Employee stated they were in the process of helping another employee lift a piece of steel when they felt pain in their lower back. Continued to work until 4/15/26.

Employee stated EE was throwing cable over in #7 heading with another employee. After picking up the cable, EE twisted as EE was throwing the cable over and felt a sharp pain in EE's left lower back. EE continued to work until 3/31/2026.

Employee was attempting to slide the forks together on the forklift when EE lost EE's footing causing EE to mash EE's left index finger and middle finger resulting in a fracture and lacerations. 3 sutures in EE's index finger and 2 in EE's middle finger.

Employee stated they were carrying a box of foam sealant when they stepped on some broken pieces of block, lost footing and slipped resulting in pain in left knee and right hamstring. They continued to work until 11/13/25.

Employee stated they were in the process of hooking up an air line when they stepped into a hole or onto a rock causing them to slip resulting in pain in their left knee.

Employee states EE was bending roof bolt to put up. When EE went to bend the bolt, EE felt like EE's left shoulder was grinding. Employee continued to work up until surgery on 2/2/2026.

Employee loaded roof straps onto scoop. Once loaded EE took gloves off to steps them down. EE grabbed wire to tie down straps and EE's left hand slipped back causing EE to hit hand and pinky finger which resulted in a laceration to EE's pinky finger.

Employee was attempting to remove the side guards off the run through and smashed EE's right middle finger between the guard and the frame of the run through. This resulted in a closed displaced fracture and stitches.

Employee stated EE was in the process of pushing a roof bolt into the hole. While pushing the bolt into the hole EE felt a pop in EE's right shoulder. EE continued to work until 7/16/25 when taken off work by a doctor.

Employee was pulling/dragging an old cat idler away from the pan of the miner when EE felt a burning sensation in EE's right knee. Continued to work until 6/27/25 when taken off by a doctor.

Employee states they were bending a bolt to install in the top when they felt a sharp pain in both shoulders but the right one was a lot worse.

Employee stated they were in the process of installing roof support. They bent a bolt to install into the hole, when attempting to straighten the bolt, the bolt twisted resulting in pain in their right shoulder.

Employee stated they asked their bolt partner to hand them a slate bar to pull a piece of draw rock. During transition from one employee to another, the injured employee did not grasp the slate bar and it hit them in their front tooth, chipping it. Employee had the tooth temporarily repaired on 3/17/25.

Employee stepped off back of drill with left foot first when EE heard EE's ankle pop. Upon putting EE's right foot down, EE curled it. Resulted in a closed fifth metatarsal fracture in the left foot.

Employee stated was on the third row in #6 heading in the process of installing the first bolt. EE bent the bolt to install it. As EE was straightening the bolt, it kneeled and EE felt a pain in their right shoulder. Employee continued to work until surgery on 2/21/25.

Employee stated EE was boosting a diesel scoop off. After getting it started, EE went to slide battery tray back in when EE stated EE felt a pain in EE's back. Lost time as of 8/30/24.

Employee was climbing down off dozer. The ground was further down than EE thought. EE slipped down while still holding onto the push horn with right hand. This resulted in a nondisplaced fracture of coronoid process of right ulna.

Pouring concrete flooring and employee was exposed to concrete burns on right and left leg. Mine had mucked flooring too deep and employees didn't ensure proper depth before pour

Employee stated was in the process of putting a draw bar from the MRS to the retriever when the large knuckle that connects from the retriever to the draw bar fell down pinching EE's left thumb between the draw bar and knuckle. This resulted in an open fracture.

Employee states while pulling a cable bolt off drill to be installed, EE strained EE's back.

Employee was unloading piece of pipe from a pallet when the pipe began to roll. Employee attempted to catch the pipe and the sharp end cut through employee's glove and lacerated the left middle and ring fingers. This resulted in stitches.

A side lid came off in the cut getting wedged in between the miner and rub rail. Employee was removing the pin that holds the rub rail on. When EE got it free, the rub rail sprung out striking EE's leg causing it to twist. This resulted in a fracture.

Employee was moving a mantrip and as they exited the permissible ride they let go of the steering wheel. When they let go, the steering knob on the steering wheel swung around and struck their right index finger. This resulted in a fracture.

Employee stated they were in the process of pinning belt together. While positioned on knees they pulled the belt back to line the splice up and felt a pain in lower right back. They continued to work until 1/30/23.

Employee experienced pain on 10/12 but thought it was kidney stones employee had been having troubles with. 10/21, employee notified supervisor that employee's doctor has ordered a CT scan and that it may not be kidney stones, 10/27, CT scan shows no sign of kidney stones and that it may be a slipped disc.

Emp. working on excavator wiring issue. Stepped on roller frame, placed knee on track, knee popped. felt slight pain in knee rest of day. Next AM EE's knee was sore, weight sensitive. Called supervisor, Work Partner said put ice on knee and take ibuprofen. Injury did not result in restricted work days or days away from work. Carter was notified 10/2/23 of upcoming surgery.

