EE was helping flip a stuck Rip Rap flop gate, once the gate freed up it fell onto EE's upper left leg. EE initially said was ok but came to work the next morning with leg swollen. EE was taken to urgent care and diagnosed with a severe bruise and given restrictions against prolonged standing, climbing, etc. thus making this accident reportable.

EE was trying to diagnose an issue with the detach pins on a skid steer bucket. EE had the bucket raised 24" and when the pins released the bucket fell on EE's left foot fracturing the 3rd foot bone, making this accident reportable.

EE was putting v-belts on a crusher motor. As ee was rolling the sheave over, ee left hand glove got caught and pulled ee left wrist into the sheave and v-belt. The EE felt a lot of pain, went to the doctor, but had no fractured bones. Doctor restricted duty for 4 working days, thus making the accident reportable.

The EE was picking up a water hose at the plant to use to wash down. When ee picked it up, the hose clamp on the hose had a sharp edge. EE pulled the hose clamp across right palm, and the clamp lacerated palm. EE went to the doctor and received two stitches.

EE was changing bit on Gill Drill. The bit basket was stuck to the bit. When EE unscrewed bit, the bit and basket pinched left little finger. EE was wearing gloves, but the pinch lacerated left little finger requiring 3 stitches.

The ee was pulling a drop cord off of the top of the screen after performing maintenance. When the end of the drop cord came over the edge of the screen, it hit the ee in the mouth, cutting his lip and chipping a tooth. The cut was minor, but the tooth had to be repaired by placing a cap on it.

WAS STEAM CLEANING A35 HAUL TRUCK AND SAID HE CAUGHT HOSE ON TIRE AND MADE THE SPRAYER GO ACROSSHIS LEFT FOOT AND BURN IT.

THE EMPLOYEE WAS PUSHING A BARREL OF GREASE INTO A RACK ON THE BED OF A FLAT BED TRUCK. HIS FOOT SLIPPED, CAUSING HIM TO FALL TO THE GROUND. THE BARREL FELL ON HIS RIGHT SIDE, BRUISING HIS ELB OW.

WHILE TRANSPORTING A HYDRAULIC CYLINDER ONTO THEDREDGE, THE EMPLOYEE SLIPPED AND DROPPED CYLINDER ON HIS LEFT FINGERS. CYLINDER WEIGHED APPROX. 75 POUNDS, CAUSING SMALL CUT ON MIDDLE FINGER RE QUIRING FOUR STITCHES.

EMPLOYEE WAS PICKING UP CRUSHER PART IN BED OF PICK-UP TRUCK THAT HAD JUST BEEN PICKED UP AT MACHINE SHOP.PART HAD SHARP EDGE THAT CUT EMPLOYEE ON PALM OF HAND WHICH REQUIRED 4 STITCHES.

INJURED WAS PLACING GUARDS ON MODEL 41 NOTHWEST DRAGLINE.SAFETY GUARD.ON E GUARD HUNG ON 2ND GUARD AND HE TRIED TO FREE UP GUARD AND PINCHED LEFT THUMB BETWEEN BOTH GUARDSREQUIRED 3 STITCHES I N RIGHT SIDE OF LEFT THUMB.

EMPLOYEE ALLEGES THAT HE WAS HELPING REMOVE A GUARD AROUND THE CRUSHER WHEN ANOTHER EMPLOYEE DROPPED ONE END, CAUSING THE END HE WAS HOLDING TO STRIKE HIM IN THE CHIN. NO FIRST AID WAS RENDER ED NOR WAS IT REPORTED.

EMPLOYEE WAS REMVOING THE STEERING WHEEL FROM THE FUEL TRUCK TO MAKE REPAIRS WITH HIS FEET BARRED UPON THE FLOORBOARD HE PULLED THE WHEEL WITH HIS ARMS AND SHOULDERS IN A ROCKING JERKING MOTIO N, HE PULLED REAL HARD AND FELT A SHARP PAIN.

EMPL ASSISTING REMOVAING COMPRESSED GAS CYLINDER RACK AND DISMANTLEMENT OF TRUCK SERVICE BED.WELDER WAS CUTTING RACK LOOSE AS I.NAME PULLED UPPER SOLID STEEL ROD -HOSE RACK-PORTION.AS RACK BRO KE LOOSE THE SUDDEN UNCONTROLLED FORCE CAUSED EMPL TO FALL BACK AND RIGHT HAND WAS SEVERLY PINCHED AND BRUISED BETWEEN RACK AND CORNER OF SERVICE BED.
