While lowering ladder in the back of the truck to reach the manway. Driver released the safety pin of the ladder with right hand while holding the top section with their left. Somehow their right hand got caught like a vise between the ladder and its support crushing their right index finger.

Contractor driver from Midwest Industrial Supply pinched or crushed their right index finger in a ladder they were lowering on their truck. The ladder hinges down after a release of a pin. Right index finger was lacerated from the incident. I was informed by their company that part of their finger was amputated at the hospital.

Employee was reinstalling the bonnet on the 307-A motor. During this process EE felt a strain in the lower left side of EE's back. EE reported it to EE's supervisor and continued working until 10:30 pm. The next morning EE felt stiff and was experiencing some discomfort so EE went to the local clinic. EE was diagnosed with a muscle strain and prescribed a mild muscle relaxer.

A Production employee was carrying a ladder across level ground when they stumbled and fell. They dropped the ladder and put their hands out to try and catch themselves. The impact with the ground caused a radius fracture of the left wrist

Employee reached in to grab a porta power that they saw starting to slip. They put their hands in the line of fire and when the shoring spun it impacted their right thumb. Employee was taken to the local clinic where an X-ray showed an acute non displaced fracture. Employee was instructed to wear a finger splint and released to return to work.

Employee was working in the preheater tower; EE was wearing a face shield and safety glasses. When EE lifted up the face shield the dust that accumulated on the rim of the shield fell into EE's face getting dust in EE's left eye. The employees' eyes were flushed immediately, the employee followed up with a clinic and was prescribed prescription eye drops.

Employee was trying to reinstall a inspection port on a air slide, some cement flushed, and some sprayed up and impacted EE's face. EE rinsed EE's eyes out to flush any foreign material. After EE's shift, the employee was still feeling some irritation and went to the emergency room where EE was given prescription eye drops.

Employee looked up and dust from the rim of EE's face shield fell into EE's left eye. EE was wearing safety glasses under the face shield. flushed eye and finished shift. Went to doctor next morning and was prescribed eye drops, nothing found in EE's eye.

EE was having trouble with coal shaker hoist cables making limit switches. EE attempted to manipulate cables to contact switches. EE had hold of a cable to guide it and told EE on ground to jog hoist. In doing so, right index finger got pinched in drum causing crushing injury / open fracture to tip of finger.

Shipping employee placed hands in harm's way while closing silo elevator doors. Employee attempted to use nylon strap which was broken and too short to pull doors together. In the process, employee's right hand was caught between the upper and lower doors.

Employee working with another to remove large guard from head of elevator to replace chain. In process, employee's hand (finger) got caught between edge of guard and elevator housing causing middle finger to be crushed. EE wearing ANSI A4 impact resistant gloves as required.

Clinker dust fell from overhead pipe while employee was working to connect and run air hose along pipe.

Employee was removing hydraulic fitting under pressure. Hydraulic fluid under pressure struck employee in right side of head causing laceration approx. 4" long requiring emergency room treatment.

Employee part of maintenance crew removing analyzer probe housing from back of kiln. While working to guide probe and rigging around obstacles, rigging slipped causing weight to shift and end of probe housing to fall striking employee on instep of left foot behind safety toe of boot causing foot to be broken.

Pump running hot and cavitating. Employee and Supervisor working to correct operational condition.

The employee's right thumb was smashed between a pallet and the trailer deck that the employee was offloading.

On 5/11/21, the employee wasn't sure what EE had done. After a 12 hour shift, EE's right knee began to hurt. EE didn't think much about it until 5/18/21 when EE's knee was still hurting, and EE wanted to get checked out. I took EE to the doctor on 5/19/21, were restricted duty, and a couple of therapy sessions were ordered

Employee was moving filing cabinet and other miscellaneous items and felt a pull in right shoulder.

Repetitive motion while using a material handling vibrator, walking up and down stairs, to get to silo that employee was working on

While placing liners in the finish mill, mill liner fell from mill and struck employee

Hot material fell onto jacket, then material fell down on top of employees right hand. (Submitting now, due to information obtained on 3/12/2018)

Employee was rolling a steel plate on a steel rolling machine when EE's left hand was pinned in between the plate and top roll of the machine.

Employee #1 was inside the lab rest room, reaching to pull the door open to exit with left hand, with right hand ee tossed a paper towel in the trashcan. Employee #2 was on the outside and pushed the door open to enter the rest room. The Door struck Employee #1 in the face.

While removing calibration weight from a weigh feeder belt, the calibration weight fell, striking employee on the right hand, causing a small fracture to the tip of the right index finger.

While transferring cement sacks from one pallet to the next, employee felt a pain in left hip

Employees right hand was pinched in between crane headache ball and headache ball hook. This became a reportable accident on 10/28/2014.

