Mine #8 Coal

The EE had completed trimming excess skirt rubber from the #6 tailpiece located at survey station #10483. When the EE stepped down off the belt conveyor his right knee twisted and possibly came out of joint. The EE seen a phys. on 12-2-2011 and again on 12-05-2011. At this time the EE was taken off work for P.T.

A Roof Fall was discovered at approximately 4:00am on the # 5 belt conveyor. The roof fall is located 1 crosscut inby survey station 3781. The roof fall is approximately 60' feet long x 20' feet wide x 6'-7' feet high. The roof fall was reported to the MSHA Hot-line at 4:10am.

Roof Fall was discovered on 4 North Mains at 3:45 A.M. by Dwayne Maggard Size of fall 30'L X 30'W X 8'H. Fall does not effect ventilation or impede travel, roof fall will not be cleaned up

Employee was lifting a 6" hollow core concrete block, constructing a ventilation brattice, when he felt pain in his back and grion area.

Fall occurred on 12/14/10 found at approximately 6:45 am by Anthony Miller. Fall occurred on 1 West Mains in the #3 entry 2 cross cuts inby spad station 5605 and extended to spad # 3767. Fall was 100 ft long, 6 ft high, 20 ft wide. Fall did not alter the ventilation or affect the travel way. This fall was timbered off and danger signs were posted to restrict access to this area.

Employee stepped down out of supply building and tripped, fell landing on portable mine phone causing cut on left hand requiring (5) stitches to palm of hand.

Employee Slipped on mine floor and tried to catch himself with right hand. Employee suffered simple fracture to wrist, as a result of the fall.

Employee was assisting to put the section belt back together after it had broken. During the process a rope being used to pull the belt back together broke and struck employee in the face. Employee suffered a laceration to right cheek and jaw.

Employee had stopped miner to pull up miner cable for next cut. After pulling and lifting cable he felt pain in his lower back while walking across section. Resulting in a strain to the lower back.( Employee worked until 8/4/2010 and then became LT)

Roof fall was discovered in #9 entry of 1 North Mains. The fall measured 30' x 19' x 5' thick. Fall was dangered off and will not be cleaned up.

Employee stated that he twisted his left knee while he got up and turned around in the shuttle car he was operating.

Employee was helping put steel beams up using Hieuzman Jacks when the jack slipped out causing the steel beam to strike him on the back.

Roof fall was discovered on 1 Northwest Mains, 140 feet outby SS#3184, in #2 Entry. It measured 18' X 30' X 7'. Ventilation or travel was not affected.

Employee was struck by the roof bolter cable when it got jerked down due to the reel being on while the bolter was backing out of the heading.

While tramming a shuttle car, canopy got into the rib on the corner sending a small piece of coal into employee's left eye. Coal had to be removed by doctor.

Non-intentional, non-injury roof fall was discovered in the Northwest Mains, 90' inby SS# 3040 in the #4 entry. SIze of the fall was 16' wide x 12' long x 6' thick. Fall will be cleaned up and the top will be resupported.

Employee slipped on a flat rock covered in mud and fell. Injured right hip and back when he fell. Started to be Lost Time on 2/27/09 per Doctor's orders.

Employee was throwing a bundle of glue on the roof bolter, when he felt a sharp pain in his groin. He was diagnosed with a hernia.

Employee was putting roof bolt plate on roof bolt to install, and plate cut wrist requiring 2 sutures.

Employee was pulling a piece of drill steel out that had become lodged in the fender of the shuttle car. In the process, he dislocated his right index finger. Employee continued to work not missing any work days after initial accident. Surgery was performed on 10/21/08, and returned to work next scheduled shift, performing his normal duties.

Glove caught burr in drill steel, cutting hand while removing steel out of hole.

Cutting plastic bands, when knife slipped and cut his rt. wrist.

Employee was walking to a man trip with his head turned side ways, when his left ear struck a roof bolt plate.

Employee was traveling in a buggy when a shovel handle in the back of the buggy, struck the rib and caused the end of shovel to strike the employee in the right ear. This caused an ear ring to be torn from his right ear.

Employee was cutting beltline with a utility knife when the knife slipped and cut his right leg above the knee.

