12-2: Tech reported to supervisor feeling knee pop/pain while squatting to pick up a tray off the floor. 12-3: Tech had knee evaluated & diagnosed w/knee sprain-full duty. 12-10: Day before returning to rotation, follow-up for worsening pain/numbness & requested off-duty until MRI. Lost time began 12-11 pending MRI & follow-up appointments.

The employee was using a sledgehammer to knock out bolts on sag mill liners. During a swing the sledgehammer head slipped off a bolt end causing the employee's right wrist to twist awkwardly which resulted in a sprained right wrist

STS confirmed by audiologist on confirmation test. No referral required.

Employee laying out coiled-up, rigid cables, foot tangled in cable, employee fell onto right arm. Employee only felt pain, therefore did not report immediately. Medical evaluation 7/22/25 and 7/25/25 discovered small fracture in arm bone. Prescribed physical therapy and non-bearing use (restricted days)

A technician was troubleshooting an Enpak that had been intermittently going out. EE was reaching down to inspect the connections and wiring and caught EE's hand and finger on the fan blade causing a laceration. EE received several stitches at the local clinic and was released back to full duty the same shift.

Changing a drill bit, employee pinched finger between bushing and lip on drill mast requiring sutures on EE's right middle finger tip.

Employee pinched their right pinky finger between two bars requiring 4 stitches.

Employee was lifting a suction plate when a hook slipped off the plate and struck the employee in the mouth. On 8/19/24 the employee had mouth and loose teeth evaluated where xray confirmed a fractured alveolar. Teeth were braced for stabilization, and employee will need a root canal and crown in a few weeks.

Light Vehicle technician was team lifting while installing a tire. The other technician pushed too hard and the tire slipped resulting in the tire falling onto the technician's foot.

Employee was standing on 2nd step of a 4 rung step ladder performing a maintenance task under haul truck when they fell, resulting in the pipe they were holding to fall onto their hand. The employee had hand surgery on 12/22/2023.

Employee stepped off the dozer ladder onto the ground and felt pain in their ankle.

Tire technician was installing a lock ring on a loader tire when they pinched their finger between the wheel and the ring causing a laceration that required 4 sutures.

Technician was installing a ripper shank. When EE pulled it in to place the pin, EE's right hand pinky finger was caught in and between the hole and the pin.

Employee was using a hammer to remove GET causing a small piece to contact EE's ear. A small piece of foreign body was removed from EE's ear requiring a suture and returned to work full duty.

Driller on LF-90 Core Drill was removing the bit from the core barrel. The driller put a 36" pipe wrench on the bit and pulled on it. When the bit broke loose the pipe wrench slipped off. This caused the driller to fall with the pipe wrench in EE' hand. EE's thumb was pinched between the deck and pipe wrench on impact. The tip of thumb required 6 stitches to repair the damage.

Employee was changing a tire when the flange fell on EE causing a fracture to EE's right ankle.

Engineer was performing maintenance a on radar unit in the field when they rolled their ankle on a flat surface while stepping backwards. Engineer was initially evaluated and released to full duty. Six months later, the sprain was still bothersome. On 10/23/23 the injury became reportable when they had a repair procedure performed on a tendon.

Employee was cutting a water pipe with a chainsaw and slipped. They sustained a laceration to their shin that required sutures. The employee was released back to work full duty. No lost time or restricted days.

Employee was walking at the tire pad and slipped on ice, twisting right ankle. Technician requested an evaluation and the xray showed a small hairline fracture in the foot.

Warehouse technician was moving a part and failed to use proper 2 person lifting technique. They pinched their finger, causing a laceration and fracture that required stitches on the fingertip of their left ring finger.

Supervisor lifted a garbage can of used hoses. When they flipped it over to dump it, they felt pain in their bicep. They had an MRI a week later and a rupture was identified. On 1/9/23 the incident became reportable medical treatment when the employee had an outpatient procedure to fix the bicep.

