While lowering ladder in the back of the truck to reach the manway. Driver released the safety pin of the ladder with right hand while holding the top section with their left. Somehow their right hand got caught like a vise between the ladder and its support crushing their right index finger.

Contractor driver from Midwest Industrial Supply pinched or crushed their right index finger in a ladder they were lowering on their truck. The ladder hinges down after a release of a pin. Right index finger was lacerated from the incident. I was informed by their company that part of their finger was amputated at the hospital.

Employee was reinstalling the bonnet on the 307-A motor. During this process EE felt a strain in the lower left side of EE's back. EE reported it to EE's supervisor and continued working until 10:30 pm. The next morning EE felt stiff and was experiencing some discomfort so EE went to the local clinic. EE was diagnosed with a muscle strain and prescribed a mild muscle relaxer.

Employee was climbing the ladder to the 901 Dust Collector, at the top, hand grip slipped causing the employee to fall forward causing a laceration to arm pit on the metal latch for the safety chain. The laceration required the employee to undergo a procedure to close the wound.

Employee was cleaning near the Pan Conveyor at 731-B East side. While removing material from the walkway, the employee slipped on uneven ground, and the left hand contacted the moving conveyor, which lacked proper guarding. Onsite personnel assisted and transported the employee to a medical facility for stitches on two fingers.

Contractor employee was cleaning the east walkway of the 731-B conveyor. While EE was moving material EE slipped on the uneven material EE was cleaning up. As EE fell EE reached out with EE's left hand and hit the moving conveyor. EE was taken to the local hospital and received stitched on two fingers. Ongoing investigation in progress to why the guard was removed.

A Production employee was carrying a ladder across level ground when they stumbled and fell. They dropped the ladder and put their hands out to try and catch themselves. The impact with the ground caused a radius fracture of the left wrist

Employee reached in to grab a porta power that they saw starting to slip. They put their hands in the line of fire and when the shoring spun it impacted their right thumb. Employee was taken to the local clinic where an X-ray showed an acute non displaced fracture. Employee was instructed to wear a finger splint and released to return to work.

Employee was working in the preheater tower; EE was wearing a face shield and safety glasses. When EE lifted up the face shield the dust that accumulated on the rim of the shield fell into EE's face getting dust in EE's left eye. The employees' eyes were flushed immediately, the employee followed up with a clinic and was prescribed prescription eye drops.

Employee was trying to reinstall a inspection port on a air slide, some cement flushed, and some sprayed up and impacted EE's face. EE rinsed EE's eyes out to flush any foreign material. After EE's shift, the employee was still feeling some irritation and went to the emergency room where EE was given prescription eye drops.

An employee was going to retrieve the manufacturer part number off the blower pulley sheave. After looking at the sheave, EE decided to remove the bolt that was holding the sheave. EE grabbed the bolt and twisted it out of position. In doing so, EE's right ring finger was grabbed by the belt and pulled into the sheave.

Employee reached down to adjust a pallet on the palletizer without deenergizing the system. Once the employee moved the pallet a few inches the sensor started the conveyor because it thought there was not a pallet there. The tip of the employee's left index finger was severed by the conveyor chain once it energized.

Employee looked up and dust from the rim of EE's face shield fell into EE's left eye. EE was wearing safety glasses under the face shield. flushed eye and finished shift. Went to doctor next morning and was prescribed eye drops, nothing found in EE's eye.

While working on the tower in a hot suit, the employee's arms and legs started to cramp: And employee became dehydrated.

EE was having trouble with coal shaker hoist cables making limit switches. EE attempted to manipulate cables to contact switches. EE had hold of a cable to guide it and told EE on ground to jog hoist. In doing so, right index finger got pinched in drum causing crushing injury / open fracture to tip of finger.

Employee was cutting bolts to separate flanges / boom sections on material reclaimer in order to remove sprocket/shaft assembly. Although boom was supported when cutting last bolt, boom section shifted back catching EE's right foot underneath it causing crushing injury and contusions.

Shipping employee placed hands in harm's way while closing silo elevator doors. Employee attempted to use nylon strap which was broken and too short to pull doors together. In the process, employee's right hand was caught between the upper and lower doors.

Employee working with another to remove large guard from head of elevator to replace chain. In process, employee's hand (finger) got caught between edge of guard and elevator housing causing middle finger to be crushed. EE wearing ANSI A4 impact resistant gloves as required.

Clinker dust fell from overhead pipe while employee was working to connect and run air hose along pipe.

Employee was removing hydraulic fitting under pressure. Hydraulic fluid under pressure struck employee in right side of head causing laceration approx. 4" long requiring emergency room treatment.