Employee was picking up I-beam when binding strap struck their forearm resulting in a laceration requiring stitches.

Employee going under waterline and felt EE's right knee pop.

Contractor stated that they raised the lid on scoop batteries and when they went to let them down, they wouldn't close. Another employee raised the lid back up and when EE did EE stuck EE's hand in to check latch. The lid slipped out of the other employees hand and fell on EE's left hand resulting in fractures to EE's left index, middle and ring fingers.

Employee went to the back of drill and picked up approx. 20 pans. Employee turned to walk to the front of the drill and when they turned they felt a pain in left shoulder. Employee worked up until June 3, 2021 when taken off work.

Employee was pushing conveyor chain slack out of the way when left hand slipped. A bur on the conveyor chain cut through employee's leather glove resulting in a laceration to the left wrist requiring stitches.

Employee was repairing charge gate cylinder bolts and employee's hand was caught behind cylinder as employee was maneuvering cylinder into place catching hand between cylinder and frame of the charging gate located adjacent to it. This resulted in a laceration requiring sutures.

While trying to put on a belt clamp, the tensioned belt broke loose catching employee's left thumb resulting in amputation.

As employee reached for the handle on the canopy of EE's shuttle car to exit the car, EE felt a pain in EE's right shoulder. Employee stated EE pulled self up by the handle.

Employee was removing shims in a wheel unit off of a 785 haul truck. While removing the shims, one slipped falling across EE's fingers resulting in lacerations and stitches.

Employee was loading bag dust from a pallet when EE alleged that EE felt pain in lower back.

Employee stated EE was in the process of lifting the forks up on the AL Lee mini trac for the purpose of adjusting them to make a wide base for moving materials. As EE lifted the fork, EE moved it too far up pinching EE's left index finger in the slide and top of the fork base resulting in a laceration requiring 4 stitches.

Another employee handed a piece of metal over discharge end of feeder. Employee went to get it down when it slipped out of right hand causing it to strike left arm resulting in a laceration requiring 6 stitches.

Employee was in the process of pulling the curtain out of the #3 heading and thought all pogo sticks had been removed out of the curtain. When employee pulled the curtain towards self, a loose pogo stick hit employee in the mouth which resulted in a cut to the upper lip requiring stitches.

Employee was closing lid on duster. While pushing hinged lid down, mashed right pinky finger between the lid and the frame of the duster. This resulted in a fracture to the tip of EE's finger and 5 stitches.

Employee and coworker were in the process of hanging the miner cable to the mine roof when EE felt pain in left arm and shoulder.

Employee stated as EE was in the process of moving a piece of draw rock that EE had just pulled approx. 4' on the mine floor EE felt pain in EE's lower left abdomen. EE continued to work the remainder of the shift. The rock was 39" long x 14" wide by 6" thick. Employee continued to work until 2/24/20.

Employee alleged to have an on the job injury while EE was in the process of stacking block on a pallet and felt pain in the left groin area.

EE stated EE and 2 other employees were sitting an SCSR box on a shelf. The box became stuck and the EE was pulling the box while the other 2 employees were pushing. When the box freed up, it caught EE's right hand between the box and shelf. It caught EE's right middle, ring and pinky fingers. This resulted in a laceration/avulsion to the pinky finger that required 14 stitches.

Employee had finished drilling hole for a rope bolt. As EE reached to get rope bolt it became entangled. EE jerked on the bolt to get it loose resulting in pain in EE's lower back. EE was taken off work on 12/9/2019.

Two employees went to drop the lid on a tool box, employee tried to catch a tube of grease that was falling from the box when a finger was caught between the lid and the side of the tool box. After the initial treatment for a laceration, the ER Physician referred them to be evaluated by an orthopedic surgeon. The first joint of the right index finger was removed by the surgeon.

Employee stated that EE helped supply the roof bolter, helped pull cable off reel, bolted a down cut and helped move bolter. After this, EE reported to EE's foreman that EE hurt EE's back. EE finished the shift and went to the ER the next day and was taken off work.

Employee stated that EE was scooping in the #6 entry and was attempting to pull the line curtain out from under a rock that was approx. 4' x 5' x 0-18" thick. EE states that when EE pulled the curtain out from under the rock, EE felt pain in the left shoulder.

Employee was pushing a rock into the conveyor of EE's car because it had pulled down some cables. When the weight of the rock shifted into the car, EE's finger was caught between the mine roof and the rock resulting in a hairline fracture to the tip of EE's middle finger and 9 stitches. Employee worked up until 5/13/19 when EE was taken off work.

Employee stated was drilling a rope bolt hole and was pulling the starter from the hole. The last piece of steel the employee pulled by jerking on the steel and felt a sharp pain in right shoulder.
