Employee experienced a stroke in EE's personal vehicle in the parking lot, before EE's shift started, and emergency services were notified.

Employee sprained EE's back while cleaning spillage near primary crusher

Employee experienced a minor stroke and an aneurism during the start of the shift. EE called the acting foreman and they called 911.

Employee was working underneath a silo load out, while a tanker truck was parked. The tanker truck pulled forward without the employee knowing and ran over their foot causing a break in their pinky toe metatarsal.

employee experienced shoulder and elbow pain from overuse that was treated with first aid that helped but eventually worsened. The employee eventually requested to be seen by physicians where they received medical treatment required for reporting.

Two employees were moving a Flammable Storage Cabinet that was to be scrapped. They were utilizing a fork truck to move the empty cabinet out of the plant and onto a pallet. One of the employees were operating a fork truck and set the cabinet onto the pallet and the other then slid the cabinet backwards and crushed their finger between the cabinet and concrete wall.

EE claims injury (lower back sprain) occurred on 7/4/24. EE reported injury to company on 7/9/24. EE requested to seek treatment on 7/11. On 7/11 EE was taken to Concentra and received work restrictions of, "No shoveling, may lift up to 50lbs constantly, and May push/pull up to 100lbs constantly". On 7/11 the injury became a MSHA reportable.

Employee was walking downstairs when employee lost footing and slid down the stairs injuring left ankle.

Incident was initially a no treatment, incident became reportable on 11/1 when EE requested to seek treatment and was taken to clinic where EE was prescribed a steroid patch and physical therapy. On 8/28, EE reported to supervisor while shoveling outside plant around 300 feeder, EE stepped on the blade of a garden hoe causing the handle of the tool to strike elbow.

Employee claims they may have hit a bump in the quarry road while driving a water truck sometime on Friday. They called on Monday and claimed they were not able to move from the pain in their back.

Employee was in a man lift basket hand scaling rib wall when a large rock came loose and struck the outside of the basket. The employee's left leg struck the knee bar in the basket and lacerated the shin. Employee received 4 stitches to close wound.

Employee was hand scaling rib wall when bar slipped and EE hit their right hand against rock surface. Resulted in bruise to middle finger of right hand.

Employee was reaching into pants pocket to remove utility knife when blade opened and lacerated finger.

Employee was standing and waiting to start task and claims to have felt faint or lightheaded.

Employee was cleaning access dryer door and wiped finger along edge. Employee was not wearing gloves and suffered laceration to finger.

Employee lifted bag off conveyor belt and back of thumb grazed sprocket on row pusher of conveyor. Conveyor was not working at time.

Miner had pneumonia, suffered a coughing fit, passed out and struck finger.

Employee reached under pallet elevator to remove debris and was caught when the lower deck came down. Employee suffered lower face and neck trauma.

Employee was stacking bags of Lime and felt pain in arm

Employee bent over to move trailer tongue onto hitch and claimed pain in back.

Employee was swinging sledge hammer repeatedly to remove drill bit and then felt soreness in lower back.

Employee slipped while climbing down a ladder and fell onto shoulder.

ACCIDENT RESULTING IN CRUSHING INJURIES TO SUBCONTRACTOR AT THOMASVILLE PENNSY SUPPLY MINE.

Employee was trying to align a pin wheel and his right thumb became pinched, causing laceration.

The area was dimly lit and it was 1:00 a.m. Employee saw a shovel which had fallen over. As he went to pick it up, the toe of his boot hit the shovel blade and the handle flew up and struck him in the face.

Employee was stretch wrapping pallets of bagged material and felt a sharp pain in his lower back. Pain subsided but the next morning he had trouble bending over.

Employee claims he heard a "pop" in his shoulder area while climbing ladder to tank 3. He reported a personal injury to his neck/L shoulder on 11/2/10 and then on 11/4/10 reported the injury as having occurred at work on 10/29/10. Employee was placed on restricted duty by physician from 11/2/10 until 11/8/10.

EE was operating the bagging machine. When a bag fell, he bent over to retrieve it and accidently bumped the hit switch which dropped the kickplate on his hands still resting on the machine. His fingers were caught under the kickplate. Open wounds on fingers of both hands and Rt middle finger fractured.

EE was welding a plate on the crusher and a piece of slag fell into his left boot and burned the side of his left foot. He did not report the injury until his leg began to swell several days later due to infection.

Employee was going up the stairs to the control panel in the bagging area and twisted his right knee. No immediate pain was felt, but conditions worsened over the following days. Employee refused medical treatment until 6/3/10.

Was attempting to lift a dock plate out of truck and felt a pop in L shoulder. Was released with limited use of L arm.