Utility crew was pulling shovel cable from frozen mud with pick up truck when cable broke loose from ground and hit tech in EE's knee. Tech is on light duty while doing PT & should wear a brace during recovery. No surgery.

While technicians were attempting to clear the plugged chute for FE003 they had set the 25 lb. jack hammer to the side while shoveling material. The jack hammer began to teeter off the edge of the chute opening and a technician tried to catch it before it fell but the bit swung up and struck EE's right cheek causing small laceration.

Working in bed yard, switching haul beds on haul truck. The last pin was being removed from the bed. When the pin finally released, the bed dropped. It was attached to a crane to prevent this but still dropped. EE's finger was smashed under the bed.

Employee was picking up trash after a maintenance job and lacerated their hand on a crushed used air filter requiring a few sutures.

Slipped carrying trash bag down office stairs. Scraped knee/sprained wrist. Xrays negative, released full duty. Follow up a week later & completed xrays again but they were negative, full duty release. Dr said splint might help with tenderness & keep flow while the sprain heals (preventive, not treatment) this incident was FA until EE used preventive aid splint on 5/26

Employee was cutting a zip tie with a knife and cut knuckle. Employee was wearing gloves but sustained a laceration which required 4 sutures.

Haul truck wheel motor bolts sheered off the axle box causing the right rear tires to break free from truck. Operator and trainer were evacuated from cab with no injury. MSHA was contacted as it took more than 30 minutes to dismount operators. MSHA Investigator came to the mine site and released the incident.

Wheel motors of HTK240 caught fire. Operators cleared truck; sustained no injuries. WTK applied approx 2000 gallons of water. Fire was not able to be extinguished completely. MSHA hotline was called at 30 minutes. EE called back with instructions to seize extinguishing efforts, evacuate area, issue J Order 9613334. Quantification of damage-No injuries/Property damage HTK240

Employee experienced rough loading event. Diagnosed with low back strain and continued full duty. A few weeks later on 11/1/2021 the employee went to the doctor and requested time off because back still hurt. Employee is off duty until employee follows up with a specialist out of town.

Haul truck operator was getting loaded at shovel and experienced lower back pain. Went to ER diagnosed with back strain and advised to take one day off work and ibuprofen as needed.

Employee was performing pre-op when EE fell forward catching self with arm. Initially EE was seen in ER and got a full release, but followed up on 8-12-21 and was put on restricted duty pending right shoulder MRI.

While placing chock under tire, employee felt bicep muscle pain.

Electrician was cutting a wire with a box cutter and cut through their glove and sustained a laceration to right index finger. No sutures were required but electrician was released back to work with restrictions of not lifting more than 5lbs while the cut heals.

Walking to truck in parking lot, slipped on ice, fell to ground & sprained ankle. Initial ER visit was on 3/26 and EE was released to full duty. Became reportable 3/31 at follow up appt when EE was put off duty.

Employee was spraying mud with high pressure hose when water sprayed back in face causing debris to get in eye. EE was wearing safety glasses. Employee had eye irrigated at ER to remove FB and was given an eye drop antibiotic to use for one day.

This is for termination of citation 9503883 Electrician was troubleshooting electric codes on P&H2300 shovel. 600 volt disconnect switch was locked out. Employee gained access through car body into revolving frame and made contact with collector rings and received electric shock to arm and shoulder.

Technician was removing the belly pan from a 16M motor grader that had lost oil pressure and needed to be towed out of an active mine area. EE was using a pry bar to help dislodge the belly pan when it came down and pinched EE's thumb between the pan and pry bar.

Employee had been in the sun for 8+ hours on day off (7-29-20) experiencing sunburn and dehydration. On first shift back 7-30-20, Employee was lubing a shovel when Employee became hot and had heat illness. Employee went to ER where Employee was diagnosed with heat exhaustion and was advised to hydrate and take a day off of work.

A piece of metal chipped off while working on a track and cut employee's leg approx. 2-3cm in diameter that required 4 stitches. Employee returned to work with no restrictions.

