EE was removing an impeller from a pump. The crew attached a hanger to the impeller and EE began to backspin the shaft by pulling the belt. The momentum of the belt pulled EE's finger between the belt and the sheave. EE jerked EE's hand out, but EE's glove remained pinched. The event degloved skin and the fingernail and crushed the tip of EE's right middle finger.

Employee was lifting a belt clamp/valve into position then pulled to tighten on belt line, causing an injury with pain to the left lower side of back. EE was treated by a medical professional for sprain/strain, and prescribed medications/pt. Restrictions are as follows: No lifting/carrying over 20lbs, no pushing/pulling, no reaching below waist, no stooping/bending.

Employee was unhooking a large chain being held by the crane with no load on the chain. The chain became unbalanced and run thru the hook and the end came around and struck the employee on the right flank causing a contusion and two minor abrasions. The employee was terminated for not following company policy (positive drug screen).

While conducting maintenance on an underground vehicle, EE was opening the lid of the ore hauler, the lid slipped and pinched their left thumb between the lid and the frame. EE suffered a partial amputation to left hand, left thumb. Restrictions are as follows: No use of left hand

While repairing track on continuous miner, employee's thumb was caught between wooden block and gathering pan of continuous miner. Employee was wearing gloves at time of incident. Immediate Action Taken: First aid was performed, and incident was reported to supervisor. Employee was transported to local hospital for further treatment. Investigation started.

EE took toolbox out of the locker and loaded to the mantrip and felt a pop in their lower back. EE has been ordered PT 2 times a week for 4 weeks, a muscle relaxer to help with sleep. The restrictions are as follows: No lifting or carrying over 20 lbs, no pulling or pushing over 30 lbs, no reaching below waist level, and no stooping or kneeling as of 12/4/2024.

EE states that on 9/8/24, EE was replacing a drive belt in the tunnel in an awkward position that caused pain to EE's middle back. EE has been working without accommodations since the injury. Restrictions on 11/4 after unable to accommodate: No lifting, pushing, pulling or 10lbs of force, no climbing.

While shoveling out tail roller on Area 7 #4 Drive tail. Employee stepped back to discharge shovel full of muck back onto beltline when employee's left foot lost footing and all weight shifted to left leg. Employee felt a sharp pain down lower back on the left side and down left leg.

EE states EE closed the door to the mobile equipment onto right middle finger. EE has a distal tuft fracture in right middle finger. This is a small fracture at the tip of the finger. There is no indication for any further treatment for this type of fracture.

Wound to the right elbow after striking the area against the interior of a pipe. Became recordable when RX was given and restrictions put in place on 6/6/24 Restrictions: Sedentary Duty. No use of RT elbow. RX Kelex

Operator just finished removing wet feed from discharge chute & was replacing the grating on the elevator discharge chute to close up. Operator did have gloves on & when lifted up the grating slipped out hand & ended up cutting glove & employee's finger. EE received 6 stitches & Rx antibiotic. No Restrictions. Stitches came out & had to be restitched 3/14. No use of hand till 3/27.

Employee was performing vehicle inspection on loader and was wearing gloves. During inspection was closing back door and caught finger on back gate. EE received cut on RT hand right hand ring finger. EE Has nondisplaced fracture to right hand ring finger. Restrictions are no pulling or pushing or lifting.

Abrasion on right shin after being struck by wire rope cable while replacing dragline bucket dump rope on 12/16/23. After conservative treatment, EE was given prescription medication and placed on sedentary duty on 12/26/23 resulting in the incident becoming a recordable.

Employee was moving a 12-inch by 12-inch cribbing block at #5 Shaft. While adjusting block on pallet, EE's left hand slipped off the block causing EE's ring finger to wrap around the nylon webbing handle connected to the cribbing block. Employee pulled finger from nylon webbing handle and fracturing finger on LT hand. Restrictions are: Right hand only work

EE stated the pipe pushed EE back at the level of the chest and knocked EE onto EE's bottom. The MRI was on 8/15/2023. On 8/23/23 EE was placed on restrictions.

At the time of the accident, EE was traversing through a wetland on foot while performing vegetation management practices as part of the mine's reclamation processes. As EE entered an area that was densely populated w/ various types of vegetation, a Bulrush stem slid under EE's safety glasses and made contact w/ left eye, causing an abrasion of the left cornea.

Employee was removing the lower mounting plate on the leaf spring when it was loosed it fell striking the employee's left hand ring finger causing a laceration. Employee was wearing gloves.

EE was replacing a brake shoe on locomotive 3804 at South Fort Meade on the right side of journal box 2, when the shoe slipped off of the brake shoe holder pinching their right pinky between the brake rigging and a 25lb brake shoe at a drop of 3" to 4". Xray completed 4/18/22 that showed, nondisplaced fracture of the right Pinky.

Employee was moving a flanged, 4' HDPE pipe out of the way by hand. As EE moved the piping EE's left middle finger was caught between the 4' HDPE flange and the stationary steel piping.

Crew was working at the Mosaic Ona location. The crew went to take their lunch break, one employee went in to take lunch in the conex. The employee in the conex was attempting to open the window to let air in. As they were trying to slide the window open with their hands the glass broke. As a result, the employee received a laceration to the palm of the right hand.

EE states EE was taking the cable off the tub on the #18 dragline and felt a pop in EE's left arm. EE states pain with movement "7"/10 at rest "2"/10. observed bicep deformity. Deformity noted on upper bicep/shoulder. A gap in muscle at shoulder attachment. bicep muscle firm. No bruising or swelling at this time.

EE was closing gate and chain when left hand middle finger was struck. EE received Pain Medication, Stiches and was placed on Restricted Duty.

