The employee was dragging an oxygen tank to the continuous miner, to assist in making repairs to this machine. When the employee attempted to stand the oxygen tank up, he stated he felt pain and burning in his lower back.

EMPLOYEE WAS LOADING A BROKEN HEAD-JACK PIN, FROM THE CONTINUOUS MINER, ONTO THE SCOOP, AND MASHED HIS RIGHT, MIDDLE FINGER BETWEEN THE PIN AND THE SCOOP, CAUSING A LACERATION.

Employee was standing in the left crosscut of the #4 entry, on the 001 section. The continuous miner set in to shear the inby left corner of the crosscut, and a rock flew from the ripper head, striking the employee on the right knee.

The injured person had raised a lid on the continuous miner and did not hinge it all the way to the rear. The lid closed on his right index finger.

Employee was cutting plates/ties loose from track, with a torch, to remove it from the mines. Once the track was cut loose it rolled over on the employee's feet. Bruising his left foot.

Replacing final drive on JD 200 CLC excavator using crane truck to lift tracks. EE had put his hand on sprocket while installing it the chain slipped and caught his hand between the sprocket and track.

Employee was putting the Rub-Rail back on the miner, when it slipped and fell, striking him on the top of his left foot.

As EE was finished repairing a water truck, he was adjusting the side mirror to back the truck out of the garage area and the mirror broke and a piece of glass cut his left wrist. He was treated at the job site, finished his shift and then received medical attention after his shift. He received 4 stitches and was released to return to work.

EE stated that he injured his back as he was climbing down the ladder of the 777 as he was exiting the truck at he end of his shift. He stated that he was OK and refused medical treatment. He worked his shift the next day and then did not return to work. He consulted a Dr on April 17th and has not returned at this time.

EE was helping lift a metal beam when he lost his grip, causing it to land on his hand. Bruising and swelling occurred, with no broken bones.

EE was loading truck, had stabbing pain in back. No one witnessed the accident.

While pulling a piece of curtain, employee slipped and twisted his back, causing pain in his lower back.

As employee was unloading rock dust from the rail car, the pile of rock dust tipped over and caught his leg between the rock dust and the rail car.

Ee was loading bolts, plates glue and straps onto the back of a roof bolter when his left hand struck the sharp edge of a strap, causing a laceration that required 4 stitches.

Employee had lifted a five gallon can of oil up onto his 777 truck when he felt a sharp pain in lower back.

Employee was loading a speed reducer in a scoop bucket when he bent down to push the reducer in the bucket and twisted his knee.

While removing a spill board during a belt back, EE caught his finger between the spill board and the tailpiece.

EE stated that he was changing conveyor belt structure out when he felt pain in his lower back. Ee went to the E.R.Dr where it was determined that he had suffered a lower back strain.

EE was taking the rubrail off the continuous miner when it fell on his right middle finger. He consulted the ER Dr at the hospital who determined that his finger was broken. He did not lose any time with this injury.

EE stated that he was walking and pulling miner cable when he struck him right ear on a strap. He went to the harh hospital where he received 2 stitches to the head and ear.

EE stated that he was pulling a service hose from the reel of the service truck when he felt pain in his lower back. EE went home and consulted a Dr. who odered x-rays which were nega- tive. The Dr ordered an MRI and EE is currently losing time.

Employee was working on a hose clamp when it broke and cut his hand. He was transported to the hospital and received three stitches to his hand. He returned to work and has not lost any time.

EE stated that he was rolling a piece of rock off the water line when he felt pain in his right shoulder and neck.

EE stated that he was putting oil in the shuttle car when the hadle broke on the can of hydraulic oil. The can struck his left hand and cut it requiring 5 stitches.
