen. Injured employee remains off work pending further doctor evaluation.

While employee was doing clean-up work using a shovel, ee reported ee injured back. Medical attention at local hospital.

Loading holes, unrolling & moving the dewatering hose, hurt low back

While replacing a wall hammer in the crusher the hammer slipped out of position. Injured person & another employee lowered the hammer to re-position their grip so the hammer did not fall onto a 3rd employee. IP did not remove his hand causing the hammer to sit down on his hand pinching his hand/fingers. X-rays revealed no fractures & he was released to return to work.

Employee was washing off concrete pad. The employee went to pull the water hose with his left arm when the hose got caught on something and jerked his arm back. The employee heard and felt a "Pop" in his elbow and had immediate pain. The employee was treated at the Emergency Room for his pain and was instructed to follow up with an Orthopedic doctor.

While turning hammers over in the Stedman crusher and being assisted by another EE the other EE's end slipped from his hand crushing injured employee left ring finger. His finger was caught between hammer and the plated it attaches to causing laceration and contusion. First aid was performed at plant. ER treated and released day of injury. X-ray at ER showed not broken bones.

Employee was holding jaw bolt & pin while the pin was being hammered out; thought his hand was clear of pinch point but as the pin came free, his hand slipped behind pin & wall pinching his right ring finger. Medical attention on 9/3/14, Restricted work activity.

End of shift, washing out skid-steer bucket and pins; when employee pulled out pin the bucket fell on his left foot. Fracture left big toe bone. Employee remains off work

Employee airing up tire on wheel barrow, put too much air in the tire exploded. Tire parts hit employee on side of head. Treated and released from ER.

EE was on the drive side on the tunnel conveyor hooking up a comealong to pick up bottom of the belt off old wooden crosstie. New wooden c.t.'s being pushed in place across the metal frame to the drive side of conv., the EE reached down to grab c.t. and mashed finger tip between wood and conveyor metal. ER 2/22/12 Surgery 2/23/12, Rt. middle finger tip. Remain off work.

Loading a piece of steel in the loader bucket; the piece of steel slid and mashed finger between piece of steel and the cutting edge of the loader bucket. Medical treatment on 1/31/12: amputation Rt 4th finger tip: 2/1/12 return to modified work, unable to use rt hand.

EE was putting belts back on primary impact: mashed nail of left ring finger between V belt and thieve on impact motor. 2/13/11 sought medical attention at ER. 2/14/11 sought medical attention at ER. 2/14/11 return to modified work, Rt hand only. Note - 2/14/11 - 2/18/11 schedule vacation taken.

Employee was jacking trailer off the ball-hitch was stuck on ball-when hitch release it smashed right index finger between the hitch on the trailer and the military hitch on truck. He worked 5/2/08 and is currently off work, effective 5/5/08. Note: currently employee tn shop-delivery to huntsville al quarry.

TN welding shop crew working at Huntsville Quarry for modification to plant. Employee was working inside air classifier, got out and came down ladder, caught left elbow on edge of cage surrounding ladder resulting in miner cut that he cleaned & applied bandaid to. Wed. 8/15/07 elbow hurting & seeing swelling, sought medical attention 8/17/07. Follow-up 8/24/07.

EE parked truck, got out of the truck and the door came back on right hand mashing middle finger & ring finger. Currently working under restricted work activity.

Employee was shoveling under conveyor, rock fell and hit left hand. 1/17/07 restricted work. 1/22/07 released to regular work.

WORKING IN SECONDARY PLANT, WRENCHED BACK. DID NOT WANT TO GO TO THE DOCTOR AT THIS TIME. STATED HE WOULD SEEK CHIROPRACTIC CARE AT HIS EXPENSE. (HE WAS USING SHOVEL) FIRST REPORT OF INJURY WRITTEN OCT 1, 2004 FOR RECORD ONLY. MEDICAL ATTENTION SOUGHT ON OCT 1, 2004. ACCORDING TO WORK COMP INS CARRIER ON APRIL 8, 2005 - RELEASED NO RESTRICTIONS 2/16/05.

EMPLOYEE WAS TAKING OFF BOLT AND NUT ON SKIRT BOARD ON CONVEYER WHEN THE METAL BACKBOARD SLIPPED DOWN AND CAUGHT RIGHT FINGER BETWEEN METAL AND WRENCH.

CLEANING UP AROUND PLANT, ROCK STRUCK EMPL ABOVERIGHT EYE. EE OK AT TIME OF INJURY. EYE TURNED RED & SENT TO DR ON 5/14/03

USING SHOVEL TO CLEAN OUT FALLING DIRT UNDER CONVEYER, SECONDARY PLANT, TURNED WRONG WAY AND TWISTED BACK.