Employee was clearing brush around crusher equipment and was exposed to poison ivy. The next morning he broke out in a poison rash on several parts of his body.

Pipe wrench slipped off screw and smashed L finger on side of screw shell. Finger was lacerated and required stitches.

Chest pains

Employee was clearing wet material off of the overhead plate using a shovel. He felt a pain in his right shoulder but kept working. The pain was still present as his shift drew to a close. Believed to be only a strain to the R shoulder.

Employee was hammering on a steel bar when a sliver of metal struck him in the chest.

Employee was dumping a wheelbarrow when he lost his footing and slipped. As he fill his face hit the wheelbarrow.

Employee was beating on the outside of a raw stone bin trying to dislodge the frozen material. He was using a sledge hammer and scaling bar which caused pain in his right hand. The initial report of this injury was made on 12-19-08, the injury did not become reportable until 2-5-09 when a prescription was filled.

Employee called in before his shift started on 9-18-08 to complain of back pain. He was taken to our industrial medical facility where he was diagnosed with arthritis aggravated by repetitive lifting motion. Employee was previously hand stacking 50# bags of pulverized limestone.

As the driver was returning from a local delivery, a small bee was in the cab of the truck. It had gotten between the upper part of the seat and stung him in the middle lower part of the back.

While employee was walking to cone crusher from control room he stepped into a tire rut and fell.

Employees left ankle/heel was hit by the steer tires of a forklift. The injury occurred on 2/29/08 however; it did not become recordable until 3/21/08, when prescription medicaiton was needed.

Employee was using a hand wood chisel to create a space to install hinges on a wooden door. The chisel slipped striking the employee's right thumb.

Employee was drilling a hole with an electric hand drill, when the bit pierced through the material and got caught, causing the drill to twist and pinch his left hand against a nearby elevator casing. The employees finger was injured and pinched. Employee did not report injury until 10-7-07.

Employee cannot directly relate a specific condition which contributed to the injury.

Employee was drilling out a lock that would not open. The bit passed through the lock and into his finger. This resulted in a puncture wound.

Employee was shoveling ice chunks and stone off of conveyor belts at crusher plant. Reported lower back pains on 2/20/07 to supervisor.

Employee was removing tin sheeting from dust collector bag house. The piece was secured by screws when the last screw was removed the sheet shifted unexpectedly, falling onto the EE's right forearm, causing a laceration.

Employee was sorting rocks on rock pile when his foot slipped between two rocks, twisting his ankle.

Bagging by hand 50 # of lime on wood pallets. Started physical therapy on Oct 2, 2006. Employee worked from 9/13/06 w/o medical visit. Flare-up from previous injury.

No accident occurred. Employee reported back and hip soreness while operating our hand bagging machines. This involves filling 50 lb bags and hand-stacking the bags onto a wooden pallet. This requires operators to bend and twist to complete the job steps.

Employee is experiencing pain in right hand & wrist. This is during normal working conditions. The employee has been hand bagging lime and stacking the bags on a pallet. He noticed the pain approximately one month ago around July 7. Progressive type ailment.

EE ws using an air hose w/wand extension to do a clean-out process in the pin mixer chute. The employee's hand slipped off the wand. The wand thrusted upward towards his face/body. He raised his right hand to block the wand. The wand struck his right thumb, bending it backwards towards the wrist.

Employee was using a pry bar to re-postition a mill cradle. The pry bar slipped out of cradle, thus causing the opposite end to slide through the employee's right hand. This punctured the skin between the base of the thumb and the base of the finger.

Employee was using oxy/acet torch to cut a piece of steel. When steel was almost cut through, pieces of slag were blown into his face. Some slag became embedded in right eye.

OPERATING THE TRACK LOADER OVER A BERM 3'HIGH INTO A DRAINAGE DITCH WHEN LOADER WENT OVER BERM. THE FRONT END HAD A SUDDEN DROP WHICH THREW THE OPERATOR FORWARD INTO THE CAB. OPERATOR HIT FOREHEAD ON THE REAR VIEW MIRROR MOUNTED IN THE CAB. THE ONLY DAMAGE WAS TO THE MIRROR, WHICH WAS BROKEN. EE WAS WEARING HIS SEATBELT.

EE was jumping/climbing over a 4' chain link fence. His left foot landed awkwardly, twisting the foot and breaking a bone just behind the small toes.

Climbing into the water truck cab, and felt pain in left shoulder.

Constant pulling & pushing on the emulsion eventually caused shoulder & elbow discomfort.

