EE was taking the cutting edge off of a loader. A bolt slipped, EE reached for it cutting EE's hand open. EE received 13 stitches.

EE went to work on the replacement of middle deck screen weighing approx 275lbs. While EE was on bottom deck (pushing screen up to get over feed lip), other EE's were pulling the screen out, when pulling out, the screen got caught on something, it got directed back into shaker, falling downward, striking EE leg.

EE was washing out a screen deck. As EE was performing this duty EE felt a pop in EE's back, causing pain.

Employee was preforming routine duties by picking up deliveries, when they lifted a package up hitting their head on the garage door. While the employee was wearing a hard hat at the time, they felt a pain in their neck as a result. Employee is currently resting at home after medical attention, expected to return next week.

Employee was disconnecting a small water line. When kneeling down/bending over employee felt a sharp pain in lower back (left side)

Shoveling material off of belt, when they tossed the stone with shovel in hand heard a "POP" in left shoulder.

Replacing rubber punch plat screens. Pushing prying screen panels into position on incline surface.

Employee was working on screen doing a product screen change. EE was working with another employee. EE was in the hopper under the screen removing hardware for center hold down strip when a rock fell through the screen hitting them in the back of the neck.

Removing rock blockage from chute. A large rock slipped and thumb was under it and landed on another rock. Left thumb broken.

Employee picked up a 80 pound piece of steel and while holding it slid out of their hands and landed on top of boot. Employee stated that it did not hit the steel part of the boot.

While transferring grease from one barrel to the other...the barrel slid and cut the forearm of an employee.

While greasing the cylinder on the Dozer the cylinder head did not appear to be getting the grease. The employee looked over the cylinder head grease fitting and it popped off and struck EE in the right eye

Emp suffered a burn to left hand while heating up an elbow. EE used bare hand to re position it.

Ground man was cleaning tail screen (using hands) a rock fell and struck rt thumb....resulting in 9 stitches.

Contractor Employee was working on tracks of excavator and had to remove built up dry dirt to get to the bolts. As they were scrapping the dried dirt, a small sliver of metal became embedded into the left forearm

Employee was carrying concrete forms from one location on site to another. Felt pain in shoulder and neck. Went to doctor to get evaluated.

Employee was inside a head-box cleaning it out. As employee went to exit the box employee felt a strain in employee's back.

Employee was lifting a 50 pound 20 foot long piece of angle iron and felt a muscle pull in rib cage. Employee went to clinic and returned to work with lifting restrictions.

WHILE TWISTING AND PULLING ON A BLOCKED TRANSMISSION (WORKING UNDER A WATER TRUCK) EMPLOYEE FELT A POP IN HIS LEFT SHOULDER

Employee was repairing a tear in a conveyor belt. While pushing down on the belt clip a sharp edge, his rt index finger was cut

Employee was working with a partner taking out bolts on the primary crusher wear bar. one employee was working the ratchet while the other supported the socket. As the socket started to slip off the bolt the employee who was injured reached to support the socket when the socket slipped off the bolt and the employee was struck by the ratchet... injuring his rt thumb.

Employee failed to use the proper tool when lining up 2 pipes and caught his finger in the gap, cutting the tip of the left index finger.

Employee was removing a bucket pin and lost control pinching his finger.

Employee was removing load cells from a platform scale and the scale shifted and caught the tip of the index finger and severed approx. 3/8" of the finger.

Employee bent over to pick up his hard hat and when he stood back up to put it on he hit his head on the tail pulley on belt #3. He cut his head and had to have seven staples put in his head.

EE was picking clay off the belt and turned and hurt his back.

Employee was changing screens and pulled a muscle in his back.

Employee started to climb in the loader and felt something pop in his back.

Employee was carrying a gear box and injured his back.

Employee was shoveling and hurt his back.

Employee was trying to move a 4" water pump which resulted in a groin strain.