Employee was walking around a forklift and tripped and fell on the ground. Employee reported to a supervisor immediately but did not want medical attention. 6 days later employee's side was still hurting and employee saw a doctor who determined employee had fractured a rib. Employee is on restricted duty.

Employee was getting loaded in EE's haul truck by a loader and reached to get a water bottle from EE's lunch bag and the loader made contact with the side of the truck and caused to employee to hit EE's arm on the door resulting in a contusion.

Employee tripped over foot at top of stairs and fell causing back strain and knee injury. Employee missed 6 shifts and then released back to full duty/no restrictions.

Employee experienced a personal medical event and was transported to the hospital by ambulance. EE was given a full duty release to return to work.

While dumping haul truck, hoist cylinder broke and bed came down jarring employee in cab.

Employee was preparing to perform pipeline maintenance when another line EE was not working on (back flush line) came apart causing employee to slip off platform and was soaked with back flush. Employee went to ER to have eyes flushed and diagnosed with contusions/bruises on leg. 3 days light duty and antibiotic eye drops prescribed.

Employee was walking into building at start of shift and tripped. Employee put their hands out to catch themselves and cut the right palm of hand on the door requiring sutures.

Backing up in haul truck to get loaded, bucket hit bed, employee strained shoulder.

Employee pulled hamstring while walking around truck during pre-op resulting in one day of lost time.

Employee was removing plug from haul truck when the wrench slipped and lacerated employee's finger resulting in 4 sutures.

Employee was driving pin on a track hoe and a metal chip went through glove and lacerated finger requiring 2 sutures.

Employee jumped off the ladder of the truck and rolled ankle and then climbed into the cab to look at the ankle and slammed toe in the door also.

This report is being filed to terminate citation #9438261. Employee claims exposure to mixed chemicals caused an illness/reaction that caused a lung issue that was treated by medication.

Employee was turning steering wheel of haul truck and aggravated a pre-existing shoulder injury.

Employee was a passenger in a man van that hit a pot hole causing employee to strain their neck.

Employee was operating shovel and digging in a hard spot and the shovel started jumping around causing employee's back to hurt.

Large rock loaded into haul truck that caused truck to rock side to side, up and down causing a back strain and one day of light duty.

While putting a belt on a pump sheave, middle finger got caught between the belt and sheave.

Removing high tension spring, pulled hook through finger; required a couple stitches. Back to work same day.

While loosening a bolt on a drill, the wrench slipped causing EE to fall backwards. EE tried to catch self and landed on left wrist.

While lowering a 336F final drive to the ground with a crane the technician reached out to grab it and it tipped pinching the tip/pad of the left ring finger off.

Employee was simply walking on even ground and dislocated their knee.

Employee was pulling steel from the feeder and lost footing, causing employee to fall into a wall and employee hit shoulder.

Employee slipped and fell in mud putting wheel chocks down, felt left knee pop

While traveling on haul road, haul truck operator hit a pothole jarring lower back. Initially a report only, the employee reported on 5-23-19 and did not require medical care. Employee went on days off and when returned to work on 5/28/19, went to ER for strained muscle. Began 3 days off on 5/24/19, followed up with dr, restricted duty.

Tire guy climbed into wheel, slipped and smacked knee on the wheel. Initially diagnosed as a knee contusion but MRI revealed a meniscus tear.

Slipped on ice and fell on left hand/wrist in February 2019 while on tie down. At follow up appointment on 4/25/19 a fracture was discovered on MRI.

Employee was operating CAT 793C haul truck and while backing under a Letourneau 1850 loader, experienced rough loading event. Employee was taken to ER and sustained a muscle strain.

While walking across tie line, employee slipped and fell in a tire track that had been packed down with snow/ice. Employee fell on right side (hip, knee, shoulder) of body and sprained knee.