EE MOVING THE CAT WALK, DROPPED CATWALK AND THE END WENT THROUGH TOP OF SHOE. EE WAS WEARING STEEL TOED BOOTS. CHIPPED BONE AND SKINNED BIG TOE OF RIGHT FOOT. 3-20-02, SPOKE TO EE, HE REC'D AN TIBIOTICS, LOSS OF BIG TOENAIL.

EE WAS PICKING UP A 150 LB HAMMER FOR THE STEADMAN CRUSHER (522029) AND STRAINED HIMSELF. EE NOTIFIED EMPLOYER OF INJURY - JUNE 10, 2000.

EMPLOYEE WAS LIFTING A CUTTING EDGE WITH ANOTHEREMPLOYEE AND PUTTING IT IN A LOADER BUCKET AND DROPPED IT ON HIS MIDDLE FINGER. EMPLOYEE SMASHEDMIDDLE LEFT FINGER AT FIRST JOINT.

A ROCK FELL FROM THE JAW WHILE EMPLOYEE WAS WORKING UNDER CONVEYOR.

AUGER TUBE ON TRUCK SWUNG AROUND HITTING EMPLOYEE IN CHEST.

EMPLOYEE WAS PULLING ON A PUMP HOSE IN UPRIGHT POSITION

DISCHARGE TUBE SLIPPED OFF OVERHEAD AUGER TUBE STRINING EE IN THE MOUTH ONE TOOTH BROKEN AND TWO TEETH KNOCKED LOOSE.

PULLING A GEAR BOX ON PUG MILL AND AS GEAR BOX WAS BEING LIFTED OVER WALKWAY RAIL EMPLOYEE CAUGHT HIS THUMB BETWEEN RAILING AND BOX.

EMPLOYEE WAS SHOVELING MUD FROM UNDER A PUTTING AND FELT SOMETHING PULL IN LOWER BACK.

DRIVER STOPPED TO MOVE ROCK OUT OF ROAD

SHOVELING UNDER CONVEYOR IN TUNNEL & TWISTED TO MOVE SHOVEL CAUSING PULLED MUSCLES IN SHOULDER

EMPLOYEE WAS REPLACING A WATER LINE ON A TRUCK. WHILE TRYING TO LINE UP PIPE WITH LINE, THE WATER LINE CAME BACK STRIKING EMPLOYEE IN MOUTH.

EMPLOYEE WAS LOADING SCRAP IRON INTO A BUCKET, ONE PIECE HIT ANOTHER CAUSING THE SECOND PIECE TO STRIKE EMPLOYEE'S FOREARM.

CLEANING OUT CHUTE & ROCK FELL ON HAND CAUSING CUT THAT REQUIRES STITCHES.

EMPL WAS WORKING ON HAMMER MILL & STUCK PIECE OF STEEL IN 2ND FINGER ON LEFT HAND. CONSIDERED AS FIRST AID TYPE INJ BUT INFECTION CAUSED EMPL TO SEEK MEDICAL ATTENTION 12/22/89.

EMPLOYEE WAS SHOVELING ROCK FROM UNDER HAMMER MILLS. WHILE THROWING CONTENTS OF SHOVEL OVER RIGHT SHOULDER, HIS LEFT SHOULDER POPPED AND WENT NUMB.

PULLED ON HUNG-UP WELDING AND PULLED MUSCLE IN BACK.

THE INJURED IS EMPLOYED BY THOMPSON MACHINERY BUT WAS WORKING ON OUR EQUIPMENT AT OUR HUNTSVILLE QUARRY WHEN HE HE WAS INJURED.AIR HOSE BECAME OFF OF EQUIPMENT THAT THE INJURED WAS WORKING ON HUITTING HIM BELOW RIGHT EYE.

EMPL WAS CLIMBING ON THE DRILL CAUGHT THE GAS CAP FOR SUPPORT IT TWISTED AND CAUSED HIM TO FALL

EMPLOYEE WAS CLEANING OUTSIDE WINDSHIELD OF TRUCK AND TWISTED HIS BACK

EMPLOYEE WAS CUTTING METAL AND CUT ARM WHILE LOADING IT INTO TRUCK

EMPLOYEE WAS STANDING ON A SPOOL WHILE ROLLING SPOOL OVER TO CLEAN PULLEYS; HIS FOOT SLIPPED OFF AND WAS CAUGHT BETWEEN HAMMER AND BACKPLATE, CHIPPING BONE IN LEFT GREAT TOE.

EMPLOYEE WAS DUMPING ROCK FROM A HOT PAN TO A COOL PAN WHEN THE PAN SLIPPED AND FELL ON ARM. DUE TO CLERICAL ERROR, THIS REPORT WAS NOT FILED AT TIME OF INJURY; IT IS NOW SUBMITTED AS A RESULT OF CITATION #3248964. WE APOLOGIZE FOR THIS ERROR.
