The employee was in the conveyor of the shuttle car, cleaning some loose coal from the side of the operators compartment, and when he stepped out onto the bottom, he twisted his left ankle. (Fracture)
The employee was dragging an oxygen tank to the continuous miner, to assist in making repairs to this machine. When the employee attempted to stand the oxygen tank up, he stated he felt pain and burning in his lower back.

EMPLOYEE WAS LOADING A BROKEN HEAD-JACK PIN, FROM THE CONTINUOUS MINER, ONTO THE SCOOP, AND MASHED HIS RIGHT, MIDDLE FINGER BETWEEN THE PIN AND THE SCOOP, CAUSING A LACERATION.
The employee was located at the off-side rear of the Fletcher roof bolting machine, and a roof bolt and plate had fallen off the machine. The employee bent down to pick it up and the operator began tramming the machine at this time and backed the machine up onto the employee's right foot.
The employee was removing a brattice, in a worked out area of the mine, to create a common travel way. While loading the excess brattice block into the scoop bucket, the employee stumbled and fell, striking his left little finger on a rock lying on the bottom, causing a laceration.
Employee was standing in the left crosscut of the #4 entry, on the 001 section. The continuous miner set in to shear the inby left corner of the crosscut, and a rock flew from the ripper head, striking the employee on the right knee.
The injured person had raised a lid on the continuous miner and did not hinge it all the way to the rear. The lid closed on his right index finger.
The accident victim was shoveling loose coal along the belt line. When attempting to unload the shovel, it came in contact with the belt line causing the shovel to wedge against the mine roof, straining the victims shoulder.
Employee was cutting plates/ties loose from track, with a torch, to remove it from the mines. Once the track was cut loose it rolled over on the employee's feet. Bruising his left foot.
A scoop was cleaning a crosscut to permit travel down the beltline, the scoop bucket hooked into a piece of 3" water line, causing the water line to swing around and strike the employee on the lower right leg.
#4 belt slipped in the head drive, causing minor smoke, before the drive shut off. EE stated that the smell caused him to be sick to his stomach. EE worked the remainder of his shift and went to the doctor the following morning.
Employee was drilling a hole in the mine roof to install a roof bolt, after getting his hole started, he looked down and a piece of draw rock fell out, striking him on the head and neck.
Employee was lifting a belt roller with a slate bar while another employee was trying to adjust the height of the roller. The slate bar broke striking the employee in the groin area.
Replacing final drive on JD 200 CLC excavator using crane truck to lift tracks. EE had put his hand on sprocket while installing it the chain slipped and caught his hand between the sprocket and track.
A roof fall occurred in the last open cross-cut, in the #5 entry, on the 003 section. The fall was approximately 25 feet long, 25 feet wide, and 6 feet thick. No persons or equipment involved.
EE was attempting to knock a broke head jack pin out of miner by using a bar and a 16lb hammer. The hammer kicked the side of the bar and his hand struck the lid on the miner, resulting in a cut to his right pinky finger.
EE was attempting to get on the 992D loader. He had one foot one the loader step and one foot on the ground, when his foot that was on the ground slipped, causing him to fall and land on his back.
EE had been working on the 16G Cat Grader, when he was finished and was climbing down the ladder he slipped off of the steps and landed on his left shoulder.
Employee was putting the Rub-Rail back on the miner, when it slipped and fell, striking him on the top of his left foot.
EE was cutting a piece of beltline at a splice. He was holding the belt with his right hand and cutting with his left hand. He cut through the belt and cut his right index finger.
EE said he was taking bolt machine cable down, that was hung from top when the bolt machine operator engaged the cable reel causing the cable to jerk, he jumped out of the way, twisting his knee.
EE was helping another ee repair the suction on the drill head of a roof bolter where the repair were completed ee inadvertently lowered the drill head on ee leg causing swelling or bruising to the upper right leg & ankle.
EE was operating a roof bolter when hi stated a small pipe of draw rock fell, hitting him on his left hand, (finger).
EE was rock dusting on the 001 section when he stated, that debris off a roof bolt strap got into his eye. A thin piece of metal was removed from his eye and small pieces of coal were also removed from his right eye. 2 days light duty.