Employee was turning a valve and shoulder popped. Employee treated conservatively with physical therapy for 6 months at full duty/no restrictions but then decided employee wanted to opt for surgery for possible better results.

While operator of HTK280 was being loaded at SHV005, the operator felt the haul truck go up and down and felt pain in the lower back.

Employee had slipped on some ice knocking off the safety glasses. Employee picked up safety glasses and didn't clean them off. Debris got in the right eye causing irritation 5 hours later employee couldn't see out of the right eye.

Orally blew electrical connection out. Corrosion entered under safety glasses and into the eye.

Employee was walking on ice and slipped and fell. Employee was taken to ER on 11/29/18 to be evaluated and was released with no findings. On 12/7/18 employee was still in pain and was taken to an Orthopedic where a hairline fracture was found on elbow.

Employee did not hear the horn while backing up and hit the shovel bucket softly.

Employee noticed the bit was wobbling in the jack hammer and then turned the hammer upside down to inspect finding the bid had broke. Employee removed the protruding part of the bit at which time the hammer was sat down accidently firing the mechanism. When it fired it shot the inner piece of broken bit out hitting employee on the bridge of the nose and safety glasses.

Employee was operating a loader and shovel. Employee did not report having complications till end of shift and only filled out a C-1. On Oct 21, 2018 employee complained of lower back pain and wanted to be scene by a doctor.

Employee was helping contractor unload GET (loader teeth) from the back of a Service Truck. Employee had unloaded 4 teeth successfully. The box normally held 5 teeth. Employee had 3 points of contact but when reaching for the box to pull it forward due to it being empty lost balance due to the little weight and fell off of service truck.

Grader was making a U-turn and hit blade against tire of the Rubber Tire Dozer

Employee initially reported wrist pain but no injury on 8-28-19. Claim was initially denied by WC in October 2018 pending medical investigation. Medical investigation showed underlying arthritis that was nonindustrial related but did indicate industrial illness of left wrist tenosynovitis and surgery was done on 3-8-19. No loss time.

Employee was in a Haul Truck being loaded by a loader. Employee was tensed up bracing for the load to be dropped. Loader jarred Haul Truck by hitting teeth on the headache rack.

Employee and a coworker were disposing of rollers from a conveyor belt. Roller slipped off of hitch and fell on employee's right foot.

Employee reported that the haul truck bed was slamming down when lowering jarring the lower back on 08/01/18. Employee was taken to ER and released to full duty on 8/01/18. On 8/11/18 employee complained about pain in the lower back and was taken back to ER. On 8/12/18 employee was put on Lost Time.

The Mill Operator attempted to pull the chain away from the magnet not knowing that if the dart is moved out of sight of the ultra-sonic cone and the target the dart would drop and the operator had right hand in the pinch point and amputation of two fingers.

Employee was helping guide a service truck into a tight area where a water truck was going to be pumping water to Wash Bay Sump. Right foot got stuck in a hole which caused EE to lose bearings. The left foot was raised up to avoid a hose and when coming down employee dislocated foot causing tendon damage employee tried to fall gracefully but the fibula broke in 3 places.

Employee was backing up in a dozer with body turned to see where they were going. Ran over a rock and jarred the lower back.

During routine drilling operations the driller helper was adding a quill rod. There was slack in the cable which lifts the rods and when the helper moved it the rod slipped and caught their finger between the drill rod and the drill head.

Employee was driving a Haul Truck and hit a pot hole on dump. This caused the haul truck to go side to side locking employee's seat belt up and jarring the employee's shoulder and neck.

While cleaning up at drill and shovel yard, employee was moving a 40 lb bushing and dropped it. Employee had one finger trapped under bushing when it contacted the concrete. Right middle finger was hit by the bushing causing a cut and swelling.

The employee was attempting to loosen a jam nut using a pipe wrench and a ball peen hammer. During this process, the employee accidentally struck lower left shin with the ball peen hammer causing a lower left shin contusion.

Employee was in Haul Truck and was being loaded by a shovel and got loaded rough.

