While loading scoop lid onto scoop employee twisted left knee.

Reporting as a part of Part 50 Audit. Employee was removing the bolts to replace rear end, a piece of debris fell and hit his left eye. Employee received medical treatment, foreign body was removed and employee received prescription medication for the left eye.

The continuous miner was stuck, was placing 4' x 8" crib block under the tracks of the continuous miner for traction, EE picked up a crib block and pulled his back.

Employee bent over to pick up a bolt and a pin fell and hit employee on the back of the head.

Putting safety glasses on, something went in eye. Either came off glasses or could have come off cap.

Loading parts up at the office to bring to the mine, employee hurt his lower back.

Removing the boom from a roof bolting machine, the boom came off the machine unexpectedly and pulled the employee's left arm & shoulder.

ee was injured when he lifted a rock out of crusher on 3/4/06.It was reported to company 5/3/06 when ee went to have a MRI on his back.

Pulled muscle in lower back while repairing Mack truck drive line.

PULLING CONVEYOR BELT WHILE SPLICING EE EXPERIENCED PAIN IN LOWER STOMACH AREA. DIAGNOSED WITH HERNIA . SURGERY PERFORMED 2/7/05