As EE was finished repairing a water truck, he was adjusting the side mirror to back the truck out of the garage area and the mirror broke and a piece of glass cut his left wrist. He was treated at the job site, finished his shift and then received medical attention after his shift. He received 4 stitches and was released to return to work.
Employee was working a head drive when a water line busted. He turned away from the water line and began to run away when he ran into a guide roller that was positioned beside of the belt line.
EE stated that he injured his back as he was climbing down the ladder of the 777 as he was exiting the truck at he end of his shift. He stated that he was OK and refused medical treatment. He worked his shift the next day and then did not return to work. He consulted a Dr on April 17th and has not returned at this time.
While assisting the mechanic make a splice on the miner cable, EE cut his left wrist with a belt knife, requiring 4 stitches.
EE was operating a Joy SC10 Shuttle Car when this accident happened. EE was getting off of his shuttle car when he stepped on the ground (bottom) the bottom was uneven, causing him to twist his knee.
Driver was driving tandem coal truck from underground mine to prep plant on the normal haul road, when he got too close to the ditch in a curve and started spinning in the mud on the side of the road and slid into the ditch, causing the truck to lay on its side. The driver hit his head on something inside the cab, giving it a small cut.
Employee was working on the belt line, using a metal bar to change out a roller when the bar slipped hitting him in the mouth.
The time of this incident is unknown, it was discovered on the pre-shift exam. A roof fall was discovered on the 2B beltline at break #6 in an intersection. The fall measured 20' x 30' x 10' high. This beltline is abandoned, therefore, the fall will not be cleaned up. The area has been cribbed and timbered to insure the safety of surrounding areas.
The coal truck driver was leaving the mine pit area which had more coal accumulated than normal and the pit area was tight. The loader man directed the truck driver on where to back in, loaded him and then told him to pull forward. He pulled forward approx. 25 feet and turned over on the right side.
Employee was preparing to start drilling the hole for a roof bolt when he slipped and hit the rotation lever, causing him to twist his wrist.
As ee was standing in intersection of the last open crosscut, a piece of draw rock approximately 4'x6'x5" fell, striking him on the back and left side.
Injured was hanging a roof bolter cable, as the roof bolter was tramming tension came into cable causing him to cut his finger on the insulated hanger, requiring 2 stitches.
EE was helping lift a metal beam when he lost his grip, causing it to land on his hand. Bruising and swelling occurred, with no broken bones.
A rock fall was discovered during the preshift exam in the last open break in #3 entry of the 001 section. The fall measured 20' x 25' x 8'. The fall has been cleaned up and arches installed for support.
Employee was loading a truck when dust and fine pieces of rock came back on him hitting him in his right eye. His right eye lid was bruised but no apparent damage to his right eye.
EE was installing roof bolts when a portion of the rib rolled out striking him on the back. Treated and released at hospital with scratches & bruises.
THE WATER TRUCK DRIVER WAS TAKING A LOAD OF WATER TO THE STRIP MINE ROAD WHEN HE CAME DOWN A HILL TOO FAST AND RAN INTO A SMALL STOCKPILE OF GRAVEL. HE THEN BACKED UP AND CONTINUED ON TO PUT THE LOAD OF WATER ON THE ROAD.
A rock fall was discovered on 5/21/09 by a representative of the company and MSHA inspector on an old panel, 1A panel. The rock fall has been timbered off and dangered off. This is a worked out area.
Injury stated that a rib rolled off on him while scooping section.
Injury was bolting top, when a piece of rock from the rib fell off hitting him and pushing him against the bolt machine. Breaking his right arm and cutting his left arm, that required 12 stitches.
TRUCK DRIVER WAS TIGHTENING HIS WHEELS WHEN HE TURNED AND TWISTED HIS ANKLE.
Employee was unloading a box out of scoop bucket when he lost his footing and fell landing on his butt.
EE was loading truck, had stabbing pain in back. No one witnessed the accident.
As employee was finishing a belt splice, the come-along being used to pull the belt together slipped and struck him in the left shoulder.
EE was walking toward the miner to see why it wasn't running and tripped over the shuttle car cable, striking his right knee against a small piece of rock on the bottom.
