Mostrando los 17Employee called in before his shift started on 9-18-08 to complain of back pain. He was taken to our industrial medical facility where he was diagnosed with arthritis aggravated by repetitive lifting motion. Employee was previously hand stacking 50# bags of pulverized limestone.
Employee was working on quarry water pump. He reached behind the pump to access bolts and felt a pulling in his groin. He continued to work until his surgery Aug. 26, 2008 when this became a lost time accident.

He was cleaning debris from an auger trough and pealed back open area of trough and his finger slipped into hole and hurt his finger. It cut the tip of his left middle finger off, took him to the doctor and they put stitches in it and sent him back to work. Not a lost time accident
Employee was adjusting plastic top sheeting inside caged area that was properly shut down. He turned and cut himself on a piece of metal.
