TOMS CREEK COMPLEX Coal

Employee experienced pain on 10/12 but thought it was kidney stones employee had been having troubles with. 10/21, employee notified supervisor that employee's doctor has ordered a CT scan and that it may not be kidney stones, 10/27, CT scan shows no sign of kidney stones and that it may be a slipped disc.

Employee was repairing charge gate cylinder bolts and employee's hand was caught behind cylinder as employee was maneuvering cylinder into place catching hand between cylinder and frame of the charging gate located adjacent to it. This resulted in a laceration requiring sutures.

Another employee handed a piece of metal over discharge end of feeder. Employee went to get it down when it slipped out of right hand causing it to strike left arm resulting in a laceration requiring 6 stitches.

Driver was dumping a load of coal when the hoist on the trailer failed. The trailer fell quickly and did some damage to the trailer. The driver was inside the tractor and was shaken up.

Received a hearing loss diagnosis on 1/16/2019 for employee. They last worked on 12/15/2017 and was termed on 6/15/2018.

EE was assisting in lowering an electric chain hoist into a train load-out hopper with a hand operated chain hoist. The chain from the hand operated hoist got caught around EE's pinky finger on left hand pulling EE's hand into the door frame causing injury to pinky finger. (EE was wearing leather gloves at the time of accident.)

Received a CWP diagnosis for EE. EE was termed on 11/19/15.

Received a hearing loss claim on 11/18/2013. Employee's last day worked was 10/11/2013 and is no longer employed with the company. Accident date of 10/11/2013 is to accommodate online reporting per MSHA (Denver) instructions.

Employee was climbing down from vibration screen where he had unhooked welding lead that was hung up. He slipped and fell onto the concrete floor resulting in a fracture to his lower right leg.

Received a CWP claim for EE on 5/14/2013.

Received a CWP claim on 5/1/2013. Employee was terminated on 3/21/12 and is no longer employed with the company. Accident date of 3/21/2012 also to accommodate online reporting per MSHA (Denver) instructions.

Employee was removing a water hose that had been used out of the road when he pulled the hose he turned and felt his left knee pop. Injury resulted in dislocation to the knee, however knee had reset itself before reaching medical attention.

Employee was moving three sheets of metal in shop when one piece slid down cutting the wrist area requiring stitches.

Employee was cleaning stock pile feed belt and got the sleeve of his coat caught in the roller pulling his arm over the roller.

Changing out skirt rubber when his hand slipped, hitting his mouth which casued a laceration requiring stitches.

Turning classifying sieve and caught his arm against the sieve and the pipe beside the sieve, causing fracture to his right wrist.

Employee was replacing blown hydraulic hose on trailer attached to Peterbilt tractor (coal truck); slipped in hydraulic oil and fell from back of tractor onto pavement (approx. 43"); bruised middle back and hip.

Employee was dragging flights away from repair site when he felt burning in his groin area. This did not become a lost time until 01/11/2008 when the employee is scheduled for surgery.

Employee was removing metal from raw coal magnet, caught left thumb between metal and magnet.

Employee was using a welding rod that was bent in order to pull the water ring from the pump housing. A sharp burr on the rod scratched the end of the employee's left thumb. The employee received medical attention on 02/16/2007 and was given intravenous antibiotics, which made this injury reportable at that time.

EMPLOYEE WAS REMOVING A DISCHARGE CHUTE BELT WHEN HIS FINGER WAS CAUGHT BETWEEN THE CHUTE & GATE, CAUSING A LACERATION THAT REQUIRED STITCHES.

Employee was walking between the plant and bathhouse when he slipped and caught himself, causing an injury to his neck. EE continued with his regular duties, lost time began 5-11-05 due to medical testing.

Employee was climbing a ladder when his foot slipped from a rung, causing a twist to the left knee. Employee continued with his regular duties, lost time began 5-20-05 due to surgery.