Employee was loading some belt rail back onto a scoop that had fallen off the trailer, when he caught his left ring finger between the scoop and the rail.

Employee was putting up a metal strap, when he turned around and impacted the metal strap with the lower right part of his face, resulting in a laceration.

Employee was traveling down slope in scoop when brakes failed and scoop struck supply car.

Employee was driving a buggy when the steering wheel spun around and the knob on the steering wheel struck the top of his right hand, resulting in a fracture.

Employee was plastering brattice when mortar entered left eye.

Employee was resetting a 480 volt breaker, when he recieved an alleged electrical shock.

Employee was hanging miner cable when he experienced pain in the groin area.

Employee was using a utility knife to peel insulation on cable , knife slipped and cut the palm of his left hand.

Employee was operating a shuttle car with his left hand on top of the canopy when his hand was caught betweeen the mine roof and the canopy.

A rock fall was discovered during the normal weekly exams. The fall was in the number 1 entry of the south west mains approximately 200 feet inby S.S. number 416. The fall was approximately 20 feet wide, fourty feet long and six feet tall.

A roof fall was discovered by the mine examiner during his regular routine. The roof fall was in the #2 entry of the 1 NW Mains, approx. 60 feet inby spad no. 3020. The roof fall measured 20' wide by 20' long by 4' thick. It does not effect the mine ventilation. The area was barricaded to prevent persons from traveling in the area.

Lowering shuttle car canopy when canopy slipped it fell hitting rt hand (middle finger) between canopy and frame of car.

Roof fall was at Hales Branch Mains, outby spad # 1699 in #5 entry. It was 20' long X 20' wide and 5' thick. No equipment was involved and ventilation was not effected.

Roof fall was found by Preshifter at 2:55pm. It was in #5 entry on 1 north mains, spad # 3772. The fall was 20' wide X 5' long X 4.5' thick. It has been dangered off and will be cleaned up.

Employee put his hand around the base of the drill steel caught his glove between the steel and the chuck. When he started his rotation it twisted his finger resulting in a dislocation of his left ring finger.

Employee was lifting a timber and felt pain in left side of groin.

Employee was moving miner cable out of roadway and felt sharp pain in lower back.

Employee was lifting a crib block, he turned to throw in the bucket and had pain in his back.

Employee was lifting a case of water, foot got stuck in mud. While attempting to pick foot up, body weight shifted causing employee to pull muscle in left shoulder area. Employee was released to work by ER doctor. Followed up with family doctor and started to miss work.

Employee was greasing head drive and tail piece, he struck old belt wipe resulting in a laceration to the lower left leg.

EE WAS PULLING A SET OF BATTERY LEADS FROM ONE BATTERY TO THE OTHER.

Employee was moving miner cable out of the way, strained lower back.

Employee was helping roof bolt machine operator move to another location and strained lower back (slip & fall). (Employee starting missing 03/13/06.)

Employee was operating roof bolting machine and felt pain in lower back.

Employee was working on a roof bolter, screwdriver fell off boom and struck right eye.

Employee recieved small cut right hand while opening an oil can.

Employee was cutting belt, the knife slipped resulting in a cut on the right leg just above the knee.

Employee was geting out of repairman's buggy, slipped and fell in mud.

Employee was bolting top when a rock approximately 200 pounds fell striking his left hand, causing injury to three fingers on the left hand

Employee was tramming a ram car, lost brakes, hit panic bar causing head and neck pain

Employee was bolting top and twisted right knee.

Employe was drilling hole for roof bolt, when a piece of rock fell striking him in the ankle.

A rock fall occured at the number 6 entry 1 cut inby the number 8 head drive. The fall measured approximately 6 to 9 feet in height, approximately 50 foot long, and 20 foot wide.

Employee was drilling a hole for a tail piece, slipped and fell back on his left hand resulting in a sprain.

Employee was loading glue on roof bolter, felt pain in lower back

Employee had a heart attack (Acute Myocardial Infraction) according to the corner and autopsy results.

Employee was removing drill steel from pot on roof bolter and got struck by a rock on his right thumb causing a fracture in the first joint.

This mine experienced a rock fall that measured 18'W X 65'L X 15'H one break inby spad number 3103.

