During grinding an employee stopped to wipe sweat from his forehead and eyes. Afterwards he began to feel irritation in his left eye. He proceeded with eyewash.After 15 minutes of flushing irritation was still present so he was driving to a medical center to have sliver of metal removed from eye. He was released to full duty afterword and reported back to work that afternoon

Doing regular routine work and later noticed my right eye was irritated and very red and felt like something was in it while working at SX Plant.

Employee was positioning idler on pallet and got left pinky finger caught between the idler and the pallet causing the end to be cut off.

Laceration to finger

While cleaning out tail area of feeder with water hose, materials sprayed back and some got into left eye.

Uncontrolled lime dust, water creates a muddy, slippery situation, pulled wire was coiled up on top of pipe rack-when pulled down, lime mud spalsher on face and goggles. When goggles were removed, lime entered eye.

Employee tried to swing heavy tool box up onto the back of a truck and felt pain in right forearm. EMT checked out and employee returned to work. On 10/08/08 employee requested to be checked out because the arm was still hurting. Taken to doctor and given a shot.

While removing an Outer Eccentric Bushing from #1 Crusher Mainframe employee cut his arm when he and another employee were lifting a broken piece of brass from the crusher. While lifting the brass from the Crusher, partner swung the brass toward himself and employee let go. The brass made contact with left forearm cutting it.

Employee was moving 6" hope pipe and caught right hand 4th digit between two pieces smashing finger at the fingernail.

Mounting tire on CAT 785 haul truck. After holding retaining ring and ring was on wheel, the forklift released the tire and tire fell on right hand. Compound fracture of pinky finger and tendon damage to ring finger, both on right hand.

Removing sink with co-worker in men's restroom. Moved the sink on one side and then moved the other side - Third finger on left hand got smashed.

Employee was moving wooden cribbing and felt a pain in his lower back.

Employee was working on the 994 loader when a 200 Lb lip shroud fell on employees left foot.

The employee was trying to move a four inch check valve off a slope. He pulled the valve to him and when the valve broke free his thumb was smashed between the valve and a large rock. The valve weighed around thirty pounds.

Employee was walking up stairs at fine crusher from #2 tunnel to apron feeders, while using the handrail, left shoulder popped out of socket.

EE was transceting (surveying) land on the Tonto National Forest - adjacent to the Pinto Valley Mine. Even though he was wearing protective eyewear, a branch from a mesquite tree scratched his cornea on his left eye. EE was seen by Dr in Globe, AZ. He was given Vigamox drops and was told to see a doctor on Thursday, 8/30/07.

Employee was preparing to cut a piece of steel grating and swatted at a bug, catching safety glasses. The earpiece pulled away and struck his right eye.

# employees were trying to pick up the pipe high enought to slip a strap underneath so they could rig up their chain fall to hang the piece. Where it needed to go. Pipe was prepped with anti seize which caused it to be slippery. One employee had pipe slip out of his hands, causing it to fall on Ee s finger.

Employee was digging out under a 6" service water line that was leaking when be felt a pop in his back. X-rays taken @ hospital-no injues seen, light duty ordered for one day.

Employee was buffing stainless steel blanks, lifted edge of blank to move around on table. Felt pain in left shoulder.

Employee was lifting sheet rock - possibly twisted body wrong.

Employee caught finger between rigging and motor while taking up slack on slings prior to removing the motor from a flat bed truck located in the Electric Shop.

Employee was helping to carry an office desk up the stairway in the Mill when his left knee popped and injury occurred. Difficult to walk or to put weight on left leg. CVCH report - Tear Med. Meniscus (left) Knee.

As EE was attempting to step over rail, the rail was not secured properly & when his weight was put on the rail the rail moved pinching his finger between the rail & the floor. EE was taken to hospital for treatment. Received sutures on left ring finger.

While stripping copper off of starter sheets, the sheet broke off the rack and hit him on the steel cap of his left foot causing a fracture to little toe. He was examined and immediately released to return to work.