EE WAS PUTTING SLIP FLANGES ON 12" BASALT PIPE. THE FLANGE FELL OFF PIPE AND SMASHED EE'S FINGER. MEDICAL TREATMENT ONLY.

EMPLOYEE STATES HE SUFFERED EYE IRRITATION AFTER COMPLETING HIS NIGHT SHIFT ON 11/8/04. EYE PROGRESSIVELY GOT WORSE AFTER 3 DAYS & HE REPORTED INJURY TO SUPERVISOR ON 11-11-04. PHYSICIAN REMOVED FOREIGN OBJECT FROM RIGHT EYE.

EMPLOYEE WAS TIGHTENING BOLTS ON PIPE AND RATCHET SLIPPED. HE FELL ON A PIECE OF PIPE, LANDING ON HIS BACK ON THE FLOOR, INJURING HIS RIGHT SHOULDER. INJURY NOT REPORTED UNTIL 11/15/04.

Employee was using a rod to clean the raw coal feeder which had stopped up. When the coal broke free it pushed the rod into a piece of metal and caught his hand, causing a fracture to his thumb on the right hand.

EE WAS INSTALLING SHEETING AND WALL DUST & INSULATION FELL WHILE PUTTING UP THE LAST SECTION BECOMING LODGED IN HIS RIGHT EYE.

EE WAS CUTTING STEEL OVER HIS HEAD & DEBRIS ENTERED HIS EYES.

EE GOT HIT IN THE BACK AND SHOULDER WITH A CHAIN HOIST WHILE TAKING THE RIGGING DOWN.

EE HAS FILED OCCUPATIONAL PNEUMOCONIOSIS CLAIM WITH WEST VIRGINIA WORKERS COMPENSATION.

EMPLOYEE WAS PULLING BAR GRATING ACROSS THE FLOOR AND GOT FINGER CAUGHT BETWEEN GRATING AND FLOOR STEEL.

EE REPORTED THAT HE STRAINED HIS BACK WHEN HE LOST HIS FOOTING WHILE MOVING A BARREL CONTAINING SCRAP METAL.

EMPLOYEE WAS SHOVELING COAL OFF THE TOP VESSEL DILUGE SUMP WHEN HE FELT PAIN IN HIS LOWER ABDOMEN. HE FINISHED THE SHIFT WITH VERY LITTLE PAIN, BY THE TIME HE GOT HOME THE AREA WAS SWOLLEN. HE WAS DIAGNOSED WITH A HERNIA WHICH REQUIRED SURGICAL REPAIR.

EMPLOYEE WAS MOVING A PIECE OF 8" PIPE & JAMMED HIS FINGER BETWEEN PIPE & AN OBSCURED BOLT.

EE WAS BENDING OVER TO ADJUST WATER VALVE ON #2 COLUMN, AS HE PULLED ON THE VALVE HE FELT A STRAIN IN HIS BACK. LOST TIME DID NOT START UNTIL 07-29-03

INJURED & FELLOW EE WERE MAKING REPAIRS WHEN PRY BAR SLIPPED FROM FELLOW EE'S HAND STRIKING INJURED ABOVE THE RIGHT EYE.

INJURED EMPLOYEE WAS OPERATING DOZER PUSHING COAL INTO #3 SECONDARY FEEDER WHEN A ROCK FLEW OUT OF THE SECONDARY STACKING TUBE DOOR THAT FEEDS DIRECTLY INTO #3 FEEDER. ROCK WENT THROUGH THE D OOR GLASS AND STRUCK OPERATOR ON THE RIGHT SHIN JUST ABOVE THE EMPLOYEE'S BOOT CUTTING HIS RIGHT LEG REQUIRING 3 SUTURES.

EE WAS ATTEMPTING TO CLEAR A SMALL ROCK FROM THEOUTSIDE OF THE REFUSE CHUTE WITH A FLAT BAR WHENTHE BAR BOUNCED BACK HITTING HIS FRONT TOOTH.