Attempting to scale down a large piece of rock. She reached out with bar to pry down a piece of roof and felt a pop in her back.

Twisted left foot while turning on a muck pile.

EE LIFTED A SNOW COVERED PIECE OF PLYWOOD FROM A DOUBLE STACKED FALLETS OF BAGGED LIME. APPROX. HEIGHT 6 FT. WHILE LIFTING THE PLYWOOD TWISTED HIS RIGHT WRIST. HAS HAD SLIGHT PAIN UNTIL 3/20. PAIN INCREASED, HE WENT TO EMERGENCY ROOM. EMERGENCY ROOM REFERED HIM TO FAMILY DR. INSTRUCTED TO WEAR BRACE & PRESCRIBED PREONIZONE. DID NOT LOSE ANY TIME

CONSTANT LOWER RIGHT BACK PAIN. EE WAS RUNNING GRADALL DUE TO BUMPING & ROUGH NATURE OF TASK BACK HURTS.

CLEANING GOBBED OR CHOKED UP ON SHUTTLE BELT WITH SCALING BAR - PRYING BELT TO REMOVE STONE - BELT MOVED PUSHING SCALING BAR INTO SHUTTLE BELT - CONVEYOR STEEL.

WAS MOVING A ROLL OF V-SHEETING BAYS AND SMASHED LEFT INDEX FINGER - FRACTURE.

EMPLOYEE REMOVED SIDE COVER PLATE (8" X 10") FROM THE FULLER KINYON PUMP & REACHED INSIDE WITH HIS LEFT HAND. PUMP WAS IN OPERATION & HOUSED A 6" AUGER. HE WAS LOOKING FOR THE CAUSE OF THE RIY MONS GRINDING MILL TO BE OUT OF MATERIAL. EMPLOYEE LOST 4 FINGERS ON HIS LEFT HAND.

EE WAS CLEANING UP GARBAGE LEFT BY OTHER EMPLOYEES AND SLICED FINGER ON A BROKEN LIGHT BULB. LACERATION REQUIRED 2 STITCHES - NO RESTRICTIONS.

EMPLOYEE WAS EXITING VOLVO L-330C LOADER AND SLIPPED ON SNOW AND FELL TO GROUND FROM STANDING POSITION CAUSED A CONTUSION TO HIS RIGHT HIP, NO LOST TIME BUT HAS RESTRICTIONS.

EMPLOYEE WAS SCALING AND A SMALL ROCK SPALLED FROM THE BACK AND STRUCK HIS RIGHT HAND 4TH FINGER CAUSING A LACERTION. EE WAS WEARING GLOVES. EMPLOYEE RECEIVED STITCHES ON FINGER.

EE WAS ASSISTING THE CRUSHER MAN AT THE PRIMARY CRUSHER. IT WAS CHOKED WITH OVERSIZED ROCK AND AS EE TRIED TO LOOSEN THE CHOKE WITH A PRY BAR, ASMALL ROCK SLID DOWN THE BAR AND STRUCK HIS RIGH T PINKY FINGER, FRACTURING THE TIP.

EE WAS WORKING WITH OTHER SCALING AN AREA OF 9 EAST/K SOUTH WHEN A ROCK OR ROCKS FELL FROM AN AREA THAT WAS JUST CHECKED MOMENTS BEFORE THE INCIDENT. THEY WERE WORKING THE RIB AND DID NOT SEE ANYTHING WHICH WOULD HAVE INDICATED THERE WAS UNSTABLE ROCK. THEY WERE ABOUT 15-20 FEET FROM THE GROUND WHEN THE ROCK OR ROCKS FELL STRIKING EE SHE REC'D CONTUSIONS TO HER NECK SHOULDER ARM .

EE HAD JUST STARTED HIS SHIFT WHEN HE NOTICED THAT THERE WAS SOMETHING IN HIS EYE. HE WAS NOT SURE WHAT OR HOW IT GOT IN HIS EYE. HE WAS SENT FOR EVALUATION AT HANOVERWORKS.

EE WAS ATTEMPTING TO REMOVE A ROCK THAT WAS LODGED BETWEEN THE DUAL WHEELS ON THE MACK WATER TRUCK. HE FIRST USED A PRY BAR TO TRY TO REMOVE THE ROCK AND WAS UNSUCCESSFUL. HE THEN TRIED TO BRE AK THE ROCK WITH A HAMMER. THE TIRE BLEW WHEN HE STRUCK THE ROCK WITH THE HAMMER AND IT THREW THE HAMMER OUT OF HIS HAND AND SENT HIM HURTLING APPROX 8'. BREAKING HIS THUMB, REQUIRING SURGERY