Employee was securing a metal pipe when a hardened piece of material struck the opposite end of the pipe causing pipe to move upward hitting employee in the face.

Employee was remounting a vibrator, but the bolts he grabbed were too short. When he was removing the bolt, he lost hold of the vibrator and it fell striking his upper right forearm.

The employee was lowering a propane fuel tank from the back of a forklift when the tank bracket pinned his finger between the bracket and the forklift body, causing lacerations to his finger requiring stitches

Employee was using his foot to push a railcar unloading door closed when the door swung back and struck his foot, causing a "pop" in his knee. Initial Dr visit indicated bruising and employee returned to work w/no restrictions. This accident did not become a LTA until 08/14/2012 after a follow-up visit to the DR.

The employee was separating two pieces of metal plate that were tack welded together with one piece secured in a bench vice. When the employee pulled down on the other piece to break the weld, the sudden movement down caught his free hand and lacerated the back of the hand, requiring 6 stitches.

Employee was removing a metal inspection door on the Old Center feed chute when it dropped down, pinching his little finger between the door and a metal flop gate positioning lever. The employee was being trained during his first shift in this job position.

Employee sustained a right bicep tear while attempting to move an 8 ft x 4 ft x 1/4" plate. The employee continued to work, but following an MRI on 04/01/2011, it was determined that he would have to have surgery. The first lost work day was 04/04/2011.

Oiler was opening small door to the drip oiler when the movement of this action caused dust accumalation on the guard to fall down and become airborne and in left eye.

Welder/Repairman checking operation of Firebal grease pump after rebuild. Applied air to 50 psi pump would not actuate but air was escapiing around housing. Repairman was feeling area where air was escaping when pump engaged slicing off end of right thumb.

Sitting at crusher operator chair when he leaned over to adjust chair and his head came in contact with an on/off switch on the operator control panel. A portion of the on/off switch was broken off causing a pointed edge. The operator sustained a 1" laceration on top of his head.

Employee was assisting another employee installing a motor. The employee tried to lift the part and felt a twinge in the rib area.

The employee was trying to wash off the aqua ammonia chemical that streamed onto his back from a ruptured hose. However while washing off the employee jammed his finger on a concrete block and broke the tip of his thumb. The injury was the broken thumb.

Employee was moving a steel plate and the plate slipped and caught his finger.

While fitting a new diverter plate behind Raw Mill #2, roller #4, the plate slipped and fell against the roller pinching a finger tip causing a fracture of the 3rd digit, right hand, recieving two stitches. The ee was allowed to return to work the same day with minimal restriction. A followup evaluation was scheduled for 1/25/08.

The ee states he was helping install a new conveyor belt on top of the raw bins. He says as the belt was being pulled the roller bar for the new belt slipped out and struck him in the lip causing a laceration. He received sutures to repair the wound and was released to regular duty.

The Utility ee was loading tools into a utility mule when he felt a "pop" in his right wrist as he lifted a 30lb jackhammer. Medical attention was not sought initially. On 03/16/2006 the wrist was still swollen so medical treatment was sought and the employee was placed on restricted duty, after being diagnosed with a sprained right wrist.

While the employee (welder/repairman) was replacing a bolt on a cooler drive rail, his left middle finger was caught between the drive arm and bolt he was replacing. This resulted in a lacerated finger requiring two sutures. He returned to full duty the same day.

The ee bent down to lift an air vibrator out of a jorox storage cabinet on the 3rd floor of the tower. When he lifted the vibrator he felt a pain in his lower back. He was placed or restricted duty beginning 9/27/05 after a dr visist on 9/26/05. He is receiving pt for a lower back muscle strain which wasn't getting better

During a tower plug, EE was using Cardox tubes to dislodge a feed pipe. After the first 2 Cardox tubes misfired, the employee opened the Cardox pot to insert a 3rd Cardox tube. When he opened the pot hot material flowed out the opening causing burns to his forearms and left wrist. He received 2nd degree burns to his left wrist. He was treated & released to restricted duty.

EE WAS SETTING METAL LINERS IN PLACE WHEN FINGER GOT CAUGHT BETWEEN THE LINER AND MILL SHELL FRACTURING THE TIP. EE WORKED NIGHT SHIFT. RIF'D MORNING OF 2/23/05

Crew was removing metal liner plates when one plate shifted and came down onto the employee's boot mashing the steel toe of his boot onto his foot causing fractured toes.

The ee was picking up worn out steel liner plates that had been dumped on the ground. he was placing them in wood boxes for shipment back to the many factor. while doing this he noted his elbow hurting but continued. he did not seek medical advice until 8/9/04 and was given an elbow support strap and a prescription. he was allowed to work at full duty. at a follow up.