Employee started a new job that week working on the Lube Truck. Employee noticed that their legs were really tired on Friday. The next day the left knee started hurting but was manageable on 03/06/2018 employee could barely walk with hurt knee.

Employee was mounting Haul Truck and felt a pain in left shoulder.

Haul Truck Driver was doing shift change after sitting idled for 11 hours. The employee dismounted haul truck and foot slipped on a rock and twisted employee's ankle.

Employee was dismounting Haul Truck and slipped on ladder losing footing hitting left check on handrail. Initial injury was 1/24/2018 at 14:30pm. Employee was seen by physician and released to full duty with no restriction. Employee continued to have headaches and neck pain. Employee was taken to an Occupational Therapist for further treatment and placed on restricted duty.

Slipped on ice in Met Ops Parking area and came down on one knee.

Employee was walking around a light vehicle and slipped on ice. Employee tried to catch themselves by reaching out for tailgate. In doing that it pulled their shoulder making them fall to the ground on their upper body.

Employee was using coupler jack with hot gloves on to take couplers apart and pinched finger cutting it.

After doing a pre-op inspection of HTK 262 operator went to pick up chock blocks and sprained their back.

Mill Operator was closing a drain valve and back popped a few times.

While lifting a pothead out of a coffin box, employee felt a pop in the back.

Employee and co-worker were carrying tugger down stairs at crusher. Employee had chains over shoulder and co-worker had tugger. Co-worker dropped tugger, pulling employee to ground

During a lime delivery, a release of material out the lime storage area resulted in a cloud which drifted over towards the south side of the mill. The cloud passed over lab employees who then reported inhalation exposure symptoms. Affected employees were taken for medical evaluation as a precautionary measure.

While backing haul truck under shovel bucket, the bucket swung forward and struck headache rack of haul truck resulting in impact, jamming operator.

While loading material out on the 7000 bench of the RE3 layback, the operator, of LDR003 encountered a hard spot in the floor causing the loader to articulate hard and "jar" employee's back.

Employee was pulling out cable dip stick on excavator and frayed wire impaled left index finger

Elevator door would only open approximately 2 inches, not allowing employee to exit. Employee was entrapped until approximately 5:45pm. MSHA was notified at 5:13pm

Employee strained back while operating rubber tire dozer on 2/26/17. Employee saw doctor on 4/24/19 for back pain that was attributed to 2017 incident and was referred to specialist and given restrictions.

Backing over material/rocks repeatedly and jarring back.

Employee was cleaning floor in wash bay and fell when coming down stairs

Miner was descending a CAT loader between the tires rather than using the access ladder and 3 points of contact. Employee jumped down and caught employee's foot in the tire chains. Employee fell to the ground.

While diagnosing a transformer with 13.8KV and 24.9KV feeding into it, the employee and coworker determined a fuse had blown. They could not locate a replacement transformer and decided to install "jumper" to return power. They had determined that since there was additional fuses to the outside it would not by pass the any safety devices.

Truck Driver fell approximately 3 feet to the ground while mounting an articulated haul truck.

Assisting co-worker lift 90-100 pound viscometer, at end of shift while driving home had shooting pain down right arm.

Employee slipped/tripped on steps and fell down to knee.

During an offload of product into storage tank and ignition had occurred. Mill operator and delivery driver began immediate evacuation and activated Mayday. The storage tank erupted when pressure elevated.

Coming down ladder of haul truck to step on ground an felt "tear" in abdomen at location of previous non work related surgery.

While climbing ladder of haul truck, employee felt a "twinge" in left shoulder.

Employee began climbing ladder into haul truck and ladder moved underneath employee. Foot slipped and right knee was injured.

Employee was air-arcing feather ring out of bore and a spark landed on back between shoulders.

Climbing off water truck when left hand slipped on wet handrail and gripped tighter with right hand

While lifting the lid of the Boyd to crush samples, the lid got stuck and employee was trying to get the lid loose. Employee reported a tweak in lower back.

