Employee was adding coolant to a CAT 980M when the hood came down quickly and knocked the employee back into the catwalk /fender support where the hood came down and rested across employee abdomen.

Miner was lifting a belt from the ground when they fell backwards onto the ground.

Employee was changing roller frame and as they were trying to lower it to the ground, the frame twisted and caused a laceration to the employee's arm

Employee was cleaning a belt roller when it popped off and smashed EE's finger between the roller and angle iron

Employee was unloading new screens from a skid when EE lost hold and the edge of the screen fell and scraped EE's hand causing a finger laceration.

Employee was removing v-belts on vibratory screen when the pivot point that the motor was mounted on rotated down and pinched EE's left middle finger. Fracture to left middle finger.

Miner was inspecting primary crusher when EE's left hand made contact with a jagged edge on a lift leg. Miner cut left palm and received stitches.

The employee was shoveling stemming into a blast hole when they strained their lower back. This resulted in a minor muscle strain that was treated with rest and anti inflammatories.

Employee was replacing a guard on a crusher motor, the guard slid into position faster than expected and as the guard came to a stop, employee fell forward, landing on the guard which caused strain on their wrist and thumb.

Two employees were hooking up to a portable hydraulic trailer. They found it to be about an inch short of sitting down in the hitch for a secure hookup. IE and the other employee decided to push it forward to finalize the hookup. It slid off the side and back on the block. The thumb was pinched between the tongue and the block. Employee had laceration and broken tip of thumb.

Employee was putting cap back on hydraulic tank and stated a burr punctured left pinky finger while installing.

Operator was removing a fuel feed tube from a portable fuel tank when the feed tube swung and bounced off the portable tank and struck the employee on the left inner bicep. The feed tube was sharp on the end causing a laceration needing sutures.

Miner was removing a guard to do maint. The guard slipped & landed on his left foot. The miner completed his shift & worked his next 4 scheduled days without incident. When he woke up the morning of 7/22 his foot was swollen had been on his feet all day 7/21. He came into work 7/22 & ran the plant with his foot elevated & iced. Miner left early to go to urgent care & get x-ray.

During tear down operations of the portable crushing plant the employee was raising the jack stands on the jaw crusher. The middle jack stand was malfunctioning and would not rise up for transportation. While attempting to raise the leg by removing the directional valve on the hydraulic cylinder, hydraulic fluid sprayed out from the valve.

Miner was pulling a 25 pound bag off of a truck when he felt pain in his right forearm. No lost time or restricted time.

EE was maintaining water lines and cut upper left thigh. EE was treated and returned to work without lost time.

Employee was performing maintenance duties on third shift. He was wearing safety glasses, hard hat, and gloves while performing these tasks. After the shift, the employee went home and reported to management that he had something in his left eye. He decided to seek medical treatment and a small particle was removed from the left eye.