EE strained his rt knee while boarding #830 loader. He climbed the ladder w/both hands on the railing & rt foot on ladder. E.R. dr asked that he see his personal physician though it was unclear at the time whether it was for his knee or a personal medical problem he was experiencing. Further tests were ordered by his dr & he was not released to work

WHILE INSTALLING PIPE AT THE OVERLAND PUMPS THE STINGER OPERATOR WAS POSITIONING THE PIPE. WHEN THE PIPE WAS RAISED, IT SLIPPED & PINNED THE VICTIM BETWEEN THE PIPE ELBOW & THE PUMP MOTOR. HIS LEFT KNEE TOOK THE BRUNT OF THE IMPACT. HE WAS NOT RELEASED TO WORK & WAS EVALUATED BY AN ORTHOPEDIC SURGEON.

WHILE PULLING BANDING STRAP FROM A REEL, THE BANDING STRAP SLIPPED CUTTING THROUGH BOTH THE NEOPRENE AND KEVLAR GLOVES, SLICING THE EE'S RIGHT THUMB. THE LACERATION REQUIRED 5 SUTURES. THE EE WAS RELEASED TO IMMEDIATELY RETURN TO WORK.

WHILE SECURING BEAM STRAP, WORKER PLACED ARM THROUGH BROKEN WINDOW, CUTTING RIGHT FOREARM.

WHILE TRYING TO LIFT A 78 LB BOX TO PLACE IT IN A VEHICLE FOR TRANSPORT TO THE PLANT WAREHOUSE, EMPLOYEE REALIZED THE BOX WAS TOO HEAVY AFTER GETTING IT 3 OR 4 INCHES UP. WITHIN 1/2 OF AN HOUR EMPLOYEE WAS EXPERIENCING SEVERE PAIN IN RIGHT BUTT CHEEK AND DOWN BACK OF RIGHT LEG.

EMPLOYEE BENT DOWN TO CHECK THE GROWTH IN CELL #73 AT THE PLANT. HIS RADIO CLICK BROKE AND THE RADIO FELL INTO THE CELL. HE ATTEMPTED TO SALVAGE THE RADIO AND IN DOING SO CUT HIS RIGHT HAND ON AN ANODE. IT REQUIRED 12 SUTURES. HE WAS RELEASED IMMEDIATELY TO RETURN TO WORK.

WHILE SETTING UP TO WASH THE WATER TRUCK EE POURED NO-290 (ALUMINUM BRIGHTENER) INTO THE WASH BUCKET, POSITIONED THE WAY HE WAS FUMES CAME AND HE BREATHED THEM IN.

EE WAS ON FOOT PATROL IN OFFICE AREA. WHEN ENTERING A DOORWAY SOME DUST FELL AND EE GOT SOME IN LEFT EYE.

EE STRAINED HIS LOWER BACK WHILE PULLING HYDRAULIC HOSES THROUGH THE MUD AT THE #2 TAILINGS EAST MINING SETON JANUARY 26, 2001. THE EE REPORTED THE INCIDENT TO HIS SUPERVISOR ON JANUARY 31, 20 01. HE WAS EXAMINED ON FEBRUARY 2 BY AN ER DR. AND RELEASED TO WORK WITH LIFTING, BENDING, AND SITTING RESTRICTIONS FOR ONE DAY.

EE PINCHE LEFT HAND BETWEEN SAFETY LATCH HANDLE & HANDLE STOP OF STRONGBACK. THE EE WAS WEARING GLOVES BUT REC'D A PINCHING INJURY TO THE PALM OF HIS HAND THAT REQD 3 SUTURES

EE LACERATED HER RIGHT HAND BETWEEN THE THUMB AND INDEX FINGER WHILE BANDING CATHODES. SHE WAS WEARING NEOPRENE GLOVES WITH KEVLAR INSERTS BUT WHEN EE PULLED ON BANDING MATERIAL IT SLIPPED AND CUT HER. SHE WAS EXAMINED BY THE EMERGENCY ROOMDR., GIVEN 2 SUTURES AND A TETANUS SHOT AND GIVEN A FULL RELEASE TO RETURN TO WORK THAT DAY.

WHILE ATTEMPTING TO SECURE A SUCTION INTAKE LINE TO A PUMP EE FELT PAIN IN HIS LOWER ABDOMEN. THE EE WAS SEEN AT THE LOCAL CLINIC ON 2-15-00 AND RELEASED TO WORK WITH RESTRICTIONS.