Employee reached into a heater fan that he was working on to retrieve something that had fallen in. The fan came on and cut his 3rd finger on his right hand. The cut required stitches.

Employee was helping remove a rock from a truck tailgate. The rock fell out and the tailgate shut on the employee's hand cutting and pinching his two middle fingers on his right hand. He required 8 stitches in his fingers.

Employee was changing differential oil on a Cat 775 and stood up and stuck his head, causing a small cut.

EMPLOYEE WAS DUMPING A BAG OF FLOOR DRY INTO THE CONTAINER WHEN HE HAD A SHARP PAIN IN HIS LOWER BACK.

Employee was cleaning out rock chute when he hit his right wrist on the chute bruising it.

EE WAS USING A SHORT HANDLED HOE TO PUSH THE SKIRTING RUBBER BACK ONTO BELT #37. THE HOE SLIPPED CAUSING HIM TO STRIKE HIS HAND ON A RETURN ROLLER BRACKET CUTTING HIS FINGER.

EMPLOYEE WAS HELPING REMOVE A MOTOR AND HIS RIGHT HAND GOT PINCHED BETWEEN THE MOTOR AND THE FRAME.

HE GOT IN DOZER AND WAS SETTING DOWN AND DOOR SHUT ON HIS THUMB.

WAS TAKING CAP OFF OF HYDRAULIC TANK, WHILE THE LOADER WAS RUNNING. THE TANK HER PRESSURE BEHIND IT AND THREW HOT OIL OUT.

WHILE CHANGING SCREENS ON A SCREEN DECK, HE PINCHED HIS MIDDLE FINGER LEFT HAND IN SCREEN.

EE WAS CLEANING ROCKS OUT OF THE CRUSHER, WHEN A ROCK FELL DOWN, STRIKING HIM IN THE BACK.

EE WAS PUTTING SCREENS BACK ON AND SMASHED HIS FINGER WITH THE SCREEN.

EE WAS REMOVING A FUEL FILTER FROM THE FD-30 DOZER. WHILE PULLING ON THE FILTER WRENCH HE FELT SOME PAIN IN HIS STOMACH. THE DR DIAGNOSED THE INJURY AS A HERNIA. THERE HAS BENN O LOST TIME TOD ATE, PENDING SURGERY TO REPAIR HERNIA

EE WAS WORKING THE MAIN FEED BELT WHEN A BLOCK LODGED. EE GOT UP ON THE BELT AND PULLED THE BLOCK TOWARDS HIMSELF WHILE BEING BENT OVER. EE THEN GOT OFF THE BELT AND WHEN GOING TO STAND UP, FE LT PAIN IN THE LOWER PART OF HIS BACK.

EE WAS GREASING A TAIL PULLEY ON A DUST JACKBELT WHEN A PIECE OF STEEL ABOUT 5"X6" FELL FROM THE #2 8X20 AND HIT HIM ON THE BACK OF THE LEFT HAND AND THE LEFT FOOT.

EE WAS CHANGING WHEEL ON TRUCK. UPON REMOVING WHEEL FROM VEHICLE, THE RIM SLIPPED MASHING THE 3RD DIGIT OF THE LEFT HAND CAUSING A FRACTURE AND LACERATION.

EMPLOYEE WAS REMOVING BOTTOM PIPE FROM CYCLONE. 3 BOLTS WERE REMOVED HOLDING THE PIPE. PIPE WAS RESTING INSIDE ANOTHER PIECE OF PIPE HELD BY A PEICE OF FLAT BAR WELDED TO THE SIDE. THE TOP PIP E BROKE FREE FROM THE FLAT BAR PINNING THUMB UNDER THE FLANGE.

EE WAS BETWEEN THE FRAME AND THE TIRE REPLACING AN AIR HOSE. HE TWISTED TO HIS LEFT TO REACH THE AIR LINE AND FELT A SHARP PAIN IN THE LOWER BACK.
