A piece of steel with a lifting eye welded on top of it was sitting in the upright position for lifting. The employee attempted to lay the piece of steel over to keep it from possibly falling over and mashing someone's foot. When EE did this the glove became caught on the lifting lug and EE was unable to get hand away.

IT SLID DOWN THE PILE AND CAUGHT ONE EMPLOYEE'S LEFT THUMB BETWEEN IT AND THE STEEL FEED THROAT OF THE CRUSHER, BREAKING HIS LEFT THUMB.
EE STATED THAT WHILE SETTING A NEW BEARING HOUSING ASSEMBLY INTO PLACE ON THE CONCRETE SAND SCREW, THE HOUSING SLIPPED/FELL 3-4 INCHES CRUSHING FINGER BETWEEN HOUSING AND ANGLE IRON FRAMEWORK ON REAR OF CONCRETE SAND SCREW RESULTING IN A LACERATION. EE WAS WEARING LEATHER WORK GLOVES AT TIME OF INCIDENT.

EE WAS PICKING UP A BELT ROLLER TO LOAD ON WELDING TRAILER. WHEN HE PULLED A MUSCLE IN HIS BACK.
EE WAS SHOVELING WET, FROZEN MATERIAL FROM UNDER FEEDER. WHILE SHOVELING FELT SHARP PAIN IN RIGHT SHOULDER. COMPLETED SHIFT 11-24. REPORTED AND WORKED 11-27-00. DID NOT COMPLAIN OTHER THAN SAI D HE WAS SORE. 11-28-00 CAME TO WORK AND COULD HARDLY MOVE RIGHT ARM. TOOK TO ER LOCAL HOSPITAL. RECEIVED SHOT AND SLING. TO BE OFF UNTIL 11-30-00. DR. NOW THINKS POSSIBLE ROTATOR CUFF PROBLE
PULLING SAMPLES OUT OF CONVEYOR 60-70 LIFTED UP AND HAD SHARP PAINS IN LOWER BACK.
