Employee was lifting battery lid when he felt a sharp pain in his groin area and back.

Assisting another employee move miner cable and waterline. As employee lifted the cable he felt pain in his mid to upper back.

Employee was placing crib blocks under a diesel engine that was being held by a forklift. When he bent over to put the crib block in place he struck his head on one of the forks resulting in a laceration requiring staples in his head.

Employee stated he was replacing the main jack on ATRS roof bolter FB#21. The ATRS was secured to mine roof by chains, the main jack was secured to drill by bottom pin. Employee started machine at operator's station which position on end of ATRS bar, the ATRS bar fell striking him on front of hard hat and right shoulder.

Bates Contractor was recovering water line and loading it on water line sled. While loading the water line he smashed his middle finger on his right hand between sled and water line.

On August 17, 2013, EE picked up a belt roller, twisted around and felt a pop in his back.

The EE was building a brattice on the MMU. No 002 Section. The EE bent over to pick up a single concrete block and felt a burning pain in his lower left side.

Employee was assisting in the changing of the cutter drums on the continuous miner. He was standing about 8' and slightly in the front of the drums as they were using a port-a-power to push the drums off. The metal broke and it flew off striking the employee on the left leg. He was treated by M.E.T. on the section and transported to Hazard A.R.H. hospital.

Employee was lifting a 6" hollow core concrete block, constructing a ventilation brattice, when he felt pain in his back and grion area.

Employee was operating Roof Bolter as he was pulling a metal strap off the bolting machine, he cut his arm just above the left wrist requiring (13) stitches.

EE's were lifting belt stand out of the man trip. EE felt pain in his lower back. EE went to the doctor and went back to work but he has had constant pain and just started to miss work on 11/21/10.

Employee was lifting dryer basket to remove socket from underneath when he felt pain in his back.

Employee had stopped miner to pull up miner cable for next cut. After pulling and lifting cable he felt pain in his lower back while walking across section. Resulting in a strain to the lower back.( Employee worked until 8/4/2010 and then became LT)

Employee was attempting to loosen a brake line fitting on a surface drill when the fitting suddenly broke loose causing him to strike his right elbow on the roller frame of the drill.

Employee was removing metal that had been cut from the floor when it slipped and caught his thumb between the handrail and piece of metal. He received a laceration that required stitches.

Employee cut his right forearm on a band that had been cut while reaching for a roof strap in the supply hole. Stitches were required.

Loading cable bolts onto scoop, when one bounced up and hit employee on forehead. Resulted in a cut that required 3 stitches.

Employee was throwing a bundle of glue on the roof bolter, when he felt a sharp pain in his groin. He was diagnosed with a hernia.

Employee was replacing metal siding. Metal fell and cut index and middle finger on left hand.

Employee was putting roof bolt plate on roof bolt to install, and plate cut wrist requiring 2 sutures.

Employee was pulling a piece of drill steel out that had become lodged in the fender of the shuttle car. In the process, he dislocated his right index finger. Employee continued to work not missing any work days after initial accident. Surgery was performed on 10/21/08, and returned to work next scheduled shift, performing his normal duties.

Employee was attaching a sweeper to a IT 14 G loader, and when he pulled on the quick disconnect hose, he experienced pain in his lower back.

Employee was repairing miner when the panel board cover slipped and caused a laceration to the little finger, left hand.

Employee was working on a rock truck, when he raised up, he experienced pain in his lower back.

Employee's fingers were pinched, in a spring loaded concrete chute, while cleaning.

Employee was loading some belt rail back onto a scoop that had fallen off the trailer, when he caught his left ring finger between the scoop and the rail.

Employee was installing metal slats in roof when a metal slat fell and caused a laceration to the little finger left hand.

Employee was plastering brattice when mortar entered left eye.

Employee was hanging miner cable when he experienced pain in the groin area.

Employee was installing gate on mine road and while lifting on gate became unbalanced. Employee originally starting missing on 4/21/2007 for personal illness. Employee notifed employer on 6/25/2007 he was going to seek medical treatment for back strain which occured on 4/20/2007.

Employee was lifting a timber and felt pain in left side of groin.

Employee was moving miner cable out of roadway and felt sharp pain in lower back.

Employee was lifting a crib block, he turned to throw in the bucket and had pain in his back.

Employee was lifting a case of water, foot got stuck in mud. While attempting to pick foot up, body weight shifted causing employee to pull muscle in left shoulder area. Employee was released to work by ER doctor. Followed up with family doctor and started to miss work.

Employee was greasing head drive and tail piece, he struck old belt wipe resulting in a laceration to the lower left leg.

EE WAS PULLING A SET OF BATTERY LEADS FROM ONE BATTERY TO THE OTHER.

Employee was removing vegitation from area around a waterline at the plant entrance to splice waterline when a weed struck him in the left eye.

Employee was cut on his right wrist by the end of a band of a bundle of roof bolts while picking up a bundle of glue lying behind the bundle of bolts. The laceration required 3 stitches.

EMPLOYEE WAS LOADING A BOX OF ROOF BOLT RESIN ONTO A SCOOP. AS HE LIFTED & THREW IT ONTO THE SCOOP HE FELT A SHARP PAIN IN HIS LOWER BACK.

Employee was pulling bolt machine cable. As he leaned forward he felt pain in left knee. Had fluid removed from knee on 05-03-05 - became reportable injury.(HAD KNEE SURGERY ON 5-16-05 BECAME LOST TIME ON 5-17-05 BY MISC MAIL 5/53/05 #10). RTW per MISC MAIL 7/5/05 #6.

EE STATED THAT EE WAS IN THE PROCESS OF MANUALLY OPENING A SET OR AIR LOCK DOORS, ONE OF THE TWO DOORS STRUCK EE CAUSING A LACERATION TO LEFT EAR

EE stated he was in the process of examining the primary sampler for the clean coal loadout, while moving the metal that was next to the sampler cutter guard, the metal fell pinning him against the floor, causing fractures to both legs and his left hip.

Employee stated that he was removing pizza pans from roof drill, when they fell striking him on his left wrist, causing a laceration.

Employee stated that he was replacing a lost boom jack on the shuttle car, boom jack suddenly sprang loose without warning striking EE in the chin causing a laceration.

EE STATED HE WAS IN THE PROCESS OF LOADING A OXYGEN TANK ONTO MANTRIP, WHEN HE STRUCK HIS RIGHT HAND MIDDLE FINGER BETWEEN THE 02 TANK AND FRAME OF MANTRIP CAUSING A LACERATION.

EE STATED HE HAD BEEN PULLING A MINER CABLE AND SETTING TIMBER ABOUT 30 MIN LATER HE COUPLINED A HAVING PAIN IN HIS GROIN ON RIGHT SIDE AND LOWER BACK MEDICAL DR REVIOUS HIM FROM WORK ON 10-21 -03.

EE STATED HE WAS IN THE PROCESS OF REMOVING HYDRAULIC HOSE FROM PARTS HOSE, HOSE FELL STRIKING EE IN THE FACE CAUSING A LACERATION TO HIS FACE.