Employee slipped/stumbled on the deck causing EE to lose balance and put EE's hands out to stabilize EE's fall when EE's right hand contacted the HQ rod in the drill head.

Injured employee was mounting dozer and strained right shoulder. Employee was prescribed modified duty.

While performing pre-op inspection of RTD slipped on packed snow at ready line

Removing wing shroud on Letourneau L1850 and a small metal fragment broke off and struck him in the neck. This was originally classified as 1st aid because the doctor said leave it in it will heal fine and put a band aid on it. We encouraged the employee to seek a second opinion, he did and the metal was removed February 4, 2016, elevating the incident to a lost time injury.

While pulling sample, the sample catcher was bound up and employee pulled back. Incident was reported to supervisor at end of shift, but no further evaluation was requested or mentioned until the employee went to ER on days off. He had reported to work the following day on 12-9-15.

Operator was making a turn around for a load of prill. When he stepped out onto the top step of the bulk truck his foot slipped and he hit his back on the top step of the truck.

Was getting loaded by electric shovel and bucket hit dove tail of truck. Truck lunged forward. Employee received evaluation and followed recommendations for ibuprofen use. As time passed further evaluation was sought due to no change in condition. Further evaluation on 11-23-15 placed employee on restricted duty.

Haul truck driver complained on pain in hand and wrists. Injured employee sought medical treatment and was prescribed no duty. The employee did not report injury as an occupational injury until 11-2-15.

pain surged through right hand during operation of pressure washer. Employee reported pain to supervisor on 10-15. On 10-19 he requested to go to clinic for further evaluation. On 10-23 physician placed employee on modified duty pending further medical evaluation.

Employee was removing belly pan. Some bolts were stuck so he got underneath to try and break them free. The welds on some other bolt brackets broke and the belly pan fell on the employee fracturing his collar bone.

While moving cable spool weighting ~ 75lbs to storage, grip slipped and compensated hurting back.

While dismounting cab of dozer that was high centered with left side higher than right the operator fell to sitting position when dismounting dozer from left side, bruising buttocks.

While driving haul truck hit a bump and back "locked up"

2 employees were working on an attachment to a skid steer. The attachment was removed from the bucket and set on 4X4's. A 3rd employee came to help and after one end of the attachment slipped off of the 4X4 the 3rd employee tried to reposition the 4X4 as the 1st employee slid the attachment back on the boards and this smashed the employee's fingers between the 4X4 and attachment.

During clean-up of electric shovel, a large rock rolled behind front left tire. When backing out tire ran over rock, rocking rubber tire dozer causing operator to strike head on roll over protection structure. Operator did not wish to seek medical attention until 5-29-15 and a follow up appointment on 6-3-15 resulted in restricted duty

Crew was pumping cement and employee bent over to pick up cement bags and the hose transferring cement split causing cement to spray in his face.

As employee entered crew van, foot slipped on van step and fell on knee extending left arm.

While grading ramp to shovel, operator turned to look behind grader for on coming traffic before turning grader on ramp. Sharp pain in neck and shoulder was reported immediately to supervisor.

Employee was dismounting a haul truck via the boarding ladder and suffered an injury to his right elbow.

Two employees were removing bolts with high torque drill and wrench. The wrench slipped off and hit the employee in the face fracturing his orbital bone.

Truck Driver hit a high spot on pit floor causing the seat to bottom out and injuring his back. Employee sought medical attention on 3/11/15. Incident was never reported to the mine by the employee. RNMC learned about the incident when we received the workers comp paperwork via the mail on 3/13/15.

Technician noticed snow and ice and went through area to get shovel to fix unsafe condition and slipped on ice

Employee was opening the passenger door on a haul truck and strained his lower back. He was in the wrong position to open the door. Employee delayed seeking medical attention until 2/24/15 and was prescribed restricted work.

Employee's left forefinger was crushed by a piece of steel plate.

