He proceeded with eyewash.After 15 minutes of flushing irritation was still present so he was driving to a medical center to have sliver of metal removed from eye. He was released to full duty afterword and reported back to work that afternoon
Employee stepped out of truck and slipped on loose ground.
While deploying liner material on the pond floor a gust of wind reached our work area. The gust lifted the liner material the IE was standing on the liner at the time the wind hit our location. The EE fell to the ground.
While doing the copper harvest, returning starter sheets to the cell, stepped onto cell and foot slipped causing a fall into the cell with one leg which hit the lead anode in the cell at his knee.

Employee was driving a pin out on a loader bucket when a piece of metal flew off and hit left ear. On 11-28-08 he asked to see doctor as ear was sore. X-ray determined that a 2mm piece of metal is lodged in the outer edge of left ear. He has not decided to have the piece of metal removed as of 12-08-08
Laceration to finger
While attempting to walk around some water walking back from crusher employee slipped and fell onto right knee and then while attempting to stand up fell a second time on left shoulder.
While cleaning out tail area of feeder with water hose, materials sprayed back and some got into left eye.
Uncontrolled lime dust, water creates a muddy, slippery situation, pulled wire was coiled up on top of pipe rack-when pulled down, lime mud spalsher on face and goggles. When goggles were removed, lime entered eye.
Employee was in break room when he noticed a burning sensation in left eye, had it washed at the dispensary at 10:00 am and again at 2:00 pm he then went to the clinic and had it checked again the clinic flushed it and sent him back to work with drops.
Employee states he was breaking loose back-up wrench when he sustained an open wound on right little finger.
Employee was attempting to cut a piece of rubber skirting when the utility knife slipped, cutting through left thumbnail and back to the first knuckle. 5 stitches required to close wound.
Employee tried to swing heavy tool box up onto the back of a truck and felt pain in right forearm. EMT checked out and employee returned to work. On 10/08/08 employee requested to be checked out because the arm was still hurting. Taken to doctor and given a shot.
2007 Chevy Silverado, 1092-0010, License # CE766859, VIN # 1GBJK33U87F199695. The vehicle caught on fire, resulting in a total loss. No injuries.
Employee was climbing down off of a haul truck, turned to climb down from bumper area, and fell approximately 4 foot 3 inches to the ground.
EE was getting out of the 933 skid loader and got right index finger stuck in the door handle that is located on the window. The window of the 933 is the door. Employee was opening the door and getting up at the same time. Finger got caught in the latch and twisted finger on handle before letting go. Door is hard to open.
While removing an Outer Eccentric Bushing from #1 Crusher Mainframe employee cut his arm when he and another employee were lifting a broken piece of brass from the crusher. While lifting the brass from the Crusher, partner swung the brass toward himself and employee let go. The brass made contact with left forearm cutting it.
Employee was walking out of the Fine Crusher and slipped in the mud and hit left knee on the cement.
Employee was working on rebuilding a cyclone feed pump. Set some 12"x12"x16" cribbing down to lay the pump casing on. Employee turned to walk to his left and tripped on the cribbing. As he was falling, he put his left hand out to catch himself and landed with all body weight on hand.
Employee was moving 6" hope pipe and caught right hand 4th digit between two pieces smashing finger at the fingernail.
Rock rolled from truck bed, as truck was being loaded, landing on fire extinguisher located on front deck. The fire extinguisher was activated and ABC extinguishing agent got into driver's eyes
Mounting tire on CAT 785 haul truck. After holding retaining ring and ring was on wheel, the forklift released the tire and tire fell on right hand. Compound fracture of pinky finger and tendon damage to ring finger, both on right hand.
Employee was decending the ladder on a cat 994 loader when he slipped. Employee grabbed the hand rail pulling shoulder.
Removing sink with co-worker in men's restroom. Moved the sink on one side and then moved the other side - Third finger on left hand got smashed.

Truck driver was stepping across from the left side of the cab deck to the right side of the cab deck to perform a fluid check for his pre-shift inspection, when he missed a step with his left foot causing him to fall to the ground.
Employee was working in a tight area grinding bad concrete on a wall with a 4" grinder. It had "bucked" back at him several times and the lead man had reinstructed him to not apply so much pressure. When he put the grinder back on the surface he applied so much pressure it broke the blade and kicked back into his left lower leg causing a laceration.
Per the operator's report the dozer was backing up and ran over a rock causing the operator to be "jarred" This incident was not reported until March 4th , which is in violation of our policy. We have reasonable doubt that this injury occured on our property. We will be forwarding more information in order to have this incident receive MSHA's "Category 10" designation.
Employee was trying to open a butterfly valve using pipe wrench in the filter plant (bottom floor), valve turned and smashed thumb between pipe wrench and steel beam.
Employee was working on the 994 loader when a 200 Lb lip shroud fell on employees left foot.
Employee used a pocket knife to open a 1 1/2 gallon grease drum. The knife slipped and cut his middle finger on right hand.

HE REACHED DOWN TO PICK UP SCREWDRIVER FROM CELL FLOOR, AT THE SAME TIME THE BAR ABOVE HIM DISLODGED FROM ITS POSITION & FELL TO THE FLOOR STRIKING HIS RIGHT INDEX FINGER AND CAUSING A LACERATION.
The employee was trying to move a four inch check valve off a slope. He pulled the valve to him and when the valve broke free his thumb was smashed between the valve and a large rock. The valve weighed around thirty pounds.
Employee got out of the Caterpillar D9N Dozer to check on something and slipped outside the cab causing left leg to go between engine compartment and tracks hurting left knee.
Employee was found on the ground in front of a Cat 777D water truck. It appears that the employee fell to the ground from the equipment. There were no witnesses to the event. Autopsy report is pending.

Even though he was wearing protective eyewear, a branch from a mesquite tree scratched his cornea on his left eye. EE was seen by Dr in Globe, AZ. He was given Vigamox drops and was told to see a doctor on Thursday, 8/30/07.
Removing trash from Ball Mill area, started walking backwards to allow truck to go by and stepped on dome grind media losing balance and falling, hitting tailbone on media and a wood plank.
Conditions were dark in a desert terrain work area. EE was dismounting his equipment, when he stepped on a rock and hurt his left ankle. First aid was administered on site and he was taken to the local hospital for evaluation and treatment.