EE got soda ash on hands, wiping eyes to remove sweat, soda ash got into right eye. EE was treated with prescription eyedrops.

Employee was traveling from area 9 to # 5 shaft and something blew into right eye, Employee wiped eye with towel. Later employee contacted supervisor and supervisor contacted safety dept.

Emp. was on the bottom floor of the KMAG Screening Plant planning clean up activities when EE was struck on the left shoulder/neck by a piece of product buildup falling from above. Emp. taken to ER and placed on restrictions and given prescription meds.

Section of handrail traveling down the stairway towards group of EEs. The ee nearest the landing heard the noise and saw the handrail coming down the stairwell at ee. In an attempt to protect self and the other ee, ee reached out to stop the handrail and left middle finger was pinched between the handrail and the outside wall of the screen.

While aligning the faceplate to the pump shell for a centrifugal slurry pump being reassembled in the mine field, a maintenance employee injured their left ring finger when it was caught between the two components. The employee was wearing gloves at the time of the incident.

Emp was descending the stairway from the electric shop breakroom and felt a pop in EE's knee. Emp. was taken to the company clinic on 7/27/21 and was placed on restrictions and given orders for physical therapy.

Emp. was carrying two buckets of miner bits each weighing approximately 26 pounds to the mantrip. The employee tripped and lost their footing causing employee to drop bit buckets and in the process twisted and strained back. Emp. sought medical assistance on their own on 7/27/21 and was given 2 injections and a prescription for a muscle relaxer.

Emp. stepping down ladder from stacker underground and right knee popped, a few min. later emp. felt pain. Emp. had MRI on 7/26/21, working without restrictions. Emp. was ordered physical therapy visits on 8/5/21 and continues to work without restrictions.

Employee was getting into svc truck and hit head on the door jam.

Employee was loading old rollers into the loader bucket for disposal and smashed EE's left little finger between two rollers.

While climbing down off the 16 dragline EE felt discomfort in EE's left knee. EE was seen by doctor on 7/1/2021 where injury was determined and restrictions were placed on EE, making the injury recordable.

Employee was in the process of moving a portable water cannon while performing housekeeping duties. During this process, they pinched a thumb between the handle of the water cannon and a pipe resulting in a laceration to the right thumb.

As an impeller was lowered, it made contact with the side of the shell causing a metal particle to become dislodged and enter miner's eye. Approved safety glasses were being worn at the time. Eye doctor had to physically remove debris from eye and miner was issued prescription eye drops.

Miner was in the process of rotating a coupling, on dragline pumping system. As miner slid hand down toward the end of the bar, EE's hand slipped off resulting in a laceration to the webbing between the thumb and forefinger. The employee was wearing gloves at the time the incident occurred. Miner has received pain meds, sutures, and is on restricted duty.

EE was discarding used cutter teeth into the scrap bin when EE felt a pain in EE's left shoulder. A short time later while changing teeth on the Liberator dredge EE felt a throbbing pain and numbness. EE then contacted supervision to report the incident. EE referred to PT on 3/19/2021

Miner pinched the tip of EE's right middle finger as they were closing the door to the Payloader. The Miner had entered and was seated in the cab of the equipment, when they pulled the door closed they caught a finger between the door and frame of the equipment.

While in the process of loading a flatbed truck, the tip of a maintenance employee's left index finger was caught between a pump impeller and a shaft sleeve. The injury occurred when the employee lost their grip while lowering the sleeve into position next to the impeller.

Emp was lifting an 8' ladder onto shoulder & back. As emp swung the ladder into place emp felt a pain radiating from left elbow and along forearm. Emp. had Cortisone injection to the left elbow on 9.24.20. Emp. on the following restrictions: light duty, sedentary work. No lost time.

When dismounting equipment (tractor) employee felt EE's right knee pop. Miner was treated conservatively, after worsening pain the injured was placed on restrictions by physician until diagnosis from orthopedic physician.

On 12/5/19 employee experienced back pain while pulling & lifting dragline power cable onto the tub. Employee was treated conservatively; Rx meds were prescribed and MRI scheduled on 1/24/19. Restrictions were issued on 1/28/20.

Employee felt discomfort in the left forearm while changing out rollers on the belt system. On 1/6/20 employee had follow up appointment with company NP who ordered OT since conservative treatment has not been successful.

EE stated was "working under the trap, lifting a 2x2 steel bar (short piece) when I felt a pain in muscle in the right shoulder." EE had MRI on 11/22/2019 showing that the EE has 2 complete tendon tears and the bicep tendon is completely unattached. The EE was placed on Restricted Duty at that time.

Employee trying to free wedged pump between pump truck bed and pump. As Operator raised the pump, employee's thumb was caught between the truck bed and pump, resulting in left thumb injury

EE states that they placed block pipe down on block with the retention build up and it slid back and struck shin causing EE unable to move leg and fell back. Possible Meniscus Tear in left knee.

Employee was in the process of tossing 5# extension cord up one level on dragline and felt pain in left shoulder.

Employee was moving a piece of pipe on a trailer and EE's right hand ring finger was injured due to impact between the pipe and the steel headache structure on the trailer.

EE was removing a roller on UG belt conveyor, right hand was on top of the structure rail. As EE was pushing the roller with EE's left hand, the roller slipped & rolled onto EE's little finger of EE's right hand, pinching/cutting the tip of the finger. EE wearing gloves at the time of injury. EE taken to clinic & received 4 stitches, antibiotic, pain med & placed on restrictions.

EE was throwing a switch at the bottom of the rail yard at Agrock when ee felt a sting in left shoulder.EE noticed stiffness to upper shoulder and when raising arm, ee applied cream but stated it's slightly worse. EE had MRI on 5/1/18 at which time was determined to be work related and put on restrictive duty.
