Victim was installing roof bolt, he reached to drop head but activated boom swing. causing boom to strike his head pushing him back into coal rib.

The face boss on the section was crawling between lo lo structure and coal rib.The front of the MBC hit tailpiece and shoved the lo lo belt structure mashing the victim's legs against the coal rib.
Loading parts up at the office to bring to the mine, employee hurt his lower back.
Removing the boom from a roof bolting machine, the boom came off the machine unexpectedly and pulled the employee's left arm & shoulder.
A piece of rock 6' X 3' X 4" fell from the rib to the bolt, striking employee in the head & shoulders forcing him to the floor. The rock also hit the roof bolting machine on the way down.
Accelerator pedal didn't return to neutral position causing operator to hit rib.