Employee was walking down the stairs on Shovel 1 and stepped wrong, which caused him to twist his left ankle.

Employee said that he was injured by applying the clutch pedal in a motor grader. He did not report the incident until 01/19/2015. Employee did not seek medical assistance until 01/22/2015. The employee was given light duty on 01/26/2015.

Employee was walking around the front of the haul truck cab and ran into the windshield washer door that was open. The door caused a laceration to the employee's leg above the knee that required sutures.

Employee was stepping down off of a step and felt something pop in his left calf muscle.

Employee was tramming loader to new work area and drove over uneven ground and caused loader to bounce injuring his back. Employee didn't seek medical attention until 1/5/15. Employee missed one scheduled shift before being released back to full duty.

It is believed that the hole was initiated when the temperature inside the hole exceeded 225 degrees. The heat may have been created by a chemical reaction caused by pyrites inside of the wet hole that would have created sulfuric acid which started a catalytic reaction. The reaction would have been intensified with the addition of the oxidizing agent contained in the emulsion.

Shovel mechanics was installing a retract rope on electric shovel when rope popped out of sheave (Green horn) and contacted his left foot and ankle causing injury. EE was taken to local hospital for evaluation and released on light duty.

Mechanic and coworker were unloading a utility box from the rear of the service truck and hit back of his knee on trailer hitch causing knee to dislocate. Employee initially refused medical treatment until 11/10/2014 when he was evaluated by the local hospital and placed on restricted duty.

Employee reported sustaining a back injury after driving the 305 water truck. Injury was not reported until 11-3-14. Employee was placed on modified duty on 11-13-14.

Weather conditions caused a stoppage of work with lightning and rain, on the stoppage and nature of the close lightning the employee was attempting to gather equipment up before proceeding down off the structure and struck the back of the right hand into a pipe support bracket.

Employee was changing out a coolant sensor and received 2nd degree burns on hand and arm due to contact with heated coolant.

On 06 June 2014 employee was moving shovel cable when the cable shifted causing employee to lose footing stumbling backwards landing on his elbow straining his right shoulder. Employee was seen by a doctor on 13 June 2014 and given light duty.

Employee was moving equipment into position and after moving equipment felt pain in back.

Employee was operating a loader and was twisted in the seat while backing up. The loader ran over a rock jarring the employee's back. The employee felt medical treatment was not needed. The employee was experiencing pain and was seen by a physician on 04/01/2014 and was placed on restricted duty.

On 20-Feb, employee was walking to her car in the employee parking lot when she lost footing on gravel, causing her to fall to the ground. She tried to catch herself with her extended arm causing injury to her elbow. The employee sought medical treatment on 24-Feb and received restricted duty. The employee did not report the incident or the medical treatment until 26-Feb.

On Feb 11, 2014 employee was climbing haul truck ladder and felt something pull in his right shoulder. EE had a first aid doctors apt on Feb 18, 2014. On follow-up visit on April 18, 2014 was placed on restricted duty.

Employee was exiting a truck when he felt a pop in his back and pain. Employee was seen at ER and a follow-up appointment was scheduled. On 2/04/2014 at the follow-up appointment the employee was diagnosed with lower back strain and given a restricted duty release.

Pinched Finger: Pinched finger between 2 pieces of #10 rebar. Employee was lifting to move a piece of #10 bar when his finger become pinched between the 2 that were present.

Equipment operator was conducting his pre operational inspection on his haul truck when he slipped on a patch of ice pulling his groin.

While walking around his Haul truck, operator stepped on a rock, spraining his left ankle.

While doing a pre-op inspection on a haul truck dust blew into an employee's eyes. The employee was seen by an optometrist and diagnosed with a small scratch on the surface of his right eye. He received a prescription for an eye drop antibiotic as a precaution.

Electrician was placing a part on a pallet when his had contacted another part causing a laceration requiring sutures.

While driving haul truck employee hit a pot hole and jarred his neck.