A rock fall was found in the right return entry at spad number 1808. The fall measured 20'W X 30'L X 8'H.

A roof fall ocurred in the #8 entry 3 breaks inby spad #1872. The fall measured 20'W X 20'L X 8'H.

Employee was getting cable out from under a sled in the bucket of a scoop and the scoop driver dropped the bucket, catching the employee's top of his left foot between the sled and the bottom.

Employee was pulling rock off of miner and got his ring finger on right hand caught between rock and roof of mine. (Required 7 stitches)

Employee was lifting roof bolts off back of pinner and twisted his lower back.

During pre-shift the pre-shift examiner heard a rock-fall. The fall was 2 breaks inby spad #2180 in the #5 entry. The fall measured 60'L X 20'W X app. 6'H.

A roof fall occurred in the #4 entry 1 break outby spad no. 1874. The fall measured app. 20'L X 20'W X 7'H.

A roof fall occurred in the #8 entry 1 break outby spad no. 1858. The fall measured app. 60'L X 20'W X 7 1/2'H.

A rock fall occurred in the #8 entry 1 break outby spad # 1844. The fall measured 50'L X 20'W X 5'H.

A roof fall occurred in the #9 entry outby spad # 1898. The fall measured 50'L X 20'W X 4.5'H.

A rock fall occured between the #8 and #9 entry 8 breaks inby #4 belt. The fall measured 9'L X 19'W X 5'H.

A roof fall occured in the #2 entry at spad #1892. The fall measured 60'L X 19'W X 6'H.

A rock occured on 001 section. The fall is from 6 right to 8 right break the fall measured 120'L X 20'W X 6'H.

A rock fall occurred in the #9 entry approx. 17 breaks inby the #4 head-drive. The fall measured 100'L X 20'W X 6'H.

A rock fall occurred at the face of 001 section. The fall measured 94'L X 18'W X 6'H.

Operator was installing a roof bolt and the bolt wrench came loose and struck operator on left wrist and arm. Broke small bone in left wrist. Started missing work on 3-2-04.

OPERATOR OF ROOF DRILL WAS DRILLING A HOLE AND GLOVES HE WAS WEARING GOT CAUGHT IN DRILL HEAD ROTATING DRILL POT.

EE WAS CUTTING THE SUCTION HOSE ON THE ROOF DRILL AND THE KNIFE SLIPPED CUTTING HIS FOREFINGER OF HIS LEFT HAND, REQUIRING SUTURES. EE RETURNED TO WORK AT NORMAL DUTY ON HIS NEXT SHIFT.

A ROOF FALL IN THE PILLAR LINE IN THE #1 PILLAR BLOCK. ABOUT 5' TO 6' AT SPAD #1066. THE ROOF IS SHALE THE BOLTS INSTALLED WERE 4' RESIN. THE FALL CAUGHT A REMOTE CONTROLL JOY MINER THAT HAS B EEN REMOVED FROM THE FALL.

RAN OVER ROCK IN #5 ENTRY AND HURT HIS BACK AND NECK.

INJURED WAS IN STOOPED POSITION ALIGNING SPADS WHEN THE SHUTTLE CAR APPROACHED WHERE HE WAS WORKING WHEN INJURED REALIZED OPERATOR WAS NOT GOING TO STOP, HE GOT OUT OF THE PATH OF THE SHUTTLE CAR. HOWEVER, HE WAS CAUGHT BY THE SHUTTLE CAR CABLE AND THROWN AGAINST THE MINE ROOF AND CAUGHT AGAINST THE RIB.

WAS HANGING MINER CABLE AND FELT SOMETHING PULL IN HIS GROIN AREA.

EE WAS LIFTING STRUCTURE OFF PALLET TO LOAD ONTO RAM CAR WHEN HE HURT HIS LEFT SHOULDER.

EMPLOYEE WAS INSTALLING ROOF BOLTS IN #8 RIGHT XCUT. DRAW SLATE FALL ALONG THE RIB STRUCK HIM.

EMPLOYEE WAS BUILDING A BRATTICE APPROX 10' HIGH WHEN BRATTICE FELL CATCHING HIS RIGHT FOOT.