Man traveling in service elevator to top of "old silos" became trapped when elevator stopped between levels NOTE: was originally reported to Safety 11/4 and relayed incorrectly to MSHA One Call occurred with preheater tower elevator. Elev svc was contacted immediately, came from San Antonio and had man out at 21:20. Elev was LOTO. Svc Co returned 11/5/21 to make repairs.

Employee part of maintenance crew removing analyzer probe housing from back of kiln. While working to guide probe and rigging around obstacles, rigging slipped causing weight to shift and end of probe housing to fall striking employee on instep of left foot behind safety toe of boot causing foot to be broken.

Pump running hot and cavitating. Employee and Supervisor working to correct operational condition.

The employee's right thumb was smashed between a pallet and the trailer deck that the employee was offloading.

Using jackhammer to chip concrete on several occasions over past 2 weeks. Fill buckets with concrete and carry to debris chute to dump. Strain/sprain to right wrist causing pain, numbness, and decreased mobility. Wanted doctor to look at wrist the afternoon of 6-2-2021 and was placed on restrictions.

Air lance broke through the packed material, the air lance lunged downward further, and the employee's left pinky finger was pinched between the lance and the hopper wall.

On 5/11/21, the employee wasn't sure what EE had done. After a 12 hour shift, EE's right knee began to hurt. EE didn't think much about it until 5/18/21 when EE's knee was still hurting, and EE wanted to get checked out. I took EE to the doctor on 5/19/21, were restricted duty, and a couple of therapy sessions were ordered

While air lancing the valve and coupling of an air lance impacted the employee's left hand and wrist.

Employee knelt down and felt a pop in left knee.

Employee was moving filing cabinet and other miscellaneous items and felt a pull in right shoulder.

A piece of metal broke off a bull pin (Metal striking tool) and embedded in employees left bicep. (1/4" x 1/4")

Employees left index finger came into contact with an a/c fan blade.

Employee stepped backward, tripped on the angle iron frame of the kiln cooling fan base. While the employee was falling, they extended right arm trying to brace the fall. The employee landed on the concrete/metal pier pedestal. The employee felt pain in right shoulder and right arm. MRI results were confirmed on 3/16/2020

Employee's right index, middle and ring finger were caught in a rotary feeder.

Rigger was tasked with assisting in stowing the jib on a crane. While walking the hoist line, EE felt EE's muscles cramp. EE told EE's operator about the cramps, and EE's operator told EE to go sit in the truck. While walking to the truck, EE's muscles began to cramp again and EE laid down and called for help.

As employee was climbing down a ladder, EE missed the last step and sprained right ankle.

Employee was working in close proximity to a moving forklift, and forklift bumped into employee's right ankle.

Rolled fingers into shive

Employee was using a five-foot long pry bar on a support tab to help move the finish mill feed chute. The chute was wedged between two 6 inch I-beams. While applying pressure on the bar the chute shifted and the bar slipped and struck the employee on the right side rib. Employees left middle finger was impacted, between the bar and one of the 6 inch I-beams.

While air lancing a plugged material chute, material blew back towards employee and went up under EE's safety glasses and material got into both eyes.

Loader impacted the man lift in the rear tire and body shroud. The impact caused the boom and basket to swing violently creating an impact between employee and the hand rail of the basket.

Repetitive motion while using a material handling vibrator, walking up and down stairs, to get to silo that employee was working on

While placing liners in the finish mill, mill liner fell from mill and struck employee

Hot material fell onto jacket, then material fell down on top of employees right hand. (Submitting now, due to information obtained on 3/12/2018)

Employee was hit in the nose by a hydraulic ram hose (Being submitted now, due to information obtained on 3/12/2018)

Employee was pushing on the cables of a hoist, the left ring finger and pinky finger were pulled into the pulley of the hoist.

Employee was using an air lance to clear a plugged hopper, hot material flushed out and hit employee in the chest, material then traveled upward going under face shield and safety glasses landing on employees left side, eye, nose and cheek. (Prescription meds)

On 1/18/2017 employee was walking and slipped in the mud and jarred shoulder after couple of weeks of shoulder not getting any better. An MRI was performed, results were given on 2/24/2017 and diagnosis was two torn rotator cuff tendons

Employee suffered a non-occupational, non-reportable stroke. EE was taken to the hospital for treatment where EE received stitches for a slight wound sustained when EE collapsed from the stroke. These stitches did not result in a lost time injury.

Employee was rolling a steel plate on a steel rolling machine when EE's left hand was pinned in between the plate and top roll of the machine.