A tails pump house employee was hosing down the floor when an old brace (pipe separator) fell from above striking the employee on the top of his hard hat. Employee received a concussion.

Drill operator used foot to steady a chain wrench while operating the controls. The wrench slipped pinching his foot against stop and wrench.

Employee was descending a stair ladder when he felt a pop in his chest area. Diagnosed as a rib strain with possible rib fracture. Fracture was suspected but could not be positively diagnosed. X-ray was inconclusive.

Employee slipped on small pebbles in the entrance to the reclaim tunnel and twisted knee. Employee saw a doctor on 10/4/13 and was release with restricted duty.

While removing choker cable a broken strand of cable went through glove on the right hand cutting right thumb. Employee received 4 sutures.

Employee was handling a chain clevis when his left index finger was caught in the clevis tearing skin from fingertip.

While preparing samples for analysis in lab employee got some debris in right eye. Employee was seen by doctor nothing found in eye but was given prescription meds for precaution.

Employee had finished training on Mustang skid steer, pulling it over next to the trailer to park it. After shutting the machine down and exiting the cab the employee felt a stinging in his left hand, the employee had cut it on door latch as he exited the cab of the mustang. The Supervisor drove employee to emergency room where he was treated. The employee received eight stitches.

While setting up to torque bit, employee's hand was caught between drill table and wrench.

Operator was attempting to climb back up and into his Komatsu 830, as operator reached for the handrail felt a pop in left shoulder area.

While backing up a D10T dozer, ran over large rock and jarred/strained neck area. Employee was unaware that a rock fell out of berm while pushing material.

Employee was walking up a approx. 10 % grade haul road, felt a pop in left knee which gave out and employee fell to the ground. Haul road was dry and no large rocks in the walking area.

While operator was cleaning and scraping the pit floor with Leterno 1850, hit a pot hole, which in turn bounced him in his seat jarring his back.

Employee was using a grinder to cut off a bolt head. The grinder wheel caught and cut employee's left index finger.

While grinding, some metal shavings collected on the employee's clothing and hard hat. While moving around a piece of metal shaving fell into employee's left eye.

While walking back to desk in the warehouse, employee tripped on some plastic banding laying on the floor. Employee states as she fell her body twisted and she landed on left side of back.

Employee was attempting to drive down a sprocket into place using a hammer and a square piece of metal tubing. Employee struck left thumb with the hammer against the square tubing resulting in a laceration.

Employee was conducting maintenance work with three others on a shovel. They were removing the upper rails on the shovel using an impact gun and wood skids. Later in the shift, the employee felt pain in his lower back. The employee did not report the pain until 8/2/12. He went to the hospital on his own and they found a disc diffusion in L4-L5.

Employee was digesting a sample using nitric acid, as employee was removing beaker it became sticky to glove which resulted in losing control of the beaker and dropping. The solution splashed up and resulted in 2nd degree burns to portions of face, neck, and arms.

While performing a pre-shift inspection on water truck, employee walked behind looking at the tail lights. Employee started to back up so that she could see better and stepped into a small hole losing her balance and falling to the ground.

Employee was attempting to unplug lime chute, while opening the inspection door, lime came down and coated his clothing and portions of his skin. Employee delayed in getting the lime off his clothing and skin. this resulted in 2nd degree burns to both his right foot and left hand.

While inside sag mill shoveling off rocks on the trommel screens, employee felt a lingering pain in lower back. Employee was bent over shoveling at the time of incident.

While opening a 16" wheel valve, employee had right hand on inner spoke to close valve. Right thumb came in contact with gate support resulting in a small laceration. Employee was wearing gloves at the time of the incident.

While lifting 57" lock ring off of lock stand, slipped out of hands and lost control. When lock ring hit the ground, caused a split in the ring to open and close on employee's hand, Resulting in cuts to both sides of left index finger.

Closing rotation barrier and finger got pinched in door, fracture/laceration to right ring finger, received 7 stitches.
