Mostrando los 30EE was putting a piece of steel in the steel dumpster and it caught onto EE's glove, causing EE's right ring finger to get smashed between 2 pieces of steel.
Employee flipped a 300lb wheel loader cutting edge and lost control, causing their fingers to get crushed.
While EE was replacing a front end loader bucket cutting edge, EE was trying to stop a bolt head from spinning using a gloved finger. Sharp bolt head cut through the glove and into the pad of EE's left index finger. EE was treated for first aid. On 5/7, EE sought medical treatment and was prescribed an antibiotic.
Employee removing empty water jugs from flatbed service truck. EE lost their footing fell to ground from bumper of truck 18" from ground landing on left side, left shoulder. EE lost their breath for a moment. Employee seen by medical and confirmed fracture to left side rib. Employee was returned to work.
A plant operator was performing clean up duties around the sand heater. The operator noticed grease around the trunnion roller and climbed up on a cement block at the base of the equipment to get a better look at the machine in operation. In the process of climbing down from the cement block the employee's glove was caught by the trunnion roller and the left hand was pulled in.
Removing steel throat bushing from Slurry pump housing. Hung up with 1/4" to go. Used left hand to push the top to free it. Came lose tipped back on the chain hoist supporting it and pinned left elbow on pump housing, simultaneously bending left hand back so fingers touched the top forearm. Diagnosis: Closed acute non-displaced intra-articular radial styloid fracture.
EMPLOYEE WAS MANUALLY MOVING ROCK THE SIZE OF A BASKETBALL FROM ATOP A CRUSHER WITH GLOVED HANDS. WHILE PICKING UP THE ROCK TO THROW TOWARDS CRUSHER HOPPER, ROCK SHIFTED AND PINCHED LEFT INDEX FINGER BETWEEN ROCK AND HINGE. RESULTS WAS BROKEN LEFT FINGER TIP AND 5 STITCHES ON FINGER.
Unloading pipe from truck. Strap slipped and fell off pipe after bumping another piece of pipe. This caused pipe to fall and catch employee in the shoulder.
EE is unsure of exactly how ee got a foreign object in eye. It is believed while ee was drilling a Bolt hole in a hopper that some material landed on glove and when ee went to adjust hat or glasses some material fell in EE eye. It continued to burn off and on until noon (4-20) when ee went in to Clinic.
Removing bungee cord from door when it slipped from hand cutting right palm
Employee was pulling a cord while working in sand. Employee caught their toe, twisted their right leg. which resulted in pressure being applied to the inner side of the right knee.
While installing skirting on the tail section of a radial stacker, the employee felt a strain to his back. Medical attention was sought by the EE on 11/9/2014. On 11/10/2014, the EE informed mine management of medical attention provided over the weekend on 11/9/2014. Injury accepted 11/10/2014.
EE & co-worker were lifting the bearing assembly into place. Not able to fit it, they were bringing back into the manlift when the flange caught on the top railing. Assembly came to rest on EE's right forearm pinching it between the top rail and assembly.
While opening the tailgate to a F250 pickup truck, the employee felt a "pop" in his right arm - biceps tendon.
Pulling heavy electrical cords through mud, and worked long hours. Felt back pain.
EE was putting V-Belt onto sheaves as belt went onto pulley the belt pinched his Right thumb between the two.

EE's were cleaning the jaw crusher out. Material became lodged in grizzly. They were pitching rocks out when co-worker threw one striking EE above his left eye causing a laceration requiring stitches.
Employee may have struck left elbow while shoveling or changing screens causing swelling.
Employee stated he strained his upper shoulder/back area climbing into screen plant to change screen Did not seek medical attention until 8/5/11. Claim was under investigation while employee continued to work.

While repairing springs on hopper gate, it wouldn't shut, so he lifted it and it slammed shut on his right hand index finger.
Employees were lifting a portable metal stairway into a semi-trailer for transport. As he slid it into the trailer, a piece of metal caught his right index fingernail and cut it open. Needed numerous stitches to close the wound.
Employee was closing a garage door and caught his finger in a pinch point. The tip of the left middle finger was cut off.
Employee was cleaning the battery terminals on the gen set. As he was using the brush to scrub the post, some of the acid flew up and struck him in the cheek then behind his safety glasses and into his left eye.
EE was bent over cleaning under a conveyor. when he twisted he felt a sharp pain in his lower back. diagnosis is a lumbar sprain.
While putting on additional wraps to the timber head, a sudden release of tension on the rope occurred. This caused injury to both hands and left arm.
